“Not on file” is not a no — approvals can be bed- or room-specific, so confirm current scope with the home on a tour. Where a number is shown it is the state’s own wording for how many residents the approval covers, not how many places are open today; where none is shown, the record simply does not state one.

Licensing Program Analyst (LPA) Dawn Segura conducted an unannounced visit to deliver investigative findings. LPA was granted entry into the facility and met with Ma Niza Ambalada, Administrator, to whom LPA disclosed the reason for the visit. Anafe Rivera, Site Manager, was called and arrived a short time later. Community Care Licensing (CCL) has investigated the above listed complaint allegation. The investigation consisted of a tour of the facility, review of facility records, and interviews with staff and outside sources. It was reported to Community Care Licensing that facility staff were not meeting the incontinent care needs of Resident 1 (R1) [an LIC 811 Confidential Names List was provided to staff to identify the resident]. It was alleged that R1 was repeatedly left soiled by facility staff who were refusing to change the resident. Interviews and a review of records generated by outside sources did not provide sufficient evidence to support the allegation. What the state has logged

California has logged 9 state visits for this home as of August 2, 2026. These are the home's own counts, straight from that record — shown beside the statewide median for small board-and-care homes (6 or fewer beds), computed across all 5,773 licensed homes of that size, because larger and longer-licensed homes naturally accumulate more visits and reports. They are facts, not a grade — a citation may be minor and since corrected, and an “unsubstantiated” complaint is not a finding of wrongdoing. No substantiated complaints are on file.
