On 10/22/25 Licensing Program Analyst (LPA) J. Leffall arrived unannounced to conduct an Annual Inspection. LPA introduced self, stated the purpose of the visit, and was greeted by Staff (S1) Maria Resendiz. LPA was granted entry. All 6 residents were present during inspection. Administrator Charito Serrano (A1) was called and arrived shortly after LPA’s arrival. LPA toured facility with S1. The facility was observed to be at a comfortable temperature, clean, in good repair, and no passageway obstructions or fire hazards were observed inside. An adequate supply of perishable and non-perishable food was observed. Freezer temperature was maintained at -20 degrees F and refrigerator temperature was maintained at 30 degrees F. Fire extinguisher was observed with a purchase date of: 10/110/24. No Fire drill conducted quarterly. Washer and dryer observed operational during visit. Carbon monoxide not observed in the facility. Smoke detectors were tested and observed to be operational. Residents' bedrooms were toured and observed to be adequately furnished with bed, dresser, and adequate lighting. All bathrooms are toured and observed to be operational. Hot water temperature was tested at a range of 106 degrees F and 119.4 degrees F in bathroom 3 out of 4 bathrooms. Bathroom 2 water temperature tested at 124.8 degrees F. Non-skid mat and grab bars observed in bathrooms. Outside of facility toured. Side gate was self-closing and self-latching. Outside was observed with adequate outdoor seatings available for residents. All medications were checked and observed kept locked in the hall closet. Residents’ MARS was reviewed. First aide kit observed with all of the required items. Both resident’s and staff files had multiple missing documents. Deficiencies are being cited on the attached 809D in accordance with California Code of Regulations, Title 22, Division 6. Exit Interview conducted. The following documents requested to be updated and submitted to Fresno CCL by 11/5/25: Lic 308, Lic 500, Lic 610E, Current Liability Insurance and current Administrator’s certificate. On 10/15/2024, Licensing Program Analyst (LPA) V. Gorban arrived unannounced at the above facility to conduct an Annual Inspection. LPA introduced self, stated the purpose of the visit, and was granted entry to the facility. LPA toured the facility with Administrator Charito Serrano. The facility was observed to be at a comfortable temperature, of 75 degrees F. Facility is free of debris, in good repair, and no passageway obstructions or fire hazards were observed. Common areas were properly furnished and well-lit throughout. During the visit the facility residents were resting after lunch. Department phone number and infection prevention information signs were posted thought the facility. Inspecting kitchen LPA observed the required 7-day supply of non-perishable food and 2- day supply of fresh perishables to be properly stored. An emergency disaster supply was observed. A fire extinguisher was observed with service date of 10/10/2024. All private residents’ bedroom observed to be at comfortable temperatures. The bathroom’s water temperature was tested and recorded reading of 109 degrees F. Medications records storage observed to be locked in a cabinet in a hallway. Cleaning supplies were observed to be in a locked cabinet in the storage it the laundry room. An outdoor seating area was observed operational for residents in care. LPA reviewed staff and residents’ files. Exit interview conducted. On 12/13/2023, Licensing Program Analyst(LPA) D. Ayers arrived unannounced at the facility to conduct a Required Annual Inspection. LPA met with Administrator Charito Serrano and announced the purpose of the inspection. At the time of inspection there were six residents residing in the facility. LPA toured the facility inside and outside. Pathways and doors were clear and free from obstruction. Smoke-detectors and carbon-monoxide detectors were present and operational. Facility was clean and odor free. LPA observed sufficient amount of perishable and non-perishable foodstuffs. Common areas were clean, adequately furnished, and adequately lit. Resident bedrooms were clean and had required minimum furnishings. Resident bathrooms were clean, odor free, and water temperature was within required temperature range. Bathrooms had required non-skid mats and grab bars. Sharp items and detergents were secured in a locked cabinet in the kitchen. Medications were secured in a locked cabinet in the kitchen, and medications appeared to be administered properly. The fence had a self-locking latch mechanism, and there was adequate outdoor seating for residents. LPA observed a a supply of clean linens and extra hygiene items for residents. LPA observed sufficient supply of personal protective equipment. LPA reviewed facility plan of operations and emergency disaster plan. LPA reviewed staff and resident files. LPA requested the following documents: LIC 500 Personnel Report, LIC 308 Designation of Administrative Responsibility, LIC 610-E the Emergency Disaster Plan, proof of liability insurance, and a copy of current Administrator’s Certificate to update the facility file.
