Licensing Program Analyst (LPA) Mariana Agban conducted the facility's annual required visit and inspection. LPA met with Staff #2 (S2) and explained the purpose of the visit. Administrator Ann Solakyan was contacted and arrived shortly after. At approximately 11:15 a.m., with the assistance of staff, LPA conducted a tour of the physical plant. All required postings were observed in the entry area. The smoke alarms were found to be interconnected and battery-operated. A functioning carbon monoxide detector was installed in the kitchen area. A fire extinguisher was located near the kitchen, with a service date of June 18, 2025. Kitchen: All kitchen appliances and fixtures were in working condition. A sufficient supply of both perishable and non-perishable food was observed, properly stored. Knives were secured in a locked drawer. Properly labeled medications were stored in a locked kitchen cabinet. Bedrooms:There were six (6) bedrooms designated for resident use. All bedrooms were private and appropriately furnished with beds, linens, and adequate lighting.Bathrooms:There are two (2) bathrooms designated for resident use. Both were properly stocked and equipped with functional fixtures. The hot water temperature measured at the bathroom sink was 106.9°F. No cleaning supplies or hazardous items were observed in either bathroom at the time of inspection. Common areas: include a living room and a dining area. These areas were appropriately furnished and in good repair. Auditory alarms on all exit doors were functional. The dining table could accommodate the facility’s licensed capacity. LPA observed a small refrigerator in the living area; the administrator stated it was used to store residents’ medications. Upon inspection, expired medications, staff medications, and medications belonging to former residents were found. The administrator immediately removed all expired, staff, and former residents' medications. (Continue on 809C) Surrounding Grounds: Entry/exits were free of obstruction. There was furniture appropriate for outdoor use. The outdoor area was free of hazards. There is a swimming pool that is fenced all around it's parameters. The fence was at least five foot high with a gate, that is also five foot high. Gate was observed locked, making it inaccessible to residents to enter. The laundry area is located in the garage, which is inaccessible to residents. The gates at both sides of the home were checked to ensure no locks were installed, and that exits and passageways were clear for emergency evacuation. Resident Files: A review of resident records was conducted to ensure compliance with licensing requirements. It was noted that Resident #1 (R1) had full bedrails and was not receiving hospice services. The administrator immediately removed the bedrails and stated that R1 had fallen while asleep. Additionally, Resident #5’s (R5) file was missing a Medical Assessment (LIC 602A) and Preplacement Appraisal (LIC 603). Resident #6’s (R6) Admission Agreement was incomplete, and the Preplacement Appraisal (LIC 603) was also missing. Staff Files:LPA reviewed staff records to verify that all required forms and training documentation were current and in compliance with licensing regulations. Medications: Medications and medication records were reviewed. It was observed that Resident #3’s (R3) Centrally Stored Medication and Destruction Record (LIC 622) was missing documentation for the month of October. Resident #4’s (R4) medication records were inconsistent. Additionally, Resident #6’s (R6) medication records were incomplete. Licensing Program Analyst (LPA) Gary Tan, met with staff Aibek Kubanychbekov for a One (1) year required visit for this facility. Staff called the administrator Ana Solakyan who arrived at the facility twenty (20) minutes later. LPA explained the reason for the visit. A tour of the physical plant was conducted at 9:11 AM and the following was noted: There is only one entrance being utilized at the facility. Screening area is located immediately upon entrance. Sign in sheet, hand sanitizer, gloves and masks are available. The facility had submitted and approved Infection Control and Mitigation plan. Hand washing, coughing etiquette, physical distancing and other necessary signs were posted in the bathroom and all over the facility. The facility has a designated visitors' area at the backyard. The facility has sufficient stock of PPE in the storage room. The facility has six client (6) bedrooms and two (2) bathrooms currently occupying four (4) residents. An additional One (1) bedroom and one (1) bathroom is designated for staff use. The facility is fire cleared for six (6) non-ambulatory resident, one (1) of which maybe bedridden. Hospice waiver for two (2) residents. Physical environment was checked for cleanliness and condition. Walls, windows, ceilings, floors and floor coverings and doors were checked, the following was noted: Living and dining room furniture were also checked. The living room is neat and clean along with the dining room. The facility maintains a comfortable