Type of visit: Case Management - Other

On 12/16/25, Licensing Program Analyst(LPA) M. Yang arrived unannounced to conduct case management visit for the purpose of checking on the health and safety of the residents in care. LPA met with Administrator Pamela Bradley. Administrator informed LPA that excluded staff was hired on 06/16/22 and was terminated 10/05/22. Excluded staff last day working at the facility was on 09/28/22. No deficiency was observed. A copy of this report was provided to Administrator. Type of visit: Required - 1 Year

On 12/16/25, Licensing Program Analyst (LPA) M. Yang arrived at the facility unannounced to conduct the Required Annual Inspection. LPA were greeted by receptionist, stated the purpose of the visit and requested to meet with the Administrator. LPA met with Administrator (A1) Pamela Bradley. LPA conducted tour of facility with A1 and Health Service Director Sarah Archuleta-Weaver. Facility consists of Assisted Living (AL) and Memory Care (MC) Unit. Residents were observed seating in dining room and in memory care dining room. The facility was observed to be at a comfortable temperature, clean, in good repair, and there were no passageway obstructions or fire hazards. Facility is equipped with pull stations and fire sprinklers throughout facility. Fire extinguishers were observed throughout the facility with a service date of:10/15/25. Kitchen was toured. An adequate supply of perishable and non-perishable food was observed to be properly stored in walk-in freezer, walk-in refrigerator, and walk-in pantry. Walk-in refrigerator temperature was maintained at 32 degree F. and walk- in freezer was observed at -1 degree F. First Aid kit observed with required items. LPA toured a sample of resident bedrooms in Assisted Living and Memory Care. Facility has sufficient furnishings inside and outside for resident use. LPA observed pulling cords in resident bedrooms. LPA observed securely fastened grab bars and non-skid mats in shower. Medications were stored in a locked medication room in a medication cart. MARs and medications were reviewed. Washer and dryer were observed operational during visit. The outside was observed to be free from debris with outdoor seating available for residents. LPA observed exits in Memory Care to have a 30-second delay egress. A sample of resident and staff files reviewed. A deficiency is being cited on the attached Lic 809D in accordance to California Code of Regulations, Title 22, Division 6. An exit interview was conducted. LPA received copies of Lic 308, Lic 500, Lic 610E, Lic 9020, current liability insurance, and Administrator certificate. Allegation investigated: Resident was sexually assaulted by another resident while in care. Staff not able to assist residents in a timely manner due to short staff.

On 07/01/25, Licensing Program Analyst (LPA) M. Yang arrived unannounced to delivered complaint findings. LPA met with Administrator Pamela Bradley. During the course of the investigation, the department conducted interviews, toured the facility, and reviewed records. The department observed all nonambulatory and bedridden female residents’ room in memory care were locked and inaccessible to other residents. Staff and residents were able to exit bedrooms. Staff were observed responded to call pendant timely pulled. Based on observation, the preponderance of evidence standard has not been met, therefore, the above allegations are found to be UNSUBSTANTIATED. An exit interview was conducted. During the course of the investigation, the department conducted interviews, toured the facility, and reviewed records. Based on interviews conducted, staff was alleged yells at resident, the preponderance of evidence standard has not been met, therefore, the above allegation is found to be UNSUBSTANTIATED. Allegation investigated: Staff neglected resident resulting in resident developing pressure injuries. Staff does not ensure resident's diapering needs were being met. Staff did not seek medical attention to resident in a timely manner.

On 01/15/25, Licensing Program Analyst (LPA) M. Yang arrived unannounced to delivered complaint findings on the above allegations. LPA introduced self, stated the purpose of the visit and met with Administrator Pamela Bradley and Memory Care Director Mikayla Goulart . During the course of the investigation, records were received, interviews were conducted, and facility was toured. Interviews conducted with residents who confirms staff provided care for residents. Staff changes resident’s brief when needed. Based on records review and interviews conducted, there was insufficient evidence to prove or disprove that staff did not seek medical attention to resident in a timely matter. Therefore, preponderance of evidence standard has not been met, therefore, the above allegations are found to be UNSUBSTANTIATED. An exit interview was conducted. Type of visit: Required - 1 Year

