Type of visit: Required - 1 Year
On 7/28/26, Licensing Program Analyst (LPA) Kyle Wellington arrived unannounced to conduct an annual inspection. LPA met with Administrator (Admin), Nikita "Nikki" Mundhada, who was informed of the purpose of the visit. The census at the facility is fifteen (15) residents. There were four (4) staff and ten (10) residents at the facility during the visit. LPA received a client roster from Admin. LPA toured the inside and outside of the facility with Admin. LPA conducted an observation and record review for the inspection. Facility Overview: Facility is a one story building with nine (9) resident bedrooms, eight (8) resident bathrooms, one (1) staff room, one (1) staff bathroom, office, kitchen, dining room, laundry room and commons area. There are no pools, bodies of water or firearms at the facility. Facility has a fire clearance to for fifteen (15) non-ambulatory adults of which seven (7) may be bedridden. Hospice waiver was granted to the facility for seven (7) residents. Infection Control: LPA observed hand sanitizers and soap dispensers throughout the facility. Cleaning equipment and cleaning supplies were kept in the locked laundry room and available for regular facility maintenance. LPA reviewed the facility’s infection control plan which met the department’s requirements. Physical Plant: LPA observed the inside and outside of the facility to be clean, safe and well kept. The floors, windows and doors were clean and well maintained. The commons area and dining room furniture was in good repair. The residents’ bedrooms were neat, organized and contained the required bedding, lighting and furniture. Bathrooms were clean, tidy and had soap, grab bars and non-slip floors in the showers. Extra linen and towels were kept in a closet in the hall. Laundry equipment appeared to be in good working condition. Laundry supplies were kept in the locked laundry room. The one (1) fire extinguisher was charged. LPA tested the smoke and carbon monoxide detectors and found them to be operational. Facility has a pull system fire alarm. The wrap around deck was free of hazards and contained outdoor furniture and shaded area for the residents. Kitchen/Food Service: LPA observed the kitchen to be clean, organized, and well maintained. The kitchen had the ability to prepare and store food in a safe and clean environment. Kitchen appliances appeared to be in good working condition. All sharp objects were kept in a locked cabinet under the sink inaccessible to residents. Cleaning supplies were kept in a locked cabinet under the kitchen sink inaccessible to residents. Facility has over a two day supply of perishable foods and over a seven day supply of non-perishable foods. Care & Supervision: LPA observed four (4) staff and ten (10) residents at the facility. Four (4) residents were away from the facility and one (1) resident was in the hospital. Facility has sufficient staff to supervise the residents. Administration: LPA observed facility sketch, personal rights, emergency and disaster plan, emergency phone numbers, complaint procedures, long-term care ombudsman information, certificate of insurance and resident roster posted near the front door. Admin holds a current Administrator Certificate, CPR/First Aid Certificate and a Criminal Record Clearance. Record Review and Resident/Staff Files: LPA reviewed the records of two (2) client files and two (2) staff files. Staff present have criminal record clearance and are associated with the facility. The files contained all the required documentation and paperwork. The staff and client files were kept in a cabinet in the locked office inaccessible to unauthorized individuals. Health Related Services/Incidental Medical Services: LPA observed clients' medications were centrally stored in a locked cabinet in the locked office inaccessible to residents. First aid kit was kept in a locked cabinet in the locked office room and it contained all the required items. LPA reviewed two (2) clients' medications to the facility’s medication log to make sure all medication was accounted for and dispensed correctly. Disaster Preparedness: LPA reviewed the facility’s emergency and disaster plan. It is current and up to date. Fire drills are done quarterly. Facility's property and liability insurance is current and expires on 4/15/27. All facility exits had signage and were clear of obstructions. No deficiencies were cited during this visit. Jan 7, 2026Complaint investigation reportUnsubstantiated
Allegation investigated: Staff did not ensure resident is appropriately dressed for the weather
On 01/07/2026, Licensing Program Analyst (LPA) Janette Romero conducted an unannounced visit to the facility to investigate the allegation listed above. LPA met with Administrator, Shrikant Kakani who was informed of the purpose of the visit. LPA toured the facility, conducted interviews, and obtained copies of pertinent documentation. Regarding the allegation, "Staff did not ensure resident is appropriately dressed for the weather" it was alleged on 12/23/2025, Resident 1 (R1) arrived to a medical appointment on a gurney transport and was transferred to a wheelchair when they were observed wearing only a shirt and brief. It was further alleged R1 was covered with blankets and without pants or any lower-body garment, despite the cold and rainy weather that day. LPA reviewed R1's medical assessment dated 11/11/2025 noting R1 exhibits memory loss, requires assistance for bathing, dressing, grooming, and toileting, and is able to communicate their needs. Unsubstantiated LPA conducted an interview with R1 who reported the following information. Sometimes they avoid wearing lower-body garments due to using a catheter. However, they have the capacity to communicate their needs to facility staff and request dressing assistance to wear lower-body garments and weather appropriate clothing. R1 was unable to recall any incident on 12/23/2025 related to the allegation. However, they reported facility staff provide dressing assistance each time they request it. R1 informed LPA where their lower-body garments are stored in their bedroom. LPA then toured R1’s bedroom and observed lower-body garments including