Allegation investigated: Facility staff do not have the required training

At approximately 09:30AM, Licensing Program Analysts (LPA) Deniz arrived unannounced to continue a complaint Investigation and delivered the findings regarding the above allegation and met with Licensee, Ayodeji Sonoiki. During the investigation, the Department requested and reviewed documents, conducted interviews, and made observations. The following allegation was investigated, “Facility staff do not have the required training” The complaint alleged that facility staff do not have the required training. The Reporting Party (RP) stated that they were not provided hands-on training or the opportunity to shadow another staff member before working with residents. The RP reported resigning the same day of hire and expressed concern that newly hired staff may not receive the required training before providing resident care. During the investigation, LPA conducted interview with staff and reviewed the staff training records. LPA learned that the facility utilizes Community Care Options to provide online staff training. Continued on LIC9099-C page... Unsubstantiated Continued from LIC9099 page... LPA reviewed training records of three (3) staff members and verified that each completed more than 20 hours of online training and 16 hours of hands-on training before working independently. Training records documented completion of all required training. LPA also interviewed a staff member, who confirmed receiving the required training prior to working independently. LPA did not identify evidence to support the allegation that the facility failed to provide required staff training. Based on the statements received, observations made, and documents reviewed this allegation is unsubstantiated. A finding that the complaint allegation of Staff threatening resident is UNSUBSTANTIATED meaning that although the allegations may have happened there is not a preponderance of evidence to prove that the allegation occurred. Aug 6, 2026Facility evaluation reportReport on file

Type of visit: Required - 1 Year

On 08/06/2026, at approximately 9:30 a.m., Licensing Program Analyst (LPA) Ali Deniz arrived unannounced to conduct a one-year required inspection of this licensed Residential Care Facility for the Elderly (RCFE). The LPA was greeted by House Manager Kiara Collins. Licensee Ayodeji Sonoiki arrived later at approximately 12:30 p.m. The facility has an approved fire clearance for a capacity of six (6) residents, including four (4) ambulatory and two (2) non-ambulatory residents, and an approved hospice waiver for two (2) residents. At the time of the visit, there were two (2) residents in care and one (1) staff member on duty. The facility is a single-story home consisting of three (3) resident bedrooms, two (2) bathrooms, and common living areas. The LPA reviewed the facility's staff roster and verified that all staff present during the inspection were fingerprint cleared and associated with the facility as required by regulation. At approximately 11:00 AM, LPA toured the building and grounds which was found to be clean and in good repair. LPA observed all walkways and exits to be unobstructed. All notices that are required to be posted have been posted and are in a highly visible area. LPA observed activity supplies for resident use. The amount of fresh and nonperishable foods is within regulation. Facility kitchen, refrigerators and freezers were clean, and food was stored properly. LPA Toxins are stored inaccessible to residents. Water temperature measured 106.2 degrees F which is within regulation between 105- and 120-degrees F at faucets accessible to residents. Fire Extinguishers found to be last bought in August 2025 at the time of visit. Carbon Monoxide and smoke detectors were present and in order. There was enough lighting in all common areas, resident rooms, and hallways. Medication is centrally stored and secure. Continued on LIC809-C... Continued from LIC809… LPA reviewed two (2) resident records and found all residents have care plans, admission agreements, and physician's report on file. Resident medication records are thorough and MAR recorded online. LPA reviewed three (3) staff members. All staff members had completed the required initial training. However, the facility failed to obtain current First Aid and CPR certifications for two (2) of the three (3) staff members and failed to maintain a completed/signed LIC 503 (Health Screening Report), for all staff members (technical violation was issued). The Licensee was instructed to submit copies of current First Aid and CPR certifications for all staff, along with completed LIC 503 Health Screening Reports, to Community Care Licensing by 08/16/2026. The Administrator Certificate for Ayodeji Sonoiki (Certificate No. 6078215740) expires on 08/13/2027. Facility has supplies enough to operate for more than 72 hours in an emergency. Facility failed to record and document a disaster drill quarterly (Technical Violation Given). Updated copies of the following documents were requested for facility file and are to be submitted to CCL by due date of 08/16/2026: -LIC 308 Designation of Facility Responsibilities -LIC 500 Personnel Summary -LIC 610 Emergency Disaster Plan (Last page) -LIC 9020 Register of Facility Client’s/Resident’s -Copy/Proof of Updated Certificate of Liability Insurance No deficiencies were observed in the areas inspected, No citations were issued during today’s visit. Type of visit: Prelicensing

On 08/15/2025 at approximately 11:30AM Licensing Program Analyst (LPA) Ali Deniz conducted a pre-licensing inspection and was greeted by Applicant, Ayodeji Sonoiki. This pre-licensing inspection is being conducted for an initial licensing. The facility is a single-story home with 4 resident rooms, 4 bathrooms, and common areas. Facility received an approved fire clearance dated 07/09/2025 that allows for a total capacity of 6 clients, where 4 can be ambulatory and 2 can be non-ambulatory. The facility currently has no clients in care at the time of this visit. LPA conducted a tour and inspection of the indoor and outdoor portions of the facility. The facility was found to be clean and comfortable temperature with bedroom doors free from obstruction. Facility has an Infection Control plan on file. LPA observed Fire extinguishers located on the kitchen wall which were bought new on 08/11/2025 and gauge were in green zone that indicates the fire extinguisher pressurized properly and ready to use. Cleaning products, sharp items, other toxins and chemicals are kept in the kitchen cabinets and laundry room were not secure and did not have lock. The applicant agrees to submit proof that all sharp and toxin item cabinets are secured and locked prior to having clients move into the facility. Proof to be submitted to Community Care Licensing (CCL) by 08/19/2025. Additional supply of hygiene products available to clients. Water was measured at 106.3- and 109.5-degrees F in faucets used by clients which is within regulation between 105- & 120-degrees F. LPA observed one all bathrooms did have grab-bars and non-slip bathmats. Mattress pads were in place or available for client use. All emergency exits were found to be unobstructed. Facility has combination smoke and carbon monoxide detectors which were operational during visit. LPA confirmed that the contents of the facility's First Aid Kit were sufficient. The facility had emergency lights, and emergency water supplies were available on site. LPA also observed facility had 6 emergency backpacks for any emergency. Continued on LIC809-C.. Continued from LIC809.. The facility has an adequate supply of kitchen tools. Per conversation with applicant, sharps such as knives will be stored and locked in the facility all the time. LPA observed a large medication cabinet with lock in the facility dining room. Per conversation with applicant, client medications will be locked all the time. LPA observed the required postings including emergency disaster plan, the Let Us Know Complaint poster and See Something Say Something poster. RCFE Component III was reviewed with Applicant. No Deficiencies or Advisories given during visit. Pre-Licensing completed. Facility is ready to be Licensed as an Residential Care Facility for Elderly (RCFE). LPA will submit Pre-Licensing Application Report to the Application Unit Analyst in Sacramento. Application Unit Analyst will notify Applicants of Status. Exit interview conducted.
