Allegation investigated: Resident sustained multiple unexplained bruises. Staff did not prevent resident from sustaining a wound.

On 03/05/26, Licensing Program Analyst (LPA) Elvira Gonzalez conducted an unannounced complaint visit to investigate the above mentioned allegations and deliver findings. LPA met with Administrator, Felicia Huff, and the purpose of the visit was explained. LPA was granted entry to the facility. The investigation consisted of the following: On 03/05/26, LPA requested the staff roster, and resident roster, and conducted interviews with staff #1-#2 (S1-S2) and witness #1 (W1). The investigation revealed the following: For the allegations: "Resident sustained multiple unexplained bruises", and "Staff did not prevent resident from sustaining a wound". On 03/05/46, LPA Gonzalez conducted interviews with S1-S2. Of those interviewed, 2 out of 2 staff could not corroborate with the allegation. Unfounded An interview with S1 revealed that there is no resident currently residing or has resided at the facility with the name listed in this complaint. Based on records reviewed, and interviews conducted, the above allegation is found to be unfounded, meaning that the allegation was false, could not have happened and/or is without a reasonable basis. On 12/02/25, Licensing Program Analyst (LPA) Elvira Gonzalez conducted an unannounced annual inspection visit. LPA was met by the Administrator, Felicia Huff, and the purpose of the visit was explained. LPA was granted access to the facility. The facility is licensed to operate for (4) non-ambulatory adults age 60 and over and has a hospice waiver for (2). Currently, there are (4) residents in the facility. The facility is a single story structure located in a residential neighborhood. It consists of the following: (2) bedrooms, (1) bathrooms, living room, dining area, kitchen, and laundry area, office area, outside shaded area and a detached garage used for storage. LPA Gonzalez and Felicia Huff toured the physical plant. There were no bodies of water or obstructions on the premises. All bedrooms were inspected. Beds and bedding supplies were in adequate condition, lighting was provided, and storage for the residents personal belongings was observed. Bed linens, comforters, and bath towels were stocked during the visit. The bathrooms were found to be within Title 22 regulation. Water temperature properly measured between 105F-120F. A comfortable temperature was maintained in the facility. LPA observed the facility to be furnished at the time of the visit. Storage areas for personal hygiene were stored and not accessible to residents. The kitchen was inspected, and sufficient perishable and non-perishable food was maintained adequately. Two fire extinguishers were fully charged. Smoke detectors and carbon monoxide were operable. Continued on LIC9099-C The facility has a landline telephone on-site in working condition. Toxins and sharps were locked and inaccessible to residents. Medications were centrally stored and properly locked. First aid kit was checked and in order with manual. Outside grounds were toured and no bodies of water were observed. Exits/ Walkways around the home were free of debris and hazards. LPA conducted a review of 3 staff records and observed them to be complete and in order. Additionally, LPA conducted a review of 4 resident service records and observed them to be complete and in order. A review of Medication Administration Records was maintained in order and accurate. During the visit, LPA observed the facility's infection control practices. LPA observed screening protocols for visitors, staff, and residents. LPA observed sanitizing stations in common areas and restrooms. The facility has a 30-day supply of Personal Protective Equipment (PPE) available. All mandated inspection control posters were posted. The facility has an approved CCLD Mitigation Plan. The last fire drill was conducted in 12/01/25. No deficiencies were cited at the time of this visit. On November 14, 2024, Licensing Program Analyst (LPA) Deborah Lee conducted an unannounced required annual visit using the CARE Inspection Tools. LPAs met with Brandi Brooks, Licensee and explained the purpose of this visit. Subsequently, Administrator Felicia Huff assisted with the visit. The facility is licensed for (4) 60-year-old or older adults and 4 may be non-1 may be bedridden. Currently, are 4 residents in the facility. Structure: The home is a two (2) bedroom, one (1) bathroom, one story home with detached garage situated in a residential neighborhood. The home includes a living, dining, kitchen, and laundry area. Living room fireplace was completely covered. The living area included sectional seating. The kitchen has a refrigerator and stove. Passageways, walkways, and steps inside and out are free from obstructions. Physical Plant LPA and Licensee Brandi Brooks toured the facility inside and outside. LPA observed There were no bodies of water on the premises. All rooms were inspected. Beds and bedding supplies were in operational condition, lighting was provided, and storage for the resident's personal belongings was observed. LPA observed that facility had required postings: Facility license, personal rights, ombudsman information poster, facility sketch, exit signs, infectious disease postings, Administrator certificates, an emergency disaster plan. page 1 of 3 Bedrooms LPA inspected both bedrooms they were observed to have the required furniture including beds, dressers, night stands with lamps, chairs, and ample storage space for personal belongings. All bedrooms were observed to be clean, in good repair, and have ample lighting. Bathrooms LPA inspected the facility bathroom. In the resident’s bathroom the toilet, faucets, and shower were fully operational. All safety handrails were securely fastened. LPA observed the showers to be clean and free of mold or mildew. The shower had a nonskid material in bottom and shower chair. Resident’s toiletries and incontinent supplies are secured in hall