On March 19, 2026, Licensing Program Analysts (LPAs) Andrew Martinez and Edith Conchas conducted a subsequent unannounced visit to the facility to continue the investigation of the above-mentioned complaint allegation and deliver findings. LPAs were greeted by Business Service Director, Pricilla Vazquez, indicating the reason for the visit and met with Administrator Wendy Barrera. Today’s investigation consisted of additional interviewing and further records review. The allegation is that staff are not allowing resident to use motorized wheelchair. Interviews with Staff 1 (S1) and Staff 2 (S2) indicate that resident’s (R1) operation of their own motorized wheelchair in the facility began posing a safety concern to self, residents and staff. Based on record review, LPA did not observe staff notes or unusual incident reports of occurrence or of safety concerns reported. On 7/24/2025 at 10:40 AM, Licensing Program Analyst (LPA) Eldin Serrano made an unannounced visit to the facility to deliver the findings of the above allegations. LPA Serrano met with Resident Care Coordinator (RCD) Jessica Valiente to explain the purpose of the visit. The investigation consisted of file review, interviews with facility staff and residents as well as facility observation. Allegation #1: Staff did not assist a resident's call button in a timely manner – Based on observation LPA tested one of the resident’s call buttons and one of the staff came to the room to check the resident within 2 minutes. Based on the interview, 6 out of 6 residents and 6 out 6 staff stated that when the call button was pressed the care partners would come right away, whoever staff that is available will come to check the residents. LPA was unable to corroborate the allegation. Licensing Program Analyst (LPA) Melody Brown conducted an unannounced visit to the facility 03/11/2022 at 09:30 AM to deliver the finding of the above allegations. LPA Brown met with Business Services Director Priscilla Mancilla . The investigation was conducted by the Department staff. The investigation consisted of file review and interviews with relevant parties. The first allegation indicated that resident had multiple falls while in care. Evidences shows that R1 sustained multiple falls while living at the facility. However, evidences also shows that the facility documented the falls, ensured that R1 was medically assessed, transported R1 to the hospital and notified R1's Power of Attorney (POA) after each fall. Also, 24- hour supervision and also placed R1 on hospice care. Licensing Program Analyst (LPA) Stephanie Williams made an unannounced visit to the facility in order to deliver findings for the above allegation. LPA Williams identified herself to Administrator, Wendy Barrera, who was also informed of the purpose of the visit. The investigation consisted of interviews with residents and staff. LPA Williams interviewed Staff #1 (S1) who stated that the facility has been experiencing several COVID-19 cases, which prompted facility staff to employ more precautions for the safety of residents. S1 stated that visitation is allowed and encouraged outdoors in the designated visitation area. S1 stated that residents are able to leave the facility; however, they encourage residents to stay in their rooms or congregate in outdoor areas while wearing face masks and practicing social distancing. LPA Williams interviewed Resident #1 (R1) who stated that they were told they could congregate with Resident #2 (R2) in an outdoor area but R1 refused to do so.
