At 8:35 a.m. on 03/15/2024, Licensing Program Analyst (LPA) Nicholas Reed conducted an unannounced, subsequent complaint visit. LPA met with Staff #1 (S1) and disclosed the reason for the visit. The licensee called LPA at 10:00 a.m. to designate S1 to sing today's reports. Regarding the allegation “Staff did not seek medical attention for resident in a timely manner” it was alleged facility staff did not provide timely care when Resident #1 (R1) was “in respiratory distress and septic”. To investigate the allegation above, LPAs Reed and Comer interviewed the licensee and three (03) staff between 11:20 a.m. and 12:30 p.m. on 03/05/2024 and reviewed records pertinent to the investigation at 12:00 p.m. including but not limited to an admission agreement, medical assessment, identification form, and consent forms. At 11:20 a.m. on 03/05/2024, Licensing Program Analysts (LPAs) Nicholas Reed and Raymond Comer conducted an unannounced complaint visit. LPAs met with staff and later the administrator and disclosed the reason for the visit. To investigate the allegation above, LPAs interviewed the administrator and staff between 11:20 a.m. and 12:30 p.m. today and reviewed records pertinent to the investigation at 12:00 p.m. today including but not limited to an admission agreement, medical assessment, identification form, and consent forms. Regarding the allegation “Staff did not ensure that resident's records were maintained as necessary” it was alleged the records of Resident #1 (R1) were not provided to emergency responders when requested. Licensing Program Analyst (LPA) Gary Tan conducted an unannounced subsequent complaint visit at this facility to further investigate the above allegations. LPA met with administrator Dina Pamatmat and explained the reason for the visit. LPA conducted physical plant tour at 10:40 AM, requested facility documents relevant to the investigation at 11:00 AM and interviewed administrator and staff between 11:15 AM to 12:45 PM. Regarding the allegation that Staff did not seek medical attention for resident in a timely manner, it was alleged that Resident #1 (R1) was found breathing, tachypneic, tachycardic and hypotensive and highly likely was in septic shock. LPA's interview with the staff present during the time of the incident revealed that on or about 4:00 PM on 11/16/21, during a routine hourly check, Staff #1 (S1) observed that R1 was having a shortness of breath and very pale so S1 called Staff #2 (S2) to confirm S1's observation and checked R1's vital (O2 sat and BP) (continued on LICCDSS inspection report, January 20, 2022 · control 31-AS-20211118162835

Licensing Program Analyst (LPA) Gary Tan conducted an unannounced initial complaint visit at this facility to investigate the above allegations. LPA met with staff Jose Alderson Bista and explained the reason for the visit. LPA conducted physical plant tour at 9:20 AM, requested copy of facility documents relevant to the investigation at 9:45 AM and conducted interview with staff and residents from 10:00 AM to 1:00 PM. Regarding the allegation that the staff did not meet's resident's hygiene needs, it was alleged that resident have not been cleaned for sometime and having bodily fluids stains on her torso. LPA interview with Staff #1 (S1) at 10:45 AM, revealed that during episode of Resident #1 (R1) having a cardiac arrest, R1 had vomited on own clothes while S1 was trying to revive R1 via CPR upon the instruction and guidance of the 911 operator while waiting for the paramedics to arrive.
