Type of visit: Required - 1 Year
At 10:00am, Licensing Program Analyst (LPA), Angela Panushkina conducted an unannounced annual inspection at the facility mentioned above. LPA was greeted by Staff #1 (S1), who granted access to the facility. Administrator arrived shortly after, and LPA explained the reason for the visit. Facility is licensed for capacity of six (6) residents of which five (5) are non-ambulatory and (1) bedridden, in room #1. LPA observed that currently there are five (5) residents in care, two (2) of which are under hospice care. At approximately 10:00am, LPA toured the kitchen area and observed enough supplies of staple non-perishable for minimum 1 week and perishable for 2 days at the facility. All knives and sharps are observed to be locked in a kitchen drawer and inaccessible to residents. There is a fire extinguisher in the kitchen, and it was last serviced on 08/11/2026. At approximately 10:20am, LPA observed medications are centrally stored and locked in six (6) drawer double cabinet, located by the kitchen area and inaccessible to residents in care. LPA also observed a First Aid Kit complete with the required items as per Title 22 Regulations. There are four (4) bedrooms designated for residents’ use and have sufficient lighting. All bedrooms are properly furnished, clean and have appropriate bedding and linens. Auditory alarms were tested and observed to be operational. LPA observed that R5 (in room #4) had a hospital bed with ½ bed rail. LPA reviewed R5's facility file and did not observe a written doctor’s order. Continue on LIC809-C At 10:30am LPA observed all bathrooms are clean and in good repair. Properly supplied with toilet paper, soap and paper towels. The hot water temperature measured at 116.2°F. LPA observed appropriate grab bar and non-skid mat. All trash cans in bathrooms had fitted lids to protect them from cross contamination. Laundry area is located in an attached garage and kept locked and inaccessible to residents. The facility maintains a comfortable temperature at 70°F. The living room and dining area appeared clean and were properly furnished. Smoke detectors located throughout the facility and the carbon monoxide was observed in a hallway, and at 10:45am they were tested and observed to be operational. At approximately 11:00am, LPA toured the outside area of the facility. LPA observed a clean covered patio and backyard furniture to accommodate the six (6) residents. LPA discussed the importance of maintaining care and supervision to meet the needs of residents. At 11:30am, LPA reviewed records of five (5) residents and two (2) staff. Resident and staff records appeared to be complete and updated. LPA collected Certificate of Liability Insurance and LIC500. Moreover, during today’s visit LPA checked the California Secretary of State website and observed that the facility's licensee “AM3 Corporation” had been suspended. LPA conducted an interview with the Administrator and was informed that they will have it in good standing by the end of March 2026. Deficiencies issued on LIC809-D. Failure to correct the deficiencies may result in civil penalties. Feb 27, 2025Facility evaluation reportReport on file
Type of visit: Required - 1 Year
At 1:10pm Licensing Program Analysts (LPAs), Angela Panushkina and Perchui Milena Khurshudyan, conducted an unannounced annual inspection at the facility mentioned above. LPAs were greeted by Staff #1 (S1), who granted access to the facility. Administrator arrived shortly after, and LPAs explained the reason for the visit. At approximately, 1:15pm LPA Panushkina and S1 conducted a physical tour the following observed: Common Areas: The facility maintains a comfortable temperature at 74°F. The living room and dining area appeared clean and were properly furnished. No obstructions and or tripping hazards throughout the facility. Kitchen: At approximately, 1:20pm LPA toured the kitchen area and observed enough supplies of staple non-perishable for minimum 1 week and perishable for 2 days at the facility. All knives and sharps are observed to be locked in a kitchen cabinet and inaccessible to residents. There is a fire extinguisher was last serviced on 07/24/2024. Medications: At approximately, 1:25pm LPA observed medications are centrally stored and locked in the cabinet, by the kitchen area and inaccessible to residents in care. Bedrooms: There are four (4) bedrooms designated for residents use and have sufficient lighting. All bedrooms are properly furnished, clean and have appropriate bedding and linens. Auditory alarms were tested and observed to be operational. Facility has awake staff. Bathrooms: At 1:30pm LPA observed all bathrooms are clean and in good repair. Properly supplied with toilet papers, soap and paper towels. The hot water temperature measured at 115.8°F. LPA observed appropriate grab bar and had non-skid mat. All trash cans in bathrooms had fitted lids to protect from cross contamination. Continue on LIC809-C Smoke detectors/carbon monoxide. Dual smoke and carbon monoxide detectors were located throughout the facility, and at 1:35pm they were tested and observed to be operational. Outside areas: At approximately, 1:40pm LPA toured the outside area of the facility and observed appropriate outdoor furniture, with a covered shaded area for residents LPA discussed the importance of maintaining the care and supervision to meet the needs of residents. There are no bodies of water. Between 1:45pm to 2:40pm, LPAs reviewed records of four (4) residents and three (3) staff. Resident and staff records appeared to be complete and updated. Administrative: Annual fee is current. All required signs are posted. LPA collected LIC500, Certificate of Liability Insurance and Administrator Certificate. No citations issued during this visit. Oct 23, 2024Complaint investigation reportSubstantiated
Allegation investigated: Staff did not respond to resident in a timely manner.
