Licensing Program Analyst (LPA) Robert Campbell conducted an unannounced annual required visit. Upon entry, LPA was greeted by Michele Fuhrman/Executive Director, and informed them of the purpose of the visit. At the time of the visit, there were ten (10) staff member and forty-six (46) residents present. Facility Overview: The facility is a two-story home with twenty-six (26) bedrooms and twenty-four (24) bathrooms. There are no pools or known firearms on the premises. Infection Control: LPA observed that hygiene and cleaning supplies were available for regular facility maintenance. The facility has infection control plan in file. Physical Plant: The physical plant, including floors, windows, and doors, was clean and well maintained. Fixtures and furniture were in good repair. The outdoor area was free of hazards. Laundry equipment was in good working condition. Sharp and dangerous objects were securely locked and inaccessible to residents. Both the smoke detector and carbon monoxide detector signal system were operational, and the hot water temperature was 117°F. Fire extinguishers located throughout the facility and have current inspection tags (1/27/2026). Food Service: The facility’s kitchen was clean and equipped to prepare food. The facility maintained the required two-day supply of perishable foods and a seven-day supply of non-perishable foods. Continued on LIC809-C.... Care & Supervision/Administration: Adequate staff were present to supervise clients during the visit. The administrator holds a current administrator’s certificate. Record Review and Resident/Staff Files: LPA reviewed files for six (6) staff members, confirming criminal clearances, updated training, and CPR/First Aid certification. Six (6)resident files were reviewed and contained all the required documents. Health-Related Services/Incidental Medical Services: All resident medications were securely locked in hallway medication carts on each floor. LPA reviewed medications for six (6) residents, confirming that all medications were listed on the Medication Administration Record (MAR) and accounted for. Personal & Incidental Funds: Disaster Preparedness: LPA reviewed the facility’s emergency and disaster plan, including documentation of the last emergency drill conducted on 12-01-2025, which met department requirements. All facility exits were clear of obstructions. No deficiencies were cited during the visit. Type of visit: Case Management - Other

On March 06, 2026, Licensing Program Analyst (LPA), Venus Mixson arrived at the facility unannounced and met with the Administrator, Michele Fuhrman. LPA introduced herself and stated the purpose of the visit. Currently at the facility are 48 residents and about 25 staff. LPA toured the facility along with the Director and made observations. There were no observable Health and Safety violations observed during todays visit. During todays visit LPA conducted a tour of the facility along with the Director, and made observations pertaining to the requested increase in capacity. The Fire Safety inspection request was received in the Regional Office and the listed facility is cleared for the requested increase. The Fire inspection was completed on February 10, 2026. Based on information obtained during todays visit there were no observable Health and Safety violations noted during today's visit, and the facility is large enough for the increase requested. Licensing Program Analyst, (LPA) Janira Arreola made an announced visit to the facility for the purpose of conducting a follow up pre-licensing inspection. LPA met with Administrator, Michelle Fuhrman, who was informed on the purpose of the visit. LPA conducted a tour, record review and interviews during the time of the visit. There are currently residents in care. The applicant has updated their application and has been approved by the local fire jurisdiction for (42) non ambulatory and (6) bedridden residents in the main building. LPA reviewed the facility sketch and observed the building and rooms. LPA observed, emergency exits, fire extinguishers, and smoke and carbon monoxide alarms meet the department requirements. LPA observed rooms that would be designated for bedridden residents with second exit doors. LPA observed the exit route around the building leading to the parking lot. On 12/16/2025 the second building at the facility was under construction and had not been yet approved by the local fire jurisdiction. LPA conducted an inspection of the second building on today’s date and observed construction is not yet completed. The administrator informed that the second building is currently being renovated to be attached the main building. The administrator attempted to contact the applicant during the time of the visit, but was unable to. The administrator was informed that at this time the main building is approved, and the attachment is not approved by the department to house residents at this time. Additional follow up may be needed in order to move forward with the licensing process if the additional of the second building would like to be included. Licensing Program Analyst, (LPA) Armando Perez made an announced visit to the facility for the purpose of conducting a pre-licensing inspection. LPA met with Administrator Michele Fuhrman, who accompanied LPA for the inspection. Applicant has submitted an application for forty eight (48) ambulatory residents. On December 6, 2024, Vista Fire Department approved a fire clearance for the facility. The facility is a two story facility with 20 client bedrooms, living room, dining area, kitchen, and activities room. LPA observed clients’ bedrooms with the required bedding and furniture, such as, clean mattresses/linen, night stands, dressers, chairs, lighting, and emergency lighting. Client bathrooms and showers had clean appliances that were operating in safe and sanitary condition and had the grab bars in toilets and showers. Facility kitchen had the ability to prepare food in clean environment and possessed equipment in good working condition. The facility met the required 2-day supply of perishable and 7-day supply of non-perishable foods. Sharps, such as knives and scissors, will be stored in the kitchen with the entrance to the kitchen be locked and not accessible to clients. LPA observed an adjacent structure that is currently under construction. According to administrator, additional rooms will be added. The floor plan will need to be updated to include the structure and fire clearance will need to reflect the approved number of clients when completed. Due to construction, the water heater and laundry has been temporarily relocated in the back yard in an enclosed shed. LPA inspected the water temperature and observed the temperature at 103.4. Client and personnel files will be in a locked closet in the administrators office on the first floor. Client medication will be centrally stored and locked in a designated room on the first floor. The facility has no pool or any bodies of water. There is a covered patio area with seating for all the clients. All passageways were free from obstruction. LPA observed fire extinguishers in the facility with the last service date of March 28, 2024. The smoke detectors and carbon monoxide alarms were operational and recently tested by Vista Fire Department. The facility does not have any known firearms and ammunition on the property. LPA observed the required postings of the emergency disaster plan, resident personal rights, complaint procedures, employee rights, visitation rights, facility sketch, and the Long-Term Care Ombudsman poster. Facility contains emergency supplies and first aid kits with the required items. The facility has working telephone for client use. LPA observed Applicant's Administrator Certification on file with expiration date of 10/13/2026. LPA observed that the physical plant is clean, in good repair, and to be hazard-free during today’s visit. LPA determined the facility meets the operational requirements for licensure. The Pre-licensing inspection is complete and this facility has no issues or concerns. Once construction is completed and updated floor plan and fire clerance will need to be submitted before accepting clients into that structure. The facility has satisfied all requirements in accordance with Title 22, California Code of Regulations. Applicant/administrator participated in COMP II at CAB via telephone call with analyst at CAB. Identification of the applicant and administrator was verified by confirming driver’s license number. During COMP II, applicant and administrator confirmed the understanding of Title 22. Component II was successfully completed. Applicant and administrator were advised to email/fax signed LIC 809 with copy of photo ID to CAB. During COMP II, CAB analyst confirmed Applicant/Administrator’s understanding of following areas: 1.
