Licensing Program Analyst (LPA) Contreras arrived unannounced to conduct a required annual inspection and was greeted by care givers Liezyl Leano and Myrna Anis. Administrator (admin) Imee Villegas was unable to attend. Facility is an Adult Residential Care Facility for the Elderly with an approved fire clearance for capacity of 6 (six) residents. There are currently (5) five residents in care. LPA toured the building and grounds which was found to be clean and at a comfortable temperature. LPA observed all walkways and exits to be unobstructed. All required postings were in a highly visible area. Fire extinguishers were charged and last inspected 6/02/2026. Fire alarm and carbon monoxide alarms all tested and operational. Water temperature check measured within regulation at faucets accessible to residents within the allowable range of 105 to 120 degrees F. Resident bathrooms observed to have bath mat and grab bar. Disaster drills are being conducted quarterly. All bedrooms were equipped with required furnishing. Facility had an ample supply of linens, towels and extra hygiene products for residents. LPA observed at least a 2 day supply of perishable and 7 day supply of non-perishable foods. Snacks accessible for residents. Emergency food was observed to meet the 72 hour emergency supply. Facility kitchen, refrigerators and freezers were clean, and food stored properly with expiration dates noted. Toxins are locked and inaccessible to residents. Sharps and knives were locked. Phone accessible for resident use. Upon walk through, LPA observed refrigerated medication fridge not locked and secure(Deficiency Cited, see 809-D). LPA observed various medications not in its original container with names of residents on containers. Medications were unlabeled and poured into plastic daily pill organizers (Deficiency cited, see 809-D). continued onto 809-C.... continued from 809.... LPA conducted a file review for 5 out of 5 residents and 3 staff. All required documentation found to be up to date. LPA and staff conducted a spot check of medication and medication records, all records up to date. Updated copies of the following documents were requested for facility file and are to be submitted to CCL within 30 days of this visit: LIC500- Personnel Report LIC308- Designation of Responsibility Liability Insurance Deficiencies are cited from the California Code of Regulations, Title 22, Division 6 of California Regulation and the Health and Safety Code. At approximately 12:50 PM, Licensing Program Analyst (LPA) Elias Magdaleno arrived unannounced to conduct a required 1-year annual inspection and was greeted by staff. Administrator Imee Villegas was contacted and arrived at approximately 1:10 PM. Facility is a Residential Care Facility for the Elderly (RCFE) with five (5) residents in care. Facility has a Dementia Care Plan, a Hospice waiver for two (2), with two(2) Hospice residents currently in care and is approved for all non-ambulatory residents. At approximately 1:20 PM, LPA initiated a tour of the facility with Administrator and observed the following: Facility is a one (1) story home, was a comfortable temperature, and passageways were free from obstructions. Fire extinguishers were last inspected 6/25. Smoke and Carbon Monoxide detectors were tested and operational during inspection. Water temperature measured 107.4- and 106.3 degrees F, which is within the allowable range of 105 to 120 degrees F per Title 22 regulations. LPA observed a supply of clean linens, hygiene, incontinent care, and paper products available for residents. Residents' bedrooms were inspected and observed to have all the appropriate furnishings as outlined in Title 22 regulations. Cabinets containing cleaning supplies and other items that could pose a risk were locked. LPA observed at least a two (2) day supply of perishable and seven (7) day supply of non-perishable food, as well as an emergency water supply. Food was found to be stored in a safe manner with open items covered. There is a shaded seating area in the backyard with outdoor space for activities. Facility has an internet access devise and internet available to clients in care, and the phone was observed operational during today's inspection. LPA observed staff regularly checking on residents and fulfilling their care requests. Continued LIC809C... Continued from LIC809... Facility conducts monthly disaster drills, and the most recent drill was conducted 6/25. LPA observed facility's infection control plan and emergency disaster plan which was last updated 4/25. LPA observed a supply of PPE, emergency supplies, a first aid kit, and flashlights. At approximately 1:50 PM LPA conducted a review of five (5) resident records. All required documentation present. At approximately 2:15 PM LPA conducted review of two (2) staff records. All required documentation present. At approximately 2:30 PM LPA and caregiver conducted a spot check of medication and medication records. Medication is centrally stored and locked. At approximately 11:00 AM, Licensing Program Analyst (LPA) Stefanie Mutialu made an unannounced annual required inspection of this licensed senior care facility. LPA was greeted by Mirna Santiago,caregiver. Administrator, Imee Villegas arrived shortly after. The facility is a single story home licensed for six (6) non-ambulatory residents one can be bedridden and a hospice waiver capacity of two (2). The facility currently provides care for 4 residents. In addition, there is one resident on hospice and there are some residents with a diagnosis of dementia. At approximately 11:15 AM, LPA and Administrator toured the building and grounds which was found to be clean and in good repair. LPA observed all walkways and exits to be unobstructed. All notices that are required to be posted have been posted and are in a highly visible area. Resident’s bedrooms, common areas, kitchen & food storage areas were inspected. There was a sufficient supply of both perishable and nonperishable foods as required by Title 22 Regulations. Toxins are stored in a locked cabinet in the facility laundry room, garage, kitchen and in backyard. Sharps and other kitchen supplies that could pose danger if available to residents were found secured in the kitchen drawer and cabinet. There was a supply of cleaners, hygiene products and paper products available for residents. All resident’s bedrooms have lighting & appropriate furnishings. Water temperature measured within regulation between 118 and 120 degrees F at three of three faucets accessible to residents. One out of one fire extinguisher was inspected and charged. Seven out of seven Smoke detectors were present and Carbon Monoxide detector was present. There was enough lighting in all common areas, resident rooms, and hallways. Emergency disaster drills are conducted quarterly with the last emergency disaster drill conducted on 05/15/2024. Continued on LIC809-Ca Continued from 809 Facility provides a family atmosphere with games, puzzles,and television in living room. Each resident room is equipped with a television. The facility temperature measured 73 degrees with air conditioning running. Air purifiers are located throughout the facility. Resident on hospice had one caregiver providing one on one assistance. Medications located in designated cabinet were found to be secured. LPA conducted a spot check of medications and found all administering and records to be in order. Resident was observed interacting with staff in bedroom and living room. Two of four residents were found in their room. Two of four residents were observed in the living room socializing and watching television. LPA observed staff checking and caring for residents often. At approximately 12:15 PM, LPA reviewed Four of four resident records which were all found to be well organized, thorough and contained the required documentation. At approximately 1:00 PM , LPA reviewed three out of three staff records which were all found to be well organized, thorough and contained the required documentation. Medication records are thorough and contained physician's orders for each resident. Administrator Imee Villegas Administrator Certification 7009121740 is expired as of 06/09/2024. Administrator provided proof of required CU and recertification re-submission.
