Allegation investigated: Neglect/Lack of Supervision: Staff failed to supervise Resident #1 (R1) resulting in R1 sustaining a fracture from a fall while in care. Facility staff are not providing adequate supervision to residents in care. Facility staff left resident in a soiled diaper for a long period of time.

Licensing Program Analysts (LPAs) Martha Arroyo and Valeria Conway conducted a subsequent complaint visit to deliver final findings for the above allegations. During today’s visit, LPAs met with Administrator, Lidia Medina, and explained the reason for the visit. On 07/28/2023, the Department received a complaint regarding an allegation of Neglect/Lack of Supervision. It was alleged that the facility staff failed to provide supervision to Resident #1 (R1) resulting in R1 falling and sustaining a hip fracture. Licensing Program Analysts (LPAs), Martha Arroyo and Valeria Conway conducted a subsequent visit to the facility to issue findings for the above allegation. The initial visit was conducted on 08/15/2023 by LPA M. Arroyo. During today's visit, LPAs met with Administrator, Lidia Medina and the reason for the visit was explained. Entrance interview. During the initial visit on 08/15/2023, LPA Arroyo conducted interviews with the Administrator and two staff between 2:50 p.m. and 3:24 p.m., conducted a resident file review at 3:35 p.m., and obtained copies of pertinent documents relevant to the investigation. Licensing Program Analyst (LPA), Martha Arroyo conducted an initial complaint visit to the facility for the above allegation. Upon arrival, LPA met with the Administrator and the reason for the visit was explained. Entrance interview conducted. During today’s visit, the LPA interviewed the Administrator at 2:50 p.m., conducted a file review at 3:00 p.m., and obtained copies of pertinent documents. Regarding the allegation: Facility failed to issue a refund. It was alleged that facility failed to issue a refund to Resident #1’s (R1’s) family. Review of documents revealed R1 was no longer living in the facility by 08/12/2023. Additionally, interviews conducted revealed R1’s family had also removed all of R1’s personal belongings from the facility by 08/12/2023. (Report Continued on LIC 9099C...) It was reported that insufficient amounts of food were observed in the facility and rotten fruit was also observed in the fruit bowl. Licensing Program Analyst (LPA) Zabel Chochian conducted a subsequent complaint visit to this facility today regarding above allegation. Upon arrival LPA met with staff and Administrator was contacted. Reason for visit was explained and allegation finding was discussed with Administrator by telephone. Following is a summary of the investigation: On 11/22/2022, a complaint visit was initiated and between 2:30pm-2:45pm, Resident (R1) records was reviewed and copies obtained. Between 2:45pm-3:15pm, LPA conducted interview with R1. On 01/30/2023, potential witness who is a reliable source was interviewed approximately 1pm. Information was provided that the Administrator did not provide R1's records when requested to assist R1 with a specific request. Administrator reported that R1's file was eventually provided however she was not certain about whether or not she can provide copy of the power of attorney records. Licensing Program Analyst (LPA) Zabel Chochian conducted an unannounced subsequent complaint visit to deliver final investigation finding regarding above allegations. During today’s visit LPA Chochian met with staff and later spoke with administrator on the phone. Reason for the visit explained. Following is a summary of the investigation: On 09/07/2021 the Department received a complaint with the above allegations. It was reported that resident #1 (R1) was not feeling well and tried to call for help and the call button was out of reach. Apparently, no staff responded until the morning. It was also reported that the same happened a couple weeks ago. On 09/16/2021, LPA Chochian conducted the initial complaint visit. Physical plant tour was conducted with staff at 10:30am-11am; during the physical plant tour residents were observed and interviewed.