On 01/15/25, Licensing Program Analyst (LPA) M. Yang arrived at the facility unannounced to conduct the Required Annual Inspection. LPA were greeted by receptionist, stated the purpose of the visit and requested to meet with the Administrator. LPA met with Administrator (A1) Pamela Bradley and Memory Care Director Mikayla Goulart. LPA conducted tour of facility with A1. Residents were observed seating in dining room and in memory care dining room. Facility consists of Assisted Living (AL) and Memory Care (MC) Unit. There are 62 residents on the Assisted Living side and 38 residents in Memory Care unit. The facility was observed to be at a comfortable temperature, clean, in good repair, and no passageway obstructions or fire hazards. Facility is equipped with pull stations and fire sprinklers throughout facility. Fire extinguisher was observed throughout the facility with a service date of: 10/25/24 LPA observed pull cords in resident bedrooms. Kitchen was toured. An adequate supply of perishable and non-perishable food was observed to be properly stored in walk-in freezer, walk-in refrigerator, and walk-in pantry. Walk-in refrigerator temperature was maintained at 38 degree F. and walk- in freezer was observed at 38 degree F. LPA toured a sample of resident bedrooms in Assisted Living and Memory Care. Facility has sufficient furnishings inside and outside for resident use. Hot water temperature measured at 114.2 degrees F in room 112, 113.8 degrees F in room 107, 113.1 degrees F in room 213, 130.3 degrees F in room 27, 125.3 degrees F in room 46, and 122.5 degrees F in room 12. LPA observed securely fastened grab bars and non-skid mat in shower. Bathroom was observed operational. Medications were stored in a locked medication room in a medication cart. MARs and medications were reviewed. The outside was toured and observed to be free from debris with outdoor seating available for residents. LPA observed exits in Memory Care to have a 30-second delay egress. Facility resident and staff files reviewed during inspection and observed to have all the required documents. A deficiency is being cited on the attached Lic 809D in accordance to California Code of Regulations, Title 22, Division 6. An exit interview was conducted. The following documents are requested and submitted to Fresno CCL by: 01/21/25. The following updated forms were requested: Lic 308, Lic 500, Lic 610E, current liability insurance, and Administrator certificate. LPA met with Health Service Director (HSD) Annette Eggleston. LPA was informed by Administrator via telephone on 11/22/24, that excluded staff member is not associated with facility and have not been employed by the facility. Based on interviews and records reviewed, the resident is ambulatory who has a history of falls, therefore there was insufficient evidence to prove or disprove that the resident received injuries while in care. LPA met with Health Service Director (HSD) Annette Eggleston who stated Administrator Pamela Anderson is unavailable. LPA toured rooms with the HSD and did not observe any cleaning chemicals, knives, and medications in room 105, 112, and 218. No deficiencies were issued during this visit. Type of visit: Required - 1 Year

On 01/23/24, Licensing Program Analyst (LPA) M. Yang arrived at the facility unannounced to conduct the Required Annual Inspection. LPA were greeted by receptionist, stated the purpose of the visit and requested to meet with the Administrator. LPA met with Health Service Director (HSD) Annette Eggleston. Administrator (A1) Pamela Bradley arrived shortly. LPA conducted tour of facility with A1. Residents were observed seating in dining room and in memory care gathering room. Facility consists of Assisted Living (AL) and Memory Care (MC) Unit. There are 57 residents on the Assisted Living side and 40 residents in Memory Care unit. LPA toured a sample of resident bedrooms in Assisted Living and Memory Care. Facility has sufficient furnishings inside and outside for resident use. The facility was observed to be at a comfortable temperature, clean, in good repair, and no passageway obstructions or fire hazards. Facility is equipped with pull stations and fire sprinklers throughout facility. Fire extinguisher was observed throughout the facility with a service date of: 10/26/23. LPA observed pull cords in resident bedrooms. Hot water temperature measured at 112.2 degrees in room 112, 111.6 degrees in room 105, 110.7 degrees in room 209, 111.6 degrees in room 24, and 110.8 degrees in room 21. LPA observed securely fastened grab bars and non-skid surfaces/mat in shower. Bathroom was observed operational. Medications were stored in a locked medication room in a medication cart. MARs and medications were reviewed. LPA observed exits in Memory Care to have a 30-second delay egress. Kitchen was toured. An adequate supply of perishable and non-perishable food was observed to be properly stored in walk-in freezer, walk-in refrigerator, and walk-in pantry. Walk-in refrigerator temperature was maintained at 38 degree F. and walk- in freezer was maintained at -12 degree F. The outside was toured and observed to be free from debris with outdoor seating available for residents. Exits tested in Memory Care and functioning properly and observed to have a 30-second delay egress. Facility resident and staff files reviewed during inspection and observed to have all the required documents. A deficiency is being cited on the attached Lic 809D in accordance to California Code of Regulations, Title 22, Division 6. An exit interview was conducted. The following documents are requested and submitted to Fresno CCL by: 01/29/24. The following updated forms were requested: Lic 308, Lic 500, Lic 610E, current liability insurance, and Administrator certificate. On 11/09/23, Licensing Program Analyst (LPA) M. Yang arrived unannounced to conduct a subsequent complaint visit and deliver finding on the above allegation. LPA introduced self, announced the purpose of the visit, and met with Administrator Pamela Bradley. During the course of the investigation, interviews were conducted, and records were reviewed. R1 had a doctor’s order for R1 to be hospitalized. R1’s doctor and R1’s family were working on an alternative placement. Placement was found for R1. Based on records reviewed and interviews which were conducted, the preponderance of evidence standard has not been met, therefore, the above allegation is found to be UNSUBSTANTIATED.
