Licensing Program Analyst (LPA) Javina George made an unannounced visit to the facility to deliver findings for the allegation(s) listed above. LPA met with Robin Rebollar-Icamen and explained the purpose of the visit and the elements of the allegation(s). The allegation(s) were investigated and the investigation consisted of observations, interviews and a review of both staff and resident records. Regarding the allegation staff does not ensure residents records are properly maintained. It was alleged that Staff #1 (S1) had not been charting the resident’s medication logs correctly. As Resident #1 (R1) has a medication that was prescribed that requires to have their vitals taken and documented before the medicine is dispensed. Upon a review of the Medical Administration Record (MAR) revealed that S1 not been had not been taking the R1's vital signs daily or charting the residents’ records for medication dispensed. Licensing Program Analyst (LPA) Javina George made an unannounced visit to the facility to commence a complaint investigation in regards to the allegation listed above. LPA met with Administrator Bernadette Best, Administrator where LPA explained the purpose of the visit and the elements of the allegation. Regarding the allegation of staff are not providing medications as prescribed to resident(s) in care. It was reported that on or around June 16, 2023, Resident #1 (R1) did not receive their 2:00am medication as prescribed. Per interviews conducted with the Administrator Rene, revealed that the staff had taken both sets to the medication room keys home. A text and phone call were made to retrieve the keys, but was not viewed until 5:30am, resulting in R1 getting the medication late. Based on interviews the allegation of staff are not providing medications as prescribed to resident(s) in care is SUBSTANTIATED. On today's date, Licensing Program Analyst (LPA) Crystal Colvin arrived at the facility unannounced for the purpose of following up on an investigation with the above allegation. LPA Colvin met with MedTech Mariel Hernandez. Below is a summary of the findings of the investigation: Regarding allegation "Resident is being molested by another resident while in care": LPA Colvin conducted interviews with residents and facility staff regarding the allegation. LPA Colvin was not provided with any names to aid in investigation of the complaint. LPA Colvin additionally interviewed some family members of resident(s) and no concerns related to the allegation were reported. LPA Colvin additionally reviewed recent Incident Reports for the facility and did not observe any related concerns. On today's date, Licensing Program Analyst (LPA) Crystal Colvin arrived at the facility unannounced for the purpose of initiating an investigation with the above allegation. LPA Colvin met with Administrator Griselda T. Garcia. Below is a summary of the findings of the investigation: Regarding allegation "Staff did not follow Covid-19 safety protocols": LPA Colvin conducted interviews with staff and resident(s) regarding the allegation. According to interviews conducted, resident (R1) was admitted to the facility on 6/21/22 without a prior negative COVID-19 test. According to statements made by the Administrator, R1 was given a rapid COVID-19 test at the facility the evening after R1 was admitted (6/22/22), though the facility does not have any records of the test. One week after R1 was admitted to the facility, the facility had an outbreak of COVID-19 and all residents were tested for COVID-19, including R1. Since the facility did not obtain a negative COVID-19 test from R1 prior to RCDSS inspection report, July 26, 2022 · control 18-AS-20220725131214