closet. The water temperature measured 119.2 degrees Fahrenheit. The bathroom was observed to be clean, in good repair and within Title 22 regulations. Linens & Hygiene LPA observed all beds to have the required linens including mattress cover, fitted sheets, blanket, comforter, and pillow. LPA observed an ample supply of linens, towels, and blankets in hall closet. Kitchen LPA inspected the kitchen and observed all appliances to be in good working repair, including stove/oven, microwave, dishwasher, refrigerator. LPA observed an ample supply of cutlery, pots, pans, and bowls to be in good repair. LPA observed knives and additional sharps to be secured in locked drawer in the kitchen and are inaccessible to residents. LPA observed a 3-day supply of perishable foods and a 7-day supply of nonperishable foods Safety LPA observed and tested smoke/carbon monoxide detectors to be fully operable. Page 2 of 3 LPA observed (2 ) fully charged fire extinguishers that was last serviced on 5/30/24. The last emergency drill was conducted on 11/1/24 . LPA inspected the First Aid kit and found it contained an ample supply of required items: Scissors, tweezers, gauze, disinfectant wipes, band aids. LPA observed all exits to be clear and easily accessible. All toxins locked and inaccessible to residents in care. Medications LPA observed all centrally stored medications in their original packaging and are secured in a locked cabinet that is inaccessible to Residents in care Files LPA reviewed ( 4 ) resident files and found that (4) out of (4) contained all the necessary documentation. LPA reviewed (4) staff files and found that (4) out of (4) contained the required documentation, certification, and training. Liability Insurance expires on 06/20/25. Infection Control During the visit, LPA observed the facility’s infection control practices. LPA observed a sanitizing station at the facility entrance. PPE supplies are readily available to staff. Outside area: During visit LPA observed the outside grounds (front and back) to be free of clutter, debris, and passage ways were free of obstruction. The status of annual fee was discussed with Administrator Brandi Brooks. There were no deficiencies cited during today’s visit. On 10/02/2023, Licensing Program Analyst (LPA) Elvira Gonzalez conducted an announced pre-licensing visit to this home. LPA was greeted by applicant Brandi Brooks and explained the purpose of today’s visit. LPA was given access to the facility. An application was submitted to CCLD on 07/01/2023 for a change of facility type from Adult Residential Facility to Residential Care Facility for the Elderly. The applicant requested a capacity of four (4) individuals, of which maybe four (4) non-ambulatory, and one (0) bedridden. Structure: The home is a two (2) bedroom, one (1) bathroom, one story home with detached garage situated in a residential neighborhood. The home includes a living, dining, kitchen, and laundry area. Living room fireplace was completely covered. The living area included sectional seating. The kitchen has a refrigerator and stove. Passageways, walkways, and steps inside and out are free from obstructions. Bedrooms Residents: The facility has two (2) bedrooms for residents, which had two beds, two chairs and two bedside tables. Both bedrooms are equipped with ceiling lights and dressers, which comply with the requirement of 8 cubic feet of space. All rooms had closets for ample storage. Bathrooms: The home has one (1) bathroom. Bathroom has a working toilet, washbasin, and shower with grab bars and non-skid mats. Continued on LIC809-C Emergency Phone Numbers, Exit Plan & Menu: Emergency phone numbers. The exit plan and menu are posted and readily available for review throughout the home. There are two (2) fire extinguishers located in the kitchen and hallway mounted on the wall. Facility has a working telephone. Emergency supplies and Personal Protective Equipment supplies are stored in the garage. The applicant has an approved Infection Control Plan on file. Food Service: Dishes, cups, and flatware are stored in the kitchen cabinets, inspected, and in good repair. Knives, cutlery, and other sharp kitchen utensils are stored in a locked kitchen drawer. Food supply is adequately stored and consist of (2) day supply of perishables and a (7) day supply of non-perishables. Smoke Detectors: Smoke and carbon monoxide detectors throughout the interior space. Smoke detectors in two (2) bedrooms and hallways. Carbon monoxide detector is in hallway. Toxins: All toxins are locked and stored in hall closet, with a locked space under kitchen sink available as well. Appliances: Kitchen appliances found to be within title 22 requirements. Home is equipped with central heaters and air conditioning systems. Water Temperature: The water temperature was found to be withing title 22 regulations throughout the kitchen and bathrooms. Medications, First-Aid Kit & Book: A first aid kit contained thermometer, tweezers, scissors, antiseptic, bandages, gauze, and current first aid manual locked and inaccessible to residents. The resident's medications will be stored in a cabinet locked in the laundry room and inaccessible to residents. Resident & Staff Files: Records of staff and residents will be stored in cabinets off in the laundry room area. Pool/Jacuzzi & Pets: There are no pets, jacuzzi, or pool on premises. A Fire Clearance inspection was conducted on 05/30/2023 and approved for a capacity for four (4) non-ambulatory and zero (0) for bedridden. Component III Orientation: Component III was completed with the Applicant during the Pre-Licensing visit. Information was provided about how to operate the facility within substantial compliance. When the applicant was asked if she understood Title 22 Regulations she responded in the affirmative. An exit interview was conducted, and a copy of this report will be emailed to aplicant Brandi Brooks. LPA Gonzalez will submit a copy of this facility evaluation report to the Central Applications Unit (CAU) for review.