At 09:55am, Licensing Program Analysts (LPAs) Angela Panushkina and Perchui Milena Khurshudyan conducted an unannounced initial complaint visit at this facility to investigate the above allegations. LPAs met with the Administrator and explained the reason for the visit. During course of the investigation, interviews and record review were made. At 10:00am, LPAs requested resident and staff roster. At 10:05am, LPAs requested copies of pertinent information which include, but not limited to Admission Agreement, Physician’s Report, Appraisal Needs and Services Plan, Client/Resident Personal Property and Valuables, relevant to the investigation. At approximately 10:10am, LPA conducted a physical plant tour, to ensure health and safety of the residents are protected and physical plant is in compliance with Title 22 Regulations. Between 10:15am – 12:30pm, LPAs interviewed the Administrator, two (2) staff, and four (4) out of five (5) residents. Continue on LIC9099-C Substantiated Allegation: Staff did not respond to resident in a timely manner It was alleged that on 9/10/2024, at about 2:30 or 3:00 AM, R1 could not breath and called the staff and no one responded for about 15-minutes. To investigate this allegation, LPAs conducted an interview with the Administrator and were informed that when an incident occurs with the resident, the staff must immediately call 911 and then notify the Administrator. Interview with S1 revealed that R1 was periodically checked on a day of an incident. LPAs were also informed that last time R1 was checked on was at 12:30am and S1 started a laundry, which is located in the garage. S1 confirmed that due to the washer and dryer noise, S1 did not hear R1 calling for help (around 2:30-3:00am), therefore, S1 provided help 15-minutes later. S1 also confirmed that the Administrator was immediately contacted and the Administrator instructed S1 to hang up and call 911. Based on interviews and information gathered, during today's visit, this allegation is Substantiated. Deficiency cited on LIC9099-D Exit interview conducted and copy of this report signed and delivered. Allegation: Staff did not meet the needs of resident in care. It was alleged that reported that a couple of days after returning from the hospital R1 was in bed and was covered in ants and the staff denied to bath R1. To investigate this allegation, LPAs conducted an interview with the Administrator and two (2) staff members and all parties interviewed informed LPAs that R1 was never observed to be covered in ants. LPAs were also informed that R1 often refused showers. Moreover, interview with four (4) out of five (5) residents, expressed no concerns regarding this allegation. Therefore, based on interviews and information gathered, this allegation is deemed, Unsubstantiated. Allegation: Staff are not providing activities for residents. It was alleged that R1 and the other residents are not provided with any activities. To investigate this allegation, LPAs conducted an interview with the Administrator and two (2) staff members and all parties interviewed informed LPAs that every morning, during the breakfast time, the staff encourage residents to participate in a different activities, such as outside walks, puzzles, bingo, karaoke, books and other games. LPAs were informed that most residents chose not to participate in a group activities. Moreover, Administrator informed LPAs that all residents were enrolled in a Day Program. However, two (2) residents refused the services and preferred to stay at the facility. In addition, interviews conducted with residents confirmed that they rather do individual activities; like read a book or watch TV, than participate in group activities. Therefore, based on the information obtained, the allegation is deemed Unsubstantiated at this time. Allegation: Staff did not safeguard resident's personal belongings. It was alleged that on 9/16/2024, R1 was removed from this facility and some of his/her clothes were missing. To investigate this allegation, LPAs conducted an interview with the Administrator and were informed that R1 was admitted to this facility on 09/08/24 and relocated on 09/16/24. The Administrator informed LPAs that although no "in advance" notification verbal/written was provided regarding R1's relocation, R1's property and valuables were inventoried and signed by R1's family member upon departure. Moreover, four (4) out of five (5) residents interviewed expressed no concerns regarding this allegation. Based on interviews and record reviews, this allegation is deemed Unsubstantiated at this time. Type of visit: Case Management - Deficiencies
