At 9:30 AM, Licensing Program Analyst (LPA) Huma Rahimi conducted an unannounced initial complaint visit to this facility to investigate the above allegations. LPA met with staff Keneshbek Minzhasharov and the staff contacted the Administrator via phone. LPA explained the reason for the visit. During course of the investigation, interviews and record review were made. At 9:35 AM, LPA requested resident and staff roster. At 9:40 AM, LPA requested copies of pertinent information which include, but not limited to Admission Agreement, Physician Report, Appraisal Needs and Services Plan, Centrally Stored Medication Destruction (CSMD) and etc., relevant to the investigation. At approximately 9:45 AM, LPA conducted a physical plant tour. Between 9:55 AM – 12:45 PM, LPA conducted an interview with the Administrator, a staff, and five (5) out of sxi (6) residents who were able to communicate. At 10:00am, Licensing Program Analysts (LPAs) Angela Panushkina and Christopher Alemoh conducted a subsequent visit to this facility to deliver final findings. LPA met with Hranush Khachatryan, Staff #1 (S1) who granted access to the facility. Administrator was contacted and the reason for the visit was explained. Administrator informed LPA that she's unable to come in and designated S1 to sign the report. During the initial visit conducted on 05/05/23, interviews and record review were made. At 10:10am, LPA met with the Administrator and requested resident and staff roster. At approximately 10:15am, LPA conducted a physical plant tour, to ensure health and safety of the residents are protected and physical plant is in compliance with Title 22 Regulations. At 11:00am, LPA requested copies of pertinent information which include, but not limited to Physician’s report, Appraisal Needs and Services Plan, Resident Appraisal, etc., relevant to the investigation. Licensing Program Analyst (LPA) Gary Tan conducted an unannounced initial complaint visit at this facility to investigate the above allegations. LPA met with Marine Grigoryan and explained the reason for the visit. LPA conducted physical plant tour at 9:32 AM, requested copies of facility documents relevant to the investigation at 9:46 AM and interviewed staff and residents between 10:00 AM to 12:30 PM. LPA also conducted record review between 12:30 PM to 1:30 PM. Regarding the allegation that the Staff do not keep the facility free from mold, it was alleged that the facility has molds. LPA's observation during physical plant tour at 9:32 AM revealed that the ceiling room #2 and kitchen's ceiling had leak and currently in disrepair. LPA also observed that there are black spots on the kitchen ceiling. Based on the information gathered during this visit, the allegation is deemed substantiated at this time. Citation issued. At 10:00am, Licensing Program Analyst (LPA) Angela Panushkina conducted an unannounced complaint visit to this facility to investigate the above allegations. LPA met with Staff #1 who granted access to the facility. Administrator and Staff #2 arrived at 10:10am and LPA explained the purpose of the visit. Allegation: Facility staff did not have the Ombudsman poster posted. During the visit conducted by the credible witness on 04/10/23 the facility did not have an Ombudsman poster posted. To investigate this allegation, LPA conducted an interview with the Administrator at 11:20am. During the interview, Administrator confirmed that the facility had no Ombudsman poster available due to the poster being damaged and removed in the beginning of March 2023. Administrator also informed LPA that a new poster was requested and provided by the Ombudsman on 04/10/23. During today’s visit LPA observed the poster being posted by the facility entryway. On 3/15/2023 Licensing Program Analyst (LPA) Melissa Spaeth arrived at the facility to conduct a 10-day unannounced complaint visit and explained the purpose of the visit. Upon arrival, LPA was greeted by caregivers and LPA was allowed entrance to the facility. At approximately 10:45 a.m., licensee Artur Yezagelyan arrived to the facility. Allegation: Staff did not issue resident a refund. At 10:45 until 10:55 am. LPA conducted a physical plant tour. LPA interviewed Licensee from 11:00 am until 11:30 am and interviewed R1's family member from 11:30 am until 11:50 am LPA reviewed resident's files at 11:50 am until 12:15 pm. LPA interviewed the resident (R1) from 12:20 pm until 12:35 pm. During LPA's interview of the Administrator, Marine Grigoryan, Administrator stated received a referral from an agency regarding resident (R1). Regarding the allegation above, it was alleged that Person #1 (P1) paid $500 for a potential resident’s pre-admission fee and did not receive a refund. LPA interviewed P1 at 8:23 a.m. on 11/09/2022. LPA interviewed the Administrator at approximately 3:00 p.m. on 11/09/2022. From interviews, P1 and their resident never entered the facility after finding out the resident was unfit for assisted living and needed a skilled nursing facility. P1 was not informed of the facility’s refund policy regarding pre-admission fees. Based on interviews, Based on observations, the allegation is deemed SUBSTANTIATED at this time.
