Allegation investigated: - Staff is not properly addressing rodent issue in the facility

On 09/25/2025, Licensing Program Analyst (LPA) Jaime Vado conducted an unannounced complaint investigation visit to deliver findings regarding the allegations received. LPA met with resident services director Ria Hernandez and explained the purpose of today's visit. During the investigation, LPA Vado and LPA Jain made observations, reviewed documentations, and conducted multiple interviews. It was found that the facility has had pest control in place for several months prior to this allegation being made. Pest control regularly came and set up traps and conducted evaluations prior to the allegation being received. Documents reviewed confirm the services from the contracted pest control. Staff were also trained to make observations and report sightings as well in order to immediately conduct spot cleaning or alerting to pest control for a return visit before the the regularly scheduled visit. Licensing Program Analyst (LPA) David Marrufo conducted an unannounced complaint investigation visit and met with Administrator (ADM) Rod Moshiri. On 01/04/2023, the Department received a complaint with the above allegations. On 01/12/2023, LPA Marrufo conducted an initial complaint investigation visit. Additional visits were conducted on 10/10/2024 and 01/09/2025. LPA Marrufo obtained copies of the Medication Administration Records (MAR) for residents R1-R4 from the months November 2022 to January 2023. R1’s MAR indicates that R1 was provided medications as prescribed. R2’s MAR indicates that R2 was provided medications as prescribed but had empty boxes on 10:30 AM and 12:00 PM on 11/27/2022 and 10:30 AM on 12/29/2022. R3’s MAR indicates that R3 was provided medications as prescribed but had empty boxes on 12:00 PM on 11/27/2022 and 6:00 AM 12/31/2022. Licensing Program Analyst (LPA) David Marrufo conducted an unannounced complaint investigation visit and met with Mehrad Moshiri, Executive Director. On 11/02/2022, the Department received a complaint with the above allegations. On 11/09/2022, LPA Marrufo conducted an initial investigation. On 10/10/2024, LPA Marrufo conducted an additional complaint investigation visit. On 09/13/2022, the facility submitted an LIC624 Unusual Incident/Injury Report (IR) to the Department. The IR states that on 09/07/2022, resident R1 had an unwitnessed fall in R1’s bathroom. Staff found R1 on R1’s left side. R1 was unable to answer staff questions and was slurring words when speaking. Staff called 911 immediately after finding R1 had fallen at 8:29 AM. Paramedics arrived onsite at 8:39 AM and transported R1 to the hospital for further evaluation.