Licensing Program Analysts (LPAs) Angela Panushkina and Perchui Milena Khurshudyan conducted unannounced visit to this facility in conjunction with a complaint control #31-AS-20241017160918. LPAs met with the Administrator and explained the reason for the visit. During the visit, LPAs was informed that on 09/10/24 R1 had a medical incident and the facility initiated 911 call. However, no incident report was submitted to the Community Care Licensing Department (CCLD) in a timely manner. LPA reviewed all incident reports on a system and did not observe an Incident Report regarding R1. In addition, the Administrator admitted that no incident was submitted to the Regional Office (RO). Based on Title 22 Regulation: a written Unusual Incident / Injury Report shall be submitted to CCLD within seven (7) days of occurrence. LPAs informed the Administrator that all staff members are mandated reporters and they are all responsible for reporting. LPA informed the Administrator to submit an incident report that occurred on: 09/10/24 (one incident) Per the California Code of Regulations, Title 22, Division 6, Chapter 8, deficiencies are cited and noted on LIC 809D. Type of visit: Required - 1 Year
Licensing Program Analyst (LPA), Lorena Casillas, met with Myline Olivas for an unannounced one (1) year Required visit for this facility. At 10:15 am LPA arrived at the facility and was granted access by a Caregiver who then called the Administrator. The administrator arrived shortly after, and LPA explained the reason for the visit. Fire clearance was granted for a total of six (6) residents of which five (5) are non-ambulatory and (1) bedridden. LPA observed that currently there are five (5) residents in care, two (2) of which are under hospice care. At 11:10 am, LPA conducted a tour of the facility, and the following was observed: Infection control: LPA reviewed facility infection control plan submitted on 01/13/2022 to make sure licensee was following current infection control recommendations. Kitchen: At 11:10 am LPA toured the kitchen area and observed enough supplies of non-perishables for seven (7) days and perishables for two (2) days at the facility. All knives and medications are observed to be locked in a kitchen cabinet and in a locked side cabinet inaccessible to residents. There is a fire extinguisher in the kitchen area, and it was last serviced on 07/17/2023. Bedrooms: There are four (4) bedrooms designated for resident use and have sufficient lighting. All bedrooms are properly furnished, clean and have appropriate bedding and linens. Auditory alarms were tested and observed to be operational. Facility has awake staff at night. Continued on LIC809-C Bathrooms: LPA observed two (2) bathrooms that are clean and in good repair. Properly supplied with toilet paper, soap, and paper towels. The hot water temperature measured 116.2 °F. LPA observed appropriate grab bars and non-skid mats. LPA observed appropriate hand washing signs posted in each bathroom. All trash cans in bathrooms had fitted lids to protect from cross contamination. Common Areas: The facility maintains a comfortable temperature at 69 °F. The living room and dining area appeared clean and were properly furnished. No obstructions or tripping hazards throughout the facility. LPA observed all trash cans throughout the facility have fitted lids. Smoke and carbon monoxide detectors were located throughout the facility, and at 11:40 am they were tested and observed to be operational. Outside Areas: LPA toured the outside area of the facility. LPA also observed a clean covered patio and backyard furniture. There were no bodies of water observed. There are two (2) sheds located in the backyard used for storage. LPA was granted access and observed that sheds are used for storage. Laundry/Garage: Laundry area is in the garage and LPA observed it to be locked and inaccessible to residents in care. LPA observed an attached locked garage door inaccessible to residents. Extra PPE supplies and food storage were also observed. Resident Files: LPA conducted a file review of resident records to ensure compliance of licensing forms. Staff Files: LPA conducted a file review of staff records to ensure compliance of licensing forms. Medications: LPA and Administrator reviewed medication and medication records for proper documentation. Staff Interviews: At 12:15 pm LPA interviewed staff. Client Interviews: At 12:45 pm LPA interviewed clients. Administrative: Annual fees are current. LPA collected Certificate of Liability Insurance, LIC500, Administrator Certificate and Resident Roster. No citations issued during this visit. Exit interview conducted.
