Licensing Program Analyst (LPA) V Gorban conducted an unannounced visit today for the facility’s annual inspection. LPA met with administrator Julie Galeano, stated the purpose of the visit and as allowed entry in to the facility. There are currently 6 residents who reside at this home and there are two of them with dementia diagnose. LPA inspected the interior and the exterior of the facility including the common living spaces, resident bedrooms and bathrooms, medication storage in the kitchen, garage and outdoor areas. Bedrooms were clean and in good repair. There is a locked storage for medications. Food supply is adequate for 2-day perishable and 7-day nonperishable. Fire extinguisher service date recorded on 12/2/2025 with emergency fire drill recorded in December, 2024 . Smoke alarms were tested and are operational. The home has a carbon monoxide detector. Water temperature was tested at 110 degrees. First Aid kit is on site. LPA reviewed facility Plan of Operations, Infection Control Plan, and Emergency Disaster Plan. LPA reviewed facility staff files, and 4 facility resident files. Resident 2 has expired medical assessment. Incomplete Plan Of Operations, Infection Control , And Emergency Disaster Plan. Resident 2 does not have annually updated needs and services plan. Emergency Disaster Plan needs to be updated and more specific/updated. Infection Control Plan needs to be more specific/updated. Staff missing required files. Facility does not have Plan of Operation available for review. On 11/07/2024, Licensing Program Analyst (LPA) V. Gorban arrived unannounced at the above facility to conduct an Annual Inspection. LPA introduced self, stated the purpose of the visit, and was granted entry to the facility. LPA toured the facility with lead staff Julie Galeano. The facility was observed to be at a comfortable temperature, of 77 degrees F. Facility is free of debris, in good repair, and no passageway obstructions or fire hazards were observed. Common areas were properly furnished and well-lit throughout. During the visit the facility residents were resting after lunch. Department phone number and infection prevention information signs were posted thought the facility. Inspecting kitchen LPA observed the required 7-day supply of non-perishable food and 2- day supply of fresh perishables to be properly stored. An emergency disaster supply was observed. A fire extinguisher was observed with service date of 12/20/2023. All private residents’ 6 bedroom observed to be at comfortable temperatures. The bathroom’s water temperature was tested and recorded reading of 112 degrees F. Medications records storage observed to be locked in a cabinet in a hallway. Cleaning supplies were observed to be in a locked cabinet in the storage it the laundry room. An outdoor seating area was observed operational for residents in care. Type of visit: Case Management - Health Checks

On 5/14/2024, Licensing Program Analyst(LPA) D. Ayers arrived unannounced to conduct a case management inspection. LPA met with Caregiver Julia Galeano and announced the purpose of the inspection. LPA conducted the inspection to verify the status of the facility and ensure the health and safety of residents in care. There was one resident and one staff member at the time of inspection. LPA toured the facility inside and outside. No deficiencies were cited during the inspection. On 12/5/2023, Licensing Program Analyst(LPA) D. Ayers arrived unannounced at the facility to conduct a Required Annual Inspection. LPA met with Administrator Cyrul Tupino and announced the purpose of the inspection. At the time of inspection there were six residents residing in the facility, three of whom were receiving hospice care services.. LPA toured the facility inside and outside. Pathways and doors were clear and free from obstruction. Smoke-detectors and carbon-monoxide detectors were present and operational. Facility was clean and odor free. LPA observed sufficient amount of perishable and non-perishable foodstuffs. Common areas were clean, adequately furnished, and adequately lit. Resident bedrooms were clean and had required minimum furnishings. Resident bathrooms were clean, odor free, and water temperature was within required temperature range. Sharp items and detergents were secured in a locked cabinet in the kitchen. Medications were secured in a locked cabinet in the kitchen, and medications appeared to be administered properly. The fence had a self-locking latch mechanism, and there was adequate outdoor seating for residents. LPA observed a a supply of clean linens and extra hygiene items for residents. LPA reviewed facility plan of operations and emergency disaster plan. LPA reviewed a sample of staff and resident files. Files contained required documantation and records. LPA requested the following documents: LIC 500 Personnel Report, LIC 308 Designation of Administrative Responsibility, LIC 610-E the Emergency Disaster Plan, proof of liability insurance, and a copy of current Administrator’s Certificate to update the facility file.
