Licensing Program Analyst (LPA) Ivashia Wright conducted an unannounced annual required visit. Upon entry, LPA was greeted by Marlya Dunham, Executive Director, and informed them of the purpose of the visit. Facility Overview: LPA tour the facility inside and out. LPA observed 7 resident bedrooms. 7 out of 7 rooms had the the required bedding and furniture. Grab bars are available in the restrooms. There is no gated pool and there are no firearms on the premises. LPA observed a functioning water fountain located at the center of the courtyard. The fountain appears to be inaccessible due to its surrounding features. It is enclosed by a two-foot-high brick structure, forming the outermost layer. Decorative rocks are placed within each tier, partially submerged in the water. Physical Plant: The physical plant, including floors, windows, and doors, was clean and well maintained. Fixtures and furniture were in good repair. The outdoor area was free of hazards. Laundry equipment was in good working condition. Hot water temperature was measured at 110.8 F, meeting the department requirements. LPA observed fire extinguishers to be in compliance with the department's requirements and with a service date of 1/19/2026. Fire system observed to be operable and tested 7/2/26 by the fire department. Food Service: The facility’s kitchen was clean and equipped to prepare food. The facility maintained the required two-day supply of perishable foods and a seven-day supply of non-perishable foods. Sharp and dangerous objects were securely locked and inaccessible to residents. A menu is posted and available for review by residents. Continued on LIC809-C.... Care & Supervision/Administration: Adequate staff were present to supervise residents during the visit. The administrator holds a current administrator’s certificate and CPR certification which expires 9/9/2026. This facility has an operable signal system. Record Review and Resident/Staff Files: LPA reviewed files for six (6) staff members, confirming criminal clearances, updated training, and CPR/First Aid certification. Five (5) resident files were reviewed and contained all required documentation. Health-Related Services/Incidental Medical Services: All resident medications were securely locked in a medication cart. No deficiencies were cited during the visit. Licensing Program Analyst, (LPA) Armando Perez made an announced visit to the facility for the purpose of conducting a pre-licensing inspection due to a change of ownership with residents in care. LPA met with Administrator Marlya Dunham, who accompanied LPA for the inspection. Applicant has submitted an application for sixty one (61) non ambulatory residents and (5) bedridden for a total capacity of 66. On June 25, 2025, Hemet Fire Department approved a fire clearance for the facility. The facility is a secured compound with four (4) structures, LPA observed 10 client bedrooms. Ten out of Ten rooms had the the required bedding and furniture, such as, clean mattresses/linen, night stands, dressers, chairs, lighting, and emergency lighting. Two out of the 10 were model bedrooms and had the required furnishing. Client bathrooms and showers had clean appliances that were operating in safe and sanitary condition and had the grab bars in toilets and showers. Facility kitchen had the ability to prepare food in clean environment and possessed equipment in good working condition. The facility met the required 2-day supply of perishable and 7-day supply of non-perishable foods. Sharps, such as knives and scissors, will be stored in the kitchen with the entrance to the kitchen be locked and not accessible to clients. A menu is posted and available for review by clients. Facility has a designated laundry areas that have locked cabinets for storing laundry chemicals. LPA observed a vehicle designated and available for the transportation needs of the clients. The vehicle has the capacity to transport up to 3 clients. The vehicle is equipped with proper safety equipment and has valid registration until February 2026. Continued on LIC809-C. Personnel files will be locked in the Director's office. Client files are locked in the memory care structure. Client medication will be centrally stored and locked in two designated medication carts in the memory care structure. There is a courtyard and covered patio area with seating for clients. All passageways were free from obstruction. LPA observed fire extinguishers in the facility. The smoke detectors and carbon monoxide alarms were operational and recently tested by Hemet Fire Department. LPA observed a functioning water fountain located at the center of the courtyard. The fountain appears to be inaccessible due to its surrounding features. It is enclosed by a two-foot-high brick structure, forming the outermost layer. Inside this perimeter, a second layer of decorative plants encircles the fountain. The fountain itself forms the third and central layer. The fountain consists of three tiers, each holding approximately one inch of water. Decorative rocks are placed within each tier, partially submerged in the water. LPA observed the required postings of the emergency disaster plan, resident personal rights, complaint procedures, employee rights, visitation rights, facility sketch, and the Long-Term Care Ombudsman poster. Facility contains emergency supplies and first aid kits with the required items. The facility has working telephone for client use. LPA observed Administrator Certification on file with expiration date of 05/04/2027. LPA observed that the physical plant is clean, in good repair, and to be hazard-free during today’s visit. LPA determined the facility meets the operational requirements for licensure. The Pre-licensing inspection is complete and this facility has no issues or concerns. The facility has satisfied all requirements in accordance with Title 22, California Code of Regulations.
