At 12:10 PM, Licensing Program Analyst (LPA) Bernadette Allen conducted an unannounced visit to conduct an annual inspection. LPA Allen was greeted by Estela Nicdao Support Staff upon arrival. LPA introduced herself and explained the purpose of the visit. At approximately 12:45 PM, Teresa Guanlao- Administrator arrived and she was informed of the purpose of the visit. The facility is licensed to serve (6) elderly adults ages 60 and above, of which (6) can be bedridden. The facility has an approved hospice waiver for (2). The facility is a single-story structure located in a residential neighborhood and consists of (4) bedrooms, (2-1/2) full bathrooms, shaded back yard, laundry room and a detached garage and a pool in the backyard with required fencing. At 12:20PM, LPA reviewed three (3) staff files for First Aid/CPR certification, criminal record clearance, training, and health screenings which appeared to be current. LPA reviewed six (6) residents files for admission agreements, updated physician reports, and needs and services plans which appeared to be current. The first aid kit and manual were checked and fully stocked the last fire drill was conducted on July 18,2026. At 1:00PM, LPA observed that the four (4) bedrooms have the required furniture which appeared to be in good repair. The rooms also have adequate lighting, and storage for the residents’ personal belongings The 2 1/2 bathrooms toilets and water faucets worked properly, shower was free of mold/mildew, and there were sufficient toiletries accessible to clients. The water temperature properly measured between 105-120 F. LPA toured the kitchen and dining area and there was sufficient seating for dining. LPA observed a 5-day supply of perishables and a 7-day supply of non-perishable food available which was adequately maintained. The fire extinguishers were charged, and the smoke and carbon monoxide detectors were tested and were operable. Overall, the facility appeared to be clean, sanitary, and in good repair All window screens are clean and in good repair. Based on the observations made during today’s visit, no deficiencies were cited. On 08/15/25 Licensing Program Analyst (LPA) Villegasz conducted an unannounced annual required visit using the CARE Inspection Tool. LPA met with Teresa Guanlao, as the purpose of the visit was explained. The facility is licensed to serve (6) elderly adults ages 60 and above, of which (6) can be bedridden. The facility has an approved hospice waiver for (2). Fenced pool on premises. Facility fees are current. The facility is a single-story structure located in a residential neighborhood and consists of; (4) bedrooms, (2-1/2) full bathrooms, shaded back yard, laundry room and a detached 2 car garage and a pool in the backyard with required fencing. Bedrooms were checked, mattresses and box springs were in good condition, adequate lighting, plenty of dresser and closet space was observed. Bathroom toilets and water faucets worked properly, shower was free of mold/mildew, and there were sufficient toiletries accessible to clients. The water temperature properly measured between 105-120 F. A supply of perishable and non-perishable food was observed, toxins and knives were stored and inaccessible to residents, no firearms on the premises, exits and walkways are free of debris/hazards. LPA conducted a records review of 3 staff records, 6 resident records, and 3 medication administration records. Medications were centrally stored and properly locked, first aid kit was checked and fully stocked. The last fire was conducted on 08/03/25, fire extinguishers fully charged, carbon monoxide and smoke detectors are interconnected and operational. On 6/13/2024, Licensing Program Analyst (LPA) Alfonso Iniguez conducted an unannounced annual required visit using the CARE Inspection Tool. LPA met with Teresa Guanlao /Licensee. LPA explained the purpose of today’s visit. The facility is licensed to serve (6) elderly adults ages 60 and above, of which (6) can be bedridden. The facility has an approved hospice waiver for (2). Fenced pool on premises. The facility is a single-story structure located in a residential neighborhood. It consists of (4) bedrooms, (2-1/2) full bathrooms, shaded back yard, front yard, laundry room and a detached 2 car garage and a pool in the backyard. LPA Iniguez and the facility staff toured the physical plant. There is a gated pool in the backyard that was securely locked, no obstructions on the premises. LPA inspected a total of (4) bedrooms and (2) bathrooms. The beds and bedding supplies were in good condition, adequate lighting was provided, and storage for the residents’ personal belongings was observed. The bathrooms were found to be within Title 22 regulations and were operational. Smoke and carbon monoxide detectors were in operable condition. The room temperature ranged from 76°F to 78°F. The evaluation Report continues on the next page, LIC 809-C, providing further details of the inspection findings. During the visit, LPA Iniguez observed that the facility was clean, sanitary, and appropriately furnished. Storage areas for personal hygiene were in place. Cleaning supplies and toxins were stored in a way that made them inaccessible to residents in care. The kitchen was inspected, and there was sufficient perishable and non-perishable food available, which was adequately maintained. All fire extinguishers were charged and operable. The last Fire/Disaster Drills were conducted on 4/4/24. A review of (3) residents' service files and (2) staff personnel files were maintained in order. LPA reviewed (3) Medication Administration Records (MARs). LPA observed the facility's infection control practices. All mandated inspection control posters were displayed throughout the facility. A copy of liability insurance will be emailed to LPA. Facility Annual Fess current. Deficiency cited under California Code of Regulations, Title 22, Division 6, Chapter 8. See details below: -Water temperature at kitchen sink reach over 120F°(122F°). - Sharp objects not locked properly (Knives found unlocked on kitchen area during facility tour). -Discrepancies found on Medication Administration Records (MAR)s for R#2 and R#3 in the month of May 2024. -Slide latch locks place on one of resident’s rooms and main entrance door. Note: *Citations not cleared by the due date will be a $100 fine assessed for each citation until it is cleared. On 11/18/23, Licensing Program Analyst (LPA) Antonine Richard conducted an unannounced Annual required visit to the above facility using the Care Inspection Tools. LPA was met by Teresa Guanlao. Administrator and the purpose of today’s visit was explained. There are currently (5) residents in the facility. (1) resident is ambulatory, (4) are non-ambulatory, (0) bedridden. The facility is a single-story structure located in a residential neighborhood. It consists of (4) bedrooms, (2-1/2) full bathrooms, shaded back yard, front yard, laundry room and a detached 2 garage. LPA and Administrator toured the entire facility inside and out. Documents are posted as mandated. Bedrooms 1-4 are occupied by residents and contain the mandated furniture. The (1-1/2) bathrooms have grab bars and non-skid mats and are clean and operational; smoke detectors and carbon monoxide detector were in compliance and operational. No firearms are stored at facility and no bodies of water present. Medications are stored, locked and inaccessible to residents. Residents file along with medications are current. Ample supply of perishable and nonperishable food. The hot water temperature is (112. 9F) degrees Fahrenheit. Linens and personal hygiene supplies are adequate, hazardous toxins and/or sharp items are inaccessible to residents, (1) fire extinguisher is fully charged. Exit, walkways and/or passageways, front and back yard are free of debris and/or hazards. The facility is in good repair. During the visit, LPA observed the facility infection control practices. LPA observed a sanitizing station at the facility entry & visitors and temperatures are logged and checked, sanitizer/soap, paper towels, in all the bathrooms and additional sanitation supplies are stored in the garage. LPA observed staff wearing masks. PPE's are enough for 30 days. The last fire drill was on 11/03/2023. The administrator certificate expired on 12/06/2023.
