On 01/21/26, at 9:15am, Licensing Program Analyst (LPA) Gina Saucedo arrived at the facility to conduct an unannounced, initial complaint visit and was greeted by Administrator, Candis Allen. LPA explained the purpose of this visit was to gather information, interview staff and residents and deliver findings for this complaint. On 01/21/26, LPA Saucedo asked for the census, staff, and resident rosters. On 01/21/26, at 9:55am, LPA Saucedo conducted a physical tour. On 01/21/26, at 10:15am, LPA Saucedo started to conduct resident and staff interviews. Licensing Program Analyst (LPA) LaQueena Lacy conducted an unannounced subsequent complaint visit to deliver findings on 05/17/2023. Upon arrival LPA LaQueena Lacy met staff Alexcis Peralta and explained the purpose of this visit. It was alleged that resident #1 (R1) may be a subject of possible financial exploitation at the assisted living facility. To investigate the above allegation LPA conducted interviews with staff on 12/28/2022 at approximately 2:38PM, and R1 on 01/13/2023 at 11:18AM. Staff denied any involvement with R1’s finances. R1 confirmed that they had filed a fraud claim with the bank in August 2022 disputing debit card transactions made against their account. During the investigation, R1 affirmed that no staff at the facility had made any unauthorized purchases with their debit card and feels that the facility staff take good care of them. Staff assist R1 when needed, and R1 is very verbal to staff if there is something that they are not doing properly. Licensing Program Analyst (LPA) Abeye Duguma conducted an unannounced subsequent complaint visit to the facility. LPA met with Alexcis Peralta and explained the reason for the visit. The Administrator designated Alexcis Peralta as the responsible staff member to sign and accept this report. It was alleged that Resident #1 (R1) was given an improper 30-day eviction notice. To investigate this allegation, on 08/22/2022, LPA conducted a physical plant tour at 11:30am, requested documents at 12:45pm and interviewed two (02) staff and seven (07) out of sixty-three (63) residents from 1:45pm - 3:45pm. Record reviews and interviews revealed that the facility issued an eviction notice based on the isolated incident of the violation of house rules. However, no appropriate actions were taken by the facility staff to prevent R1’s eviction. The incident was not discussed with R1. No verbal or written warnings were issued to R1 prior to eviction. Licensing Program Analyst (LPA) Rosaura Valenzuela conducted a subsequent complaint visit to the facility to deliver the findings for the above noted allegations. LPA met with Alexis Peralta, Manager and explained the purpose of the visit. During the investigation on 06/03/2021 between 11:04am and 1:00pm, staff and resident interviews were initiated. Interviews revealed that currently the facility has a total of eight (08) residents. There are between one (01) and two (02) caregivers per shift that are also caregivers assigned to Melrose Villas. On 07/26/2021, LPA Valenzuela reviewed staff schedule and facility residents' roster. As per record review, the facility does not have sufficient staff to assist residents. Caregivers and med-techs are shared between the two facilities: Melrose Chateau and Melrose Villas. Caregivers do not have assigned duties to assist specific residents.
