On 07/02/2026, Licensing Program Analyst (LPA) V Gorban arrived unannounced to conduct Required Annual Inspection. LPA met with administrator Charles Park, certification number 7000682740 and expiration date 11/16/2027. LPA conducted facility tour inside and out of facility with administrator. Residents observed at the facility in common area after lunch time. The facility was observed to be at a comfortable temperature of 76 degrees, clean, in good repair, and no passageway obstructions or fire hazards observed. Fire extinguisher was observed serviced on 07/29/2025 and with in compliance. Last fire drill recorded in June 2025. Dining area and Kitchen were toured. An adequate supply of perishable and non-perishable food was observed to be properly stored in freezer, refrigerator, and pantry. Food is delivered by facility staff once a week from preferable produce distributors. The facility is comprised of one level building with twenty five residents’ private rooms on the main level with dining area, kitchen, laundry room and storage. The census during the visit was 13 assisted living residents. LPA toured resident bedrooms. Residents' rooms were observed adequately furnished with bed, dresser, and adequate lighting. Hot water temperature tested at 112 degrees F. LPA observed securely fastened grab bar and non-skid mat in shower area. Medications were stored in a locked medication room. The first Aid Kit was observed with all required items. LPA toured the laundry room and observed chemicals were stored and locked in for staff use only. The facility courtyard was toured and observed to be free from debris. There was outdoor seating available for the residents. LPA requested and reviewed facility, staff and resident files. No deficiency cited during this visit, Department will conduct through records review and may return with additional case management. Allegation investigated: Staff are mismanaging resident's medication. Staff are mismanaging residents' records Facility is not sufficiently staff to meet the needs of the residents in care. Staff did not follow infection control requirements.

On 04/16/2026 Licensing Program Analyst (LPA) Gorban unannounced visited the facility to commence complaint investigation. LPA introduced self and met by Administrator; LPA stated purpose of the visit and was allowed entry. During the complaint investigation LPA toured the facility conducting health and safety checks, reviewed records, and interviews. Allegations: Based on interviews and records reviews, medications documented after each administration. Medication records appear up to date. Regarding residents' records, files updated with recent medical assessments and needs and services files are up to date. Regarding staffing, facility has substantial amount of staff to provide care and supervision. Regarding infection control, LPA reviewed facility infection control binder and interviewed administrator regarding infection control practice. Although the allegation may have happened or are valid, there are not a preponderance of evidence to prove the alleged violations did occur, therefore the allegations are Unsubstantiated. On 01/20/2026, Licensing Program Analyst (LPA) V Gorban conducted subsequent complaint inspection. LPA met with administrator. The purpose of this visit is to deliver the findings of the investigation completed by the Department. During the visit, LPA conducted health and safety tour. Allegation: Staff did not respond to a resident's pull cord in a timely manner. Based on staff and residents interviews no concerns regarding call light response time. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is Unsubstantiated. LPA met with administrator Charles Park, certification number 7000682740 and expiration date 11/19/2025. LPA conducted facility tour inside and out of facility with administrator. Residents observed at the facility during lunch time. The facility was observed to be at a comfortable temperature of 74 degrees, clean, in good repair, and no passageway obstructions or fire hazards observed. Fire extinguisher was observed with in compliance. Last fire drill recorded in June 2025. Dining area and Kitchen were toured. An adequate supply of perishable and non-perishable food was observed to be properly stored in freezer, refrigerator, and pantry. Food is delivered by facility staff from Costco once a week. The facility is comprised of one level building with twenty five residents’ private rooms on the main level with dining area, kitchen, laundry room and storage. On 7/2/2024, Licensing Program Analyst's (LPA's) D. Ayers and M. Vega arrived unannounced at the facility to conduct a Required Annual Inspection. LPA's met with Licensee/Administrator Charles Park and announced the purpose of the inspection. LPA's toured the facility inside and outside. Pathways and exits were clear and free from obstruction. Smoke-detectors and carbon-monoxide detectors were present and operational. Facility was clean and odor free. LPA's observed sufficient amount of perishable and non-perishable foodstuffs, and facility kitchen was observed to be clean. Common areas were clean, adequately furnished, and adequately lit. Outdoor areas were free from hazards and provided adequate seating and area for residents. The facility was equipped with a back-up generator and three wells which provide fresh water. Resident bedrooms were clean and had required minimum furnishings. Resident bathrooms were clean, odor free, and water temperature was within required temperature range. Sharp items were secured in a locked drawer. Medications were secured in locked cabinets within a medication room. Medications appeared to be administered properly. LPA reviewed facility plan of operations and emergency disaster plan. LPA reviewed a sample of resident files and staff files and training records. No deficiencies were cited during the inspection.