temperature at 75°F. Dual smoke/carbon monoxide alarms were tested and observed to be operational. There are fire extinguisher located in the kitchen and living room, they were observed to be full and last serviced on 07/20/24. The backyard of the facility has outdoor furniture, with a covered shaded area for clients. The swimming pool is appropriately fenced and observed to be locked. (continued on LIC 9099-C) The garage is attached to the house and observed to be locked. It is also being used as laundry area, storage for frozen foods, PPE, extra linen and old equipment. All the laundry detergents, cleaning solutions, toxins and other chemicals are observed to be locked in the garage and inaccessible to residents. Food Service/Kitchen area was sufficiently stocked with two (2) days perishable and seven (7) days of non-perishable food. Frozen foods are properly wrapped and stored appropriately. Food storage and preparation areas are clean and inaccessible to pests. All sharps and knives were also observed to be locked in the kitchen cabinet below the sink. The Clients' rooms are adequately furnished with appropriate furniture and lighting system. Hall ways/passage ways are lit. Clients have sufficient amounts of personal hygiene product which is provided by the licensee. Staff Rooms: Could only be accessed through the garage and observed to be locked. No medications are observed in the staff room. The bathrooms were checked for cleanliness and proper operation. LPA observed the appropriate grab bars for each toilet, bathtub and shower. Sink in the bathrooms were removed and under construction. The hot water temperature measured at a range 110.8°F to 115.2°F. Towels and washcloths are not shared. There is enough clean linen available in stock at the cabinet. Medications: LPA observed medication in the bedroom hallway cabinet to be locked and inaccessible to residents. Medications are listed on the centrally stored medication and destruction record. There is a complete first aid kit located in the living room cabinet. Client records: Client records are reviewed. all residents' file appeared to be complete and updated. Staff records: LPA conducted a complete file review of staff records. Staff records appear to be complete and updated. Disaster drill was last conducted on 10/30/24. Required posting are observed to be complete and current and displayed properly at the facility. On 1/17/24 RM Angela Kendrick , LPM Eva Miller and LPA Mariana Agban conducted Informal Meeting with Administrator Ann Solyan and consultant Hripsime "Ripa" Tavitian regarding complaint 31-AS-20190723103029. The Administrator was provided an opportunity to review the Facility File. Administrator described the series of events that occurred during and immediately after the investigation. RM requested of the original documents to be forwarded to CCL for subsequent review. Administrator advised that she would attempt to locate the documents and would forward if possible. On 10/06/23 Licensing Program Analysts (LPAs) Mariana Agban,Huma Rahimi and Leslie Ngo-Castandena, conducted an Annual Required visit and inspection of the facility. Upon arrival, LPAs was greeted by the staff and explained the reason for the visit. A tour of the physical plant was conducted at 9:30 AM. Kitchen: The kitchen appeared clean and the appliances and fixtures functional. LPAs found a sufficient amount of perishable and non-perishable food at the facility; properly stored. LPAs observed that sharp objects were stored in unlocked drawer. Staff immediately locked the drawer. Medications are locked in the hallway cabinet. Medications observed to be locked and inaccessible to clients. LPAs observed fully stocked first aid kit in the hallway cabinet. Bathrooms: There were two (2) bathrooms in the facility. One (1) bathroom in hallway which is the main and one (1) bathrooms in the private bedroom. All bathrooms were clean, properly supplied and had functional fixtures. Water temperatures were: 118.5 and 119.3 degrees Fahrenheit. Bedrooms: There were six (6) bedrooms designated for residents' use. All bedrooms were clean, properly furnished and had sufficient lighting. Common Areas: These included the living room and dining area. The common areas were properly furnished Surrounding Grounds: Entry/exits were free of obstruction.Temperature: Facility maintains a comfortable temperature of 78 degrees Fahrenheit. Fire extinguisher: are located in the dining room, and kitchen observed to be fully charged and was purchased on 07/23/2023 and 06/29/23. Laundry Area-Garage: located in the garage. Appliances observed to be in good repair. Garage door was locked and thus laundry detergents were inaccessible to residents. Backyard Area: a patio table and chairs shaded by a large umbrella for residents use. Patio furniture observed to be in good repair with adequate seating for the residents. There is a pool enclosed by a gate. Gate to the pool observed to be locked and inaccessible to clients. Due to discrepancy of the facility records, this annual will be continued in a another time.