several pants stored inside a dresser drawer and in hangars in the closet. Three (3) staff were identified to be present on the incident date and time. However, one (1) staff was unable to provide relevant information due to being in a different area of the facility during the alleged incident. LPA was unable to make contact with the second staff for an interview. LPA conducted an interview with the third staff noted to be present on the incident date and time who reported the following information. On 12/23/2025, R1 was transported to their medical appointment by a transportation service provided though their health insurance program. This staff was unable to recall incident details from 12/23/2025 surrounding the allegation. However, they reported there have been multiple instances in which the transportation service for R1 and other residents failed to inform facility staff of scheduled pick-ups at the facility. This staff reported on one (1) occasion, the transportation service for R1 arrived at the facility at 6:00 a.m. without prior notice and provided facility staff only five (5) minutes to prepare the resident before departure to avoid missing their medical appointment, which was insufficient preparation time. This staff reported facility staff make sure residents are clean, groomed, eat, and are appropriately dressed for the weather prior to leaving the facility for an outing. Administrator was also interviewed and corroborated the information provided by this staff and R1. He forwarded LPA an email dated 01/05/2026 that he sent to R1’s medical provider reporting transportation arrived for a resident without prior notice. In the email, administrator also requested the medical provider share the schedule for any future appointments in advance so the facility can ensure the resident is prepared on time. LPA made contact with the reporting party who was unable to provide additional information regarding R1’s transportation services to their medical appointment on 12/23/2025. Although the allegation may have happened or is valid, there is no preponderance of evidence to prove the alleged violation did or did not occur; therefore, the allegation is unsubstantiated. An exit interview was conducted, and a copy of this report was provided to administrator. Allegation investigated: Facility increased capacity without obtaining city or state approval.
On 07/18/25 Licensing Program Analyst (LPA) Javina George made an unannounced visit to the facility to commence a complaint investigation in regards to the allegation listed above. LPA met with Shrikant Kakani, administrator and explained the purpose of the visit and the elements of the allegation. The allegation was investigated and the investigation consisted of observations, interviews and records review. On 07/11/25 Community Care Licensing received a complaint alleging the facility increased capacity without obtaining city or state approval. It was alleged that parking is an issue on the street. There was a licensed facility with an approved capacity of (14) residents, by the name of King's Care Assisted Living # 374603787, that was at this address. There was a recent Change of Ownership with residents in care, and the prelicensing inspection was conducted by the department on 06/27/25. LPA conducted a records review of the application for a RCFE license which revealed that the licensee applied for and received an approved fire clearance from the city of Vista Fire department on 03/21/25, for eight (8) non ambulatory and seven (7) bedridden residents, for the total of (15) residents. In addition the facility had the state issued licensed that Unfounded revealed the facility is licensed for (15) residents effective 0707/25. Based on records review the allegation of facility increased capacity without obtaining city or state approval is unfounded. A finding that the complaint is unfounded means that the allegation is false, could not have happened, and/or is without a reasonable basis. Type of visit: Prelicensing
On 06/27/2025 Licensing Program Analyst (LPA) Javina George made an announced visit to the facility to conduct a pre-licensing inspection. LPA met with the Applicant, Shirkant Kakani, whom accompanied LPA throughout today's inspection. The applicant has applied for Change of Ownership with residents in care, and received an approved fire clearance from the Vista fire department on 03/21/25, for eight (8) non ambulatory and seven (7) bedridden residents. in addition the applicant has requested a hospice waiver for eight (8), residents. LPA conducted an inspection and observed the following: The facility is a two structure story structure with 11 bedrooms 9 of which are for residents. The other (2) bedrooms are upstairs for staff use. Each resident bedroom has an exit door leading to the exterior of the property, and a bathroom. The bathrooms were observed to have walk in showers with non skid mats. The facility was observed to have a sprinkler system, with combined smoke and carbon monoxide detectors. The fire extinguishers are fully charged. The facility is equipped with solar panels as well as a Tesla charger with two (2) batteries. The medications, staff and resident files are stored inside the locked medication closet located in the hallway before entering into the dining room area. The sharps are stored in a locked cabinet underneath the sink. There are no pools or bodies of water on the premises. The hot water was checked and was found to be within regulatory limits. The facility was observed to be in possession of valid liability insurance that expires on 04/15/26. The facility food supply was observed to be adequate as there is a 2 day supply of perishable and a 7 day supply of non-perishable foods. Facility has a first aid book and first aid supplies with the required items. Passageways were observed to be free of obstruction. There are activities for the residents such as puzzles, and games. Upon a review of the first aid kit no approved first aid manual, scissors or tweezers observed. The applicant purchased a new first aid kit during today's inspection. Component III was waived for the applicant Based on today's inspection the facility is ready for licensure.
