Licensing Program Analyst (LPA) Marcella Tarin conducted an unannounced Required-1 Year inspection. LPA met with Licensee Cecilia Sullivan and Designated Staff S1 Kimberly Sullivan. LPA stated the purpose of the visit. LPA toured the interior and exterior of the facility with S1 to include the kitchen, resident rooms, dining room, bathrooms, back and front of the facility. All exit and passageways were free and clear of obstruction. LPA toured the kitchen area and observed a perishable food supply of at least two days and a non-perishable food supply of at least seven days. LPA observed the refrigerator temperature at 39.9 F and Freezer at 0 F. LPA observed the medication storage area, knives storage area, and cleaning product storage area as locked and inaccessible to residents in care. The facility was equipped with smoke and carbon monoxide detectors. All smoke detectors functioned properly when tested by S1. Fire extinguishers were last serviced on 8/17/2026. LPA reviewed the facility first aid kit, and it was observed to be complete. The facility emergency drill log was reviewed. The facility's last drill was conducted on 8/9/2026. LPA toured 2 resident bedrooms, rooms observed with a bed, functioning lights, dresser/table, bedding and space for personal belongings. Page 1 of 2 LPA toured 3 bathrooms, and observed each bathroom with hand soap, paper towels, functioning lights, and covered trash bins. LPA measured water temperature with a range of 106.2 F to 110.1 F. LPA reviewed 2 resident records. LPA reviewed 2 resident’s Centrally Stored Medication and Destruction Records (CSMDRs). LPA reviewed 2 staff records. No deficiencies were cited during today's visit per California Code of Regulations Title 22. LPA toured the interior and exterior of the facility with ADM to include the kitchen, resident rooms, dining room, bathrooms, garage, back and front of the facility. All exit and passageways were free and clear of obstruction. LPA toured the kitchen area and observed a perishable food supply of at least two days and a non-perishable food supply of at least seven days. LPA observed the medication storage area, knives storage area, and cleaning product storage areas as locked and inaccessible to residents in care. The facility was equipped with smoke and carbon monoxide detectors. All smoke detectors functioned properly when tested by ADM. Fire extinguishers were last serviced on 12/12/2024. LPA reviewed the facility first aid kit, and it was observed to be complete. The facility emergency drill log was reviewed. The facility's last drill was on 7/14/2025. LPA toured 4 resident bedrooms. All 4 resident rooms have a bed, functioning lights, dresser/table, bedding and space for personal belongings. Page 1 of 2 LPA toured 2 resident bathrooms. All 2 bathrooms had hand soap, paper towels, functioning lights, and covered trash bins. LPA measured water temperature with a range of 108.3 degrees F to 111.9 degrees F. LPA reviewed 2 resident records. Resident records included emergency contact information, physician’s report, needs and service plans, and personal rights. LPA reviewed 2 resident’s Centrally Stored Medication and Destruction Records (CSMDRs). LPA reviewed 2 staff records. Staff records included personnel records and staff training. LPA also shared PIN 25-06-ASC and PIN 25-05-ASC and provided a copy of the Department's updated LIC 602A. No deficiencies were cited during today's visit per California Code of Regulations Title 22. Licensing Program Analyst (LPA) Steve Chang conducted an unannounced annual inspection visit, and met with House Manager (HM) Kim Sullivan. LPA observed 2 staff and 4 residents in the facility. LPA reviewed 2 resident files and 2 staff files. The centrally stored medication forms of 2 resident files were observed inaccurate and not maintained up to date. 2 out of 2 staff files were observed without valid first aid certificate. LPA toured the facility inside out with HM. License, ADM certificate and Personal rights posters were observed in the facility. Living room, kitchen, dinning room and two restrooms were inspected. 3 resident bedrooms, 2 staff live-in rooms and laundry area were inspected. Two staff live-in rooms was observed in facility. Medication closet was observed locked. Knives closet was observed unlocked, HM locked it immediately. The cleaning product closet under the sink was observed unlocked, HM added a lock before LPA finished the inspection. Room temperature was at 70 degree F, and hot water temperature was at 106 degree F in facility. First Aid box, flash lights and night lights were observed in the facility. The last time the facility conducted the emergency drill is 7/6/2024. Two days perishable food supplies and seven days nonperishable food supplies were observed sufficient. Fire extinguisher was serviced on 11/27/2023. The facility was equipped with fire alarm system, smoke and carbon monoxide detectors. Smoke detectors was tested by HM, and were working fine. Front yard and backyard were inspected. There was no obstruction to block the walkways and exit. The detached garage was as storage room. Back patio, and side patios were observed at backyard.
