LPA introduced self, stated the purpose of the visit and met with Administrator Rochelle Balaban. Based on interviews and records review the resident in the complaint does not reside at this facility. This agency investigated the complaint alleging; staff keep inappropriately sending resident to the emergency department and staff are not providing a safe environment for resident, found that the complaint was UNFOUNDED, which means it could not have happened, and/or, it is without a reasonable basis; therefore, we have dismissed the complaint. On 01/28/2026, an unannounced Complaint visit was conducted by Licensing Program Analyst (LPA) J. Duarte. LPA was met at the front by receptionist. LPA introduced self and stated purpose of visit.The receptionist contacted Administrator Rochelle Balaban and she arrived shortly after. During the visit, LPA conducted interviews, obtained records, and conducted a tour of the facility. Based on records review, observations, and interviews, the facility has an elevator that has been out of service since 11/25/25. The facility has contacted the elevator service provider they are contactracted with and the facility is waiting for the elevator service provider to obtain a part for the elevator to complete repairs needed. The facility has second elevator that was observed to be functioning and available for use. Licensing Program Analyst (LPA) J. Duarte and Shawna Doucette arrived at the facility unnounced to conduct a 10-day complaint investigation. LPAs met with administrator (AD) Rochelle Balaban. LPAs interviewed AD. LPAs reviewed and obtained copies of residents roster for assisted living and skilled nursing. Based on interviews and records review, R1 has never resided in the facility. This agency has investigated the complaint alleging; staff handled resident in a rough manner causing injury and that staff are not positioning resident in their bed according to physician's instructions, found that the complaint was UNFOUNDED, which means it could not have happened, and/or, it without a reasonable basis; therefore, we have dismissed the complaint. On 11/3/21 at 8:45 AM, Licensing Program Analysts (LPAs) Malia Thao and Lisa Salazar arrived unannounced to conduct a complaint investigation. LPA explained reason for inspection at front desk and was granted entry. LPAs met with Griscel Garcia, Memory Care Manager and Administrator Rochelle Balaban. During the course of the investigation, LPAs made observations, reviewed records, and interviewed staff. Based on the observations, record reviews, and interviews, LPAs found that the facility staff were not administering medication as prescribed. R1's medication Acetaminophen 325mg tablet was started on 10/29/21 as a routine medicine to be given two tablets by mouth every 8 hours for pain. Starting from 10/29/21 to today, only 9 doses have been administered, but 7 doses were remaining on the bubble pack card not given as prescribed. All doses for 10/29/21 through 10/31/21 were given. On 10/18/21 at 8:45 AM, Licensing Program Analyst (LPA) Malia Thao arrived unannounced to deliver findings. LPA was granted entry by front receptionist. LPA met with Administrator (ADM) Rochelle Balaban. During the course of the investigation, LPA conducted interviews and reviewed records. Based on LPA’s observations, interviews, and records reviewed, the preponderance of evidence standard has been met. Facility self reported S1 was found to have financially abused R1. S1 admitted to receiving checks from R1. Facility increased R1’s monthly fee due for January and February 2021 without the proper advance written notice of 60 days. The above allegations are substantiated. Deficiencies are being cited based on LPA observation, interviews conducted, and record review in accordance with the California Code of Regulations, Title 22, see LIC9909D. An exit interview was conducted and Plans of Corrections were reviewed and developed with the Administrator.
