Type of visit: Required - 1 Year
On 06/19/2026, Licensing Program Analyst (LPA) Jill Nakagawa arrived unannounced to conduct an Annual Required inspection and met with Paramjit Singh Sandhu, Administrator. LPA conducted a walk-through of the facility with Administrator and observed facility to be clean, well maintained and a comfortable temperature. At the time of inspection, there were 6 residents, 1 caregiver and the Administrator on site. Residents were clean and dressed appropriately. Residents' rooms were furnished per regulation. The fire extinguisher was last tested and charged on June 1, 2026. The smoke detectors and carbon monoxide detector system were operational, and are tested monthly. The last emergency drill was on June 1, 2026 and are done quarterly. Water temperature was within regulation. There was an ample supply of perishable and non-perishable foods, as well as an emergency supply. Medications were stored and locked inaccessible to residents. The backyard has a shaded area for seating and a cement labyrinth for walking. The residents enjoy watching the fruit trees change throughout the year. The facility has a dog which provides joy and enrichment. The Administrator provides a variety of activities for the residents including outings and providing supplies for residents' individual interests: sewing, musical instruments, gardening, games, trips to the beautician, church, Senior Center. LPA requested copy of: Proof of Liability Insurance Exit interview conducted with Administrator, whose signature on this document confirms receipt. On 06/20/2025, Licensing Program Analyst (LPA) Jill Nakagawa arrived unannounced to conduct an Annual Required inspection and met with Paramjit Singh Sandhu, Administrator. LPA conducted a walk-through of the facility with Administrator and observed facility to be clean, well maintained and a comfortable temperature. At the time of inspection, there were 6 residents, 1 caregiver and the Administrator on site. Residents were clean and dressed appropriately. Residents' rooms were furnished per regulation. Allegation investigated: Staff are not following resident's care plan
Licensing Program Analyst (LPA) Nakagawa arrived unannounced to complete an investigation and deliver findings regarding the above allegation. LPA requested documents, conducted interviews and made observations. LPA was met with Shova Panta at the facility. Administrator Paramjit Singh Sandhu was reached via phone to review the findings. LPA conducted an interview with resident R1. R1 stated they were satisfied with their care and supervision of medications. LPA asked them if they received their medications. R1 stated that the Administrator assisted with the pain management medications. S1 helped with the administration of medications including morphine. At the time of the interview R1 was coherent and cognizant and seemed to be in good spirits. LPA asked if visitations were occurring and R1 said yes. Continued on 9099.... Unsubstantiated Continued from 9099.... LPA conducted interviews with outside parties who stated that R1 appeared to be satisfied at the facility with their care. LPA asked outside parties if S1 had ever failed to allow hospice staff access to the facility and was informed no and that hospice staff had always been allowed access to their patients. LPA examined the medication records for R1 and found them to be in order. LPA was unable to examine the medications as they had been destroyed upon the passing of R1. MAR (Medication Administration Record) for R1 indicates that medications were administered as ordered. Refusals of medications are noted in the MAR. Based on LPA interviews, there was no one who was able to state any act of physical threats were made by S1. In addition, LPA was unable to locate mandated reporting records to substantiate acts of violence in violation of Title 22 Regulations. Based on the interviews conducted and the records reviewed, there was not evidence to meet the evidence standard therefore the allegation that staff are not following the resident’s care plan is unsubstantiated meaning that although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation(s) did or did not occur, therefore the allegation is unsubstantiated.the state’s words, verbatim · CDSS document, Jan 9, 2025 · control 21-AS-20240918153629

Allegation investigated: Personal Rights Facility in violation of fire clearance
Licensing Program Analyst (LPA) Nakagawa arrived unannounced on 08/13/2024 to conclude investigation and deliver findings on the above allegations. LPA met with Administrator, Paramjit Sandhu. The complaint alleges that Personal Rights were violated due to Staff (S1) not allowing privacy during outside visitors visiting with resident (R1). LPA Nakagawa conducted interviews and made observations. It was reported that during a visit on 7/5/2024 R1 and visitor (V1) were not allowed the right to privacy. S1 stated “that R1 had lots of visitors and there had only been one incident where S1 felt it necessary to intervene on behalf of R1 as R1 had shared to S1 and R2 that R1 did not want visitors at that time." Continued on 9099-C Unsubstantiated Continued from 9099..... Statements made by 2 of 3 individuals state R1 and the visitor (V1) were afforded a visit in private, but requested by S1 to be brief due to R1 being tired and the visit occurring after visiting hours. A short time later S1 requested that visitor (V1) leave and return during regular visiting hours. Based on LPA’s interviews the allegation that R1's Personal Rights were violated are unsubstantiated, although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation(s) did or did not occur, therefore the allegation is unsubstantiated. In addition, Licensing Program Analyst (LPA) Nakagawa also concluded the investigation and deliver findings on the allegation that Facility is in violation of fire clearance. LPA met with Administrator, Paramjit Sandhu. LPA Nakagawa reviewed documents, conducted interviews and made observations. The complaint alleges that the facility does not have a fire clearance for bedridden residents and believed that Resident (R1) was bedridden at the time of admission prior to the hospice placement. Review of admission agreement indicates R1 was admitted to the facility on 03/02/2024. The Physician’s Report dated on 03/01/2024 indicates that R1 was not bedridden nor were they bedbound prior to R1 beginning hospice care on 6/17/2024, therefore the allegation that Facility in violation of fire clearance is unsubstantiated. Type of visit: Case Management - Other
Licensing Program Analyst (LPA) Jill Nakagawa arrived unannounced to conduct an inspection and found the facility to be clean and at a comfortable temperature. There was (1) one resident receiving hospice services and (5) five other residents receiving varying levels of care. All residents were clean, comfortable and well-groomed. The Administrator and (1) one caregiver on site. Upon arrival, LPA noted that there was a great deal of heavy equipment in the area and road work being done in front of the facility. Licensee reported that he had talked with construction workers, to assure that he could leave facility when necessary/in an emergency. At the time of visit several residents were in the living room watching television. LPA was treated to a recital by one of the residents. (3) three other residents were resting in their rooms. LPA observed that the Licensee was doing some maintenance to the exterior of the facility, but walkways and back patio were clear and accessible. Licensee to inform LPA when maintenance and road construction are completed. No deficiencies were found at the time of inspection. No citations issued. Licensing Program Analyst (LPA) Jill Nakagawa arrived unannounced to conduct an Annual Required inspection and met with Paramjit Singh Sandhu, Administrator. LPA conducted a walk-through of the facility with Administrator and observed facility to be clean and well maintained and a comfortable temperature. Exterior walkways were clear of debris. Discussion was had with Administrator regarding removal and/or storage of extra equipment and building supplies currently outside. At the time of inspection, there were 6 residents and the Administrator on site. Residents were clean and dressed appropriately. Residents' rooms were furnished per regulation, with personal touches added to make the facility homey. The facility has a visitation area and provides virtual visits and phone calls for family to stay in contact with residents. The fire extinguisher was last tested and charged on May 1, 2024. The smoke detectors and carbon monoxide detector system were operational, and are tested monthly. The last emergency drill was on March 3, 2024 and are done quarterly. Water temperature measured 114.4 F. There was an ample supply of perishable and non-perishable foods, as well as an emergency supply. Medications were stored and locked. At the time of visit several residents were in the living room watching television. One resident went and got their accordion and played for the group. LPA reviewed records, conducted interviews and made observations. No deficiencies were found at the time of inspection. No citations issued.
