Type of visit: Required - 1 Year
Licensing Program Analyst (LPA) Kevin Mknelly arrived at the facility unannounced on 05/06/2026 to conduct a Annual Inspection utilizing the CARE inspection tool. LPA met with staff and explained the purpose of the visit. LPA requested for staff to notify Administrator that LPA is present at the facility to conduct an annual inspection. Administrator arrived to assist. LPA toured the interior and exterior of the facility together with staff to ensure health and safety of residents in care. Areas toured include but are not limited to: common areas, resident bedrooms, bathroom, kitchen, laundry room, and backyard. In the areas toured no immediate health, safety, or personal rights violations were observed. The home is very clean and residents stated they are happy with care. Some recent wind damage is in repair LPA reviewed 3 resident files. Files are complete and well organized. LPA reviewed 2 staff files. Files are complete. No deficiencies are being cited as a result of todays inspection. Type of visit: Case Management - Other
On 1/16/24, Licensing Program Analyst (LPA) Kevin Mknelly, conducted a case management visit while met with caregiver. Administrator was contacted and arrived to assist. On 1/16/25, the department received a cross reported regarding ongoing issues of R1's representative failure to pay fees. LPA and Administrator discussed the issue and options available. Licensee will be in touch with a social worker to continue to resolve the issue. R1 is stable and receiving the assistance needed. LPA discussed new regulation changes with Administrator, provided flier and how to accesses additional information. As a result of today’s inspection, no deficiencies were noted. Report reviewed. LPA met with staff and explained the purpose of the visit. LPA requested for staff to notify Administrator that LPA is present at the facility to conduct an annual inspection. Administrator arrived to assist. LPA toured the interior and exterior of the facility together with staff to ensure health and safety of residents in care. Areas toured include but are not limited to: common areas, resident bedrooms, bathroom, kitchen, laundry room, and backyard. In the areas toured no immediate health, safety, or personal rights violations were observed. The home is very clean and residents stated they are happy with care. LPA reviewed 2 resident files. Identification of the applicant and administrator was verified through interview questions based on photo ID and other identifying personal information. During COMP II, applicant and administrator confirmed that they have read and understand community care facility licensing laws included in the Health and Safety Codes and the California Code of Regulations Title 22.
