On 03/12/2026, Licensing Program Analyst (LPA) V. Gorban arrived unannounced delivering findings to complaint investigation. LPA explained the purpose of the visit to facility administrator Angelica Atriano and was allowed entry. During the course of the investigation, LPA conducted a facility tour, conducted interviews, and reviewed records. Allegation:Staff left residents unattended in the facility. Based on interviewes and records review staff preset at the facility during all three shifts. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is unsubstantiated. On 03/12/2026, Licensing Program Analyst(LPA) V Gorban arrived unannounced to conduct a Required Annual Inspection. LPA met with administrator Felicidad Ramos Kankelborg and announced the purpose of the inspection. During the inspection, LPA toured the facility inside and outside. All passageways and exits were clear and free from obstruction. Fires extinguishers were accessible and recently serviced. Smoke and carbon monoxide detectors were present and functioning properly. LPA observed a sufficient supply of foodstuffs which were properly stored and labeled. The outdoor areas were free from hazard and provided adequate seating for all residents in a covered area. Resident medications were centrally stored in a locked cabinet, and medications appeared to be administered properly. Fire extinguisher service date recorded on 12/01/2025. Last disaster drill conducted verbally, AD does not recalls date. Common areas were clean, odor free, and adequately lit. Resident bedrooms and bathrooms were clean, odor free, and provided the minimum required furnishings. Bathrooms were clean and odor free. Non-skid mats and secure grab bars were observed in showers, and all fixtures were functioning properly. Hot water was within required temperature range. At 3:00 PM during record review, AD provided incomplete emergency disaster plan. See attached LIC809D for deficiency cited in accordance with Health and Safety Code, Section 1569.695(a). Facility Administrator agreed to complete an Emergency Disaster Plan LIC610E for review by the department. Appeal rights were provided to the Administrator. Licensing Program Analyst (LPA) Sarah Hurt conducted an unannounced facility visit to conduct a Case Management. LPA met with facility Licensee Felicidad Kankelborg, and explained the purpose of today's visit. LPA obtained additional information about the addition to the facility and the construction taking place at the facility. LPA discussed details with the Licensee about the neighborhood complaint policy. Additional information/documentation was requested by Monday November 3, 2025. Licensing Program Analyst (LPA) Sarah Hurt conducted an unannounced facility visit to investigate the allegations listed above. LPA met with facility Licensee Felicidad Ramos Kankelborg, and explained the purpose of today's visit. Regarding the allegation Staff yells at resident. LPA interviewed 2 of 5 facility residents who both stated they are treated well by facility staff. Resident 1 stated they are happy at the facility, and none of the staff has ever yelled at them. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is unsubstantiated. Both facility residents interviewed stated the facility staff is nice and does not throw things at any residents. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is unsubstantiated. Regarding the allegation Staff forcefully restrains resident. LPA interviewed 2 out of 5 residents. Both residents stated the facility staff is nice to the residents. Resident 2 stated they have never witnessed anyone restraining, hitting, or touching any them or any other residents. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is unsubstantiated. Licensing Program Analyst (LPA) Sarah Hurt arrived unannounced to conduct a Case Management visit on 10/02/2025. LPA met with facility Licensee, Felicidad Ramos Kankelborg, and stated the purpose of the visit. LPA Hurt observed the facility front door has a baby safe door knob cover. The door knob cover is placed there to prevent dementia residents with wandering behaviors from leaving the facility. LPA observed the facility has residents with dementia diagnosis but does not have required front door alarms. On 01/21/2025, Licensing Program Analyst (LPA) V. Gorban arrived unannounced at the above facility to conduct an Annual Inspection. LPA introduced self, stated the purpose of the visit, and was granted entry to the facility by staff Maria Rosy. LPA toured the facility with facility administrator Felicidad Ramos Kankelborg, who arrived shortly after being notified of Licensing visit. The facility was observed to be at a comfortable temperature, of 72 degrees F. Facility is free of debris, in good repair, and no passageway obstructions or fire hazards were observed. Common areas were properly furnished and well-lit throughout. During the visit six residents were present. Department phone number and infection prevention information signs were posted thought the facility. Inspecting kitchen LPA observed the required 7-day supply of non-perishable food and 2- day supply of fresh perishables to be properly stored. An emergency disaster supply was observed. A fire extinguisher was observed with service date of 11/13/2024. Last fire drill was recorded on 12/02/2024. Private residents’ bedroom observed to be at comfortable temperatures. The bathroom’s water temperature was tested and recorded reading of 107 degrees F. Medications records storage observed to be locked in a cabinet in a hallway. Cleaning supplies were observed to be in a locked cabinet in the storage it the laundry room. An outdoor seating area was observed operational for residents in care. Facility phone number observed and tested as operational 831-673-1434. Report continues on attached LIC809-C LPA reviewed staff and clients’ files. No deficiencies were observed and cited during this visit. On 1/20/24, Licensing Program Analyst (LPA) M. Medina conducted an unannounced Case Management visit. LPA introduced self, stated purpose of visit, and allowed entrance by Administrator, Felicidad Ramos Kankelborg. LPA Medina conducted Case Management visit to verify if Staff 1 (S1) is currently working in the facility. Administrator advised an exclusion was ordered and issued by the Department. LPA verified with Administrator that S1 has not worked in facility since December 2023. Administrator disassociated S1 from facility personnel report during case management visit. Licensing Program Analyst's (LPA) Manuel Monter and Simi Rai conducted an unannounced case management visit. LPA's met with Administrator (ADM) Felicidad Ramos Kankelborg and explained the purpose of the visit was to deliver a letter of exclusion for facility staff S1. LPA's observed 2 staff and 4 residents during the visit. LPA's Manuel Monter & Simi Rai hand delivered exclusion letter to ADM Felicidad Ramos Kankelborg. LPA's Manuel Monter & Simi Rai hand delivered the exclusion letter to S1. LPA's explained to ADM that S1 can no longer work at the facility at this time. LPA's observed S1 vacate the facility. No deficiencies were cited at this time as per California Code of Regulations Title 22.
