On 8/26/2026, LPA Grace Donato conducted a Case Management-Incident visit. LPA met with Assistant Executive Director, Patricia Melara and explained the purpose of the visit. LPA visited the facility regarding a report received about a fire that occurred today 8/25/2026. Resident (R1) had a fire in the room. R1 is currently in the hospital recuperating. R1 also lives in the independent living area of the facility. No other residents were affected by the fire. LPA inspected the affected area. It is currently being cleaned up after the fire department gave a go signal for the clean up. Facility will submit a written plan of action regarding necessary repairs for affected area. On 3/24/2026, Licensing Program Analyst(LPA) John Calandra arrived at the facility to open this complaint received by the Department on 3/19/2026. LPA Calandra was greeted by Monica Mendoza Ceron, Senior Executive Director/Administrator and explained the purpose of the visit. Complaint alleged that staff do ensure the facility is properly maintained. Specifically, the reporting party mentioned that recently the facility has been replacing screen doors and that the new sliding doors they have installed do not have screens on them. According to the Administrator, the facility has started to replace sliding doors on balconies in their Independent living buildings but has not yet started on their Assisted Living building. The facility is currently installing new screen doors on the new sliding doors in Independent living and will begin doing the same work in Assisted Living. Based on interviews, the facility is properly maintained. Anytime something needs to be fixed staff fix it. Unsubstantiated Complaint also alleged that staff do not ensure the facility is free of insects. Based on interviews and document review, the facility has a monthly pest control company that sprays the inside of apartments and outside of the buildings and is being kept free of insects. Although the above allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is UNSUBSTANTIATED. During today's visit, LPA collected a copy of the facility's roster of residents and roster of staff. LPA also received a copy of the facility's monthly pest control report from 2026 visits. Allegation investigated: Unqualified staff are providing care and supervision to residents in care.

On 3/24/2026, Licensing Program Analyst(LPA) John Calandra arrived at the facility to deliver conclusionary findings for this complaint received by the Department on 1/8/2026. LPA Calandra was greeted by Katreena Borlaza, Business Office Director and explained the purpose of the visit. Monica Mendoza Ceron, Senior Executive Director arrived later during the visit. Complaint alleged that unqualified staff are providing care and supervision to residents in care. Reporting party stated that the Licensee hired 1 Licensed Vocational Nurse(LVN) and multiple Medtechs to provide care and supervision. According to the Administrator, all care staff including Medtechs and Caregivers are required to complete 2 weeks of training prior to starting their roles. In addition, it is the Licensee's policy that all Medtechs are required to perform the duties of a Caregiver before they become a Medtech. In addition, based on record review, staff have received the required training regarding subjects such as care and supervision, Dementia, Postural Supports and Hospice care. Unsubstantiated Although the above allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is UNSUBSTANTIATED. On 3/24/2026, Licensing Program Analyst(LPA) John Calandra arrived at the facility to conduct a Case Management visit in regards to complaint #14-AS-20260108165840. LPA Calandra was greeted by Monica Mendoza Ceron, Senior Executive Director and explained the purpose of the visit. The Reporting Party alleged that the Licensee does not have personnel sufficient in numbers to provide care and supervision to persons in care. According to the Reporting Party, Licensee hired multiple Medical Technicians(Medtechs) to provide care and supervision in place of caregivers. According to the Administrator, Monica Mendoza Ceron, all staff are required to complete basic care and supervision training as everyone assists persons in care. In addition, the Administrator stated that she has hired one Licensed Vocational Nurse(LVN) and 2 new caregivers since the change in management took place. According to the Administrator, all of her Medtechs have been employees for more than 10 years and were caregivers before they became Medtechs per company policy. In addition, based on record review, Licensee has care staff employed for each shift. No deficiencies cited during today's visit. An exit interview was conducted. Type of visit: Case Management - Incident

On 1/23/2026, Licensing Program Analyst(LPA) John Calandra arrived at the facility to conduct a Case Management visit in regards to an incident that occurred on 1/8/2026 involving a resident who had given financial gifts to a staff member. LPA Calandra was greeted by Monica Mendoza-Ceron, Executive Director and explained the purpose of the visit. LPA reviewed R1's file and conducted staff interviews. According to the Administrator, R1 provided two checks of over $1,000 to S1. When the Administrator learned of this, she interviewed R1 who denied the allegation. The Administrator then spoke to S1 who acknowledged receipt of the gifts from R1. Since the incident, R1 has moved out of the facility and according to the Administrator, S1 has been terminated. In addition, the Administrator conducted a training with all staff regarding the facility's policy on accepting gifts which is that facility staff cannot accept tips or financial gifts from residents. During file review, LPA observed that the incident occurred on 1/8/2026 but was not reported to Licensing until 1/21/2026. Per Title 22, it is required that a written report shall be submitted to the licensing agency and to the person responsible for the resident within seven days of occurrence. A Type B citation was provided for this deficiency. Deficiencies are cited under the California Code of Regulations. Failure to correct the deficiencies by the POC due date may result in Civil Penalties. An exit interview was conducted. Type of visit: Post Licensing

On 1/7/2026, Licensing Program Analyst(LPA) John Calandra arrived at the facility to conduct the post-licensing visit. LPA Calandra was greeted by Monica Ceron-Tapia, Executive Director and explained the purpose of the visit. LPA toured the physical plant. This is a 5-story building with 108 apartments, 108+bathrooms, a dining room, kitchen, and common spaces. All bedrooms had the required furniture and sufficient lighting. All bathrooms had anti-skid flooring and grab bars. The facility's fire alarms and carbon monoxide detectors were observed to be in working order. The facility's fire extinguishers were observed to be fully charged and last checked on 7/10/2025. The facility had the required 7 days of non perishables and 2 days of perishables on hand. No food was expired. The Administrator's certificate was observed to be active and not expired. All sharp objects, soap, detergents, and poisons were observed to be locked and in-accessible to persons in care. LPA reviewed 5 resident files and 5 staff files. All were observed to have the required documents except for Admission agreements bearing new Licensee's name. A Type B citation was issued for this deficiency. In addition during record review, LPA observed that S1 did not have a TB exam result. A Type B citation was issued for this deficiency. Type of visit: Prelicensing

On 10/31/2025, Licensing Program Analyst(LPA) John Calandra, arrived at the facility to conduct the Pre-Licensing inspection. LPA Calandra was greeted by Monica Mendoza Ceron, Executive Director and explained the purpose of the visit. LPA Calandra toured the physical plant. This is a 5-story building with 108 bedrooms and bathrooms, a library, kitchen, dining room, lobby, and offices. Per interview with the Executive Director, the facility's fire alarm system is directly connected to the fire department. The facility's fire alarm panel was observed to be in working order. All bedrooms had the required furniture and sufficient lighting. The facility was maintained at a comfortable temperature. The facility had the required 7 days of non perishables and 2 days of perishables on hand. No food was expired. All sharp objects, soap, detergents, and poisons were observed to be locked and in-accessible to persons in care. LPA reviewed 5 resident files and 6 staff files. All were observed to be complete. A review of Centrally Stored Medications indicated that medications for residents were properly labeled with instructions on dosage and times of day and matched the Centrally Stored Medication Records(CSMR) kept at the facility. LPA requested the following documents be sent to the Department by 11/7/2025: Admissions Agreement No deficiencies were cited during today's visit. Pre-Licensing is now complete. Immediate Licensure is recommended pending final approval from the Central Applications Bureau. An exit interview was conducted.
