Allegation investigated: Staff do not feed residents food of good quality. Staff isolate residents in their rooms. Staff does not provide residents with activities. Resident developed pressure injuries while in care.

On 3/2/26, Licensing Program Analyst (LPA) Lavinia Muscan arrived at the facility unannounced to deliver complaint findings into the allegations listed above and met with Administrator Atanas Manev. During the investigation, the Department conducted interviews and reviewed documentation pertinent to the investigation. The results of the investigation are as follows: **Report continued on 9099-C** Unfounded Staff do not feed residents food of good quality. Based on staff interview, three resident interviews (3) and department observation of the kitchen and meal service, the department found that there was an adequate amount of food for the residents. The food appeared to look appetizing and nutritious, sanitation in the kitchen appeared appropriate, residents said food was good, and portions appeared plentiful. Food supplies in facility were adequate to meet the requirements. Currently, there is no evidence to suggest that staff have failed to provide adequate food service or provide it in a timely manner. Therefore, the allegation is UNFOUNDED. A finding of unfounded means that the allegation is false, could not have happened and/or is without a reasonable basis. Staff isolate residents in their rooms. Staff does not provide residents with activities. Through the course of the investigation process, CCL conducted interviews, toured the facility, and reviewed records regarding the allegation above. During the investigation, residents and staff interviews indicated that residents are receiving all ADLs, including activities from care staff. Residents indicated that if they need anything staff are there and ready to assist. Residents indicated that all their needs are being met by facility staff and they rarely stay in their rooms. Staff indicated that they have never observed other care staff not providing ADLs or activities to residents in care. Therefore, the above allegation is UNFOUNDED. A finding of unfounded means that the allegation is false, could not have happened and/or is without a reasonable basis. Resident developed pressure injuries while in care. Through the course of the investigation process, CCL conducted interviews, toured the facility, and reviewed records regarding the allegation above. Based on documentation and interviews, R1 developed a small superficial wound on R1’s back, which was not a pressure injury. The wound resulted from repeated scratching by R1; therefore, the above allegation is UNFOUNDED. Licensing Program Analyst (LPA) Lavinia Muscan arrived on 01/21/2026 to conduct the annual inspection. During today's annual inspection, the Compliance and Regulatory Enforcement Tool was used. LPA reviewed resident (3) and staff files (2). All resident files contained the required paperwork. All staff files contained the required paperwork. All staff have current first aid and CPR training. Facility was clean and well organized. Facility is current on fire drills. Staff training contained the required initial training. LPA and Administrator Atanas Manev toured the facility together to ensure the health and safety of residents in care. The areas toured included resident rooms, bathrooms, kitchen, and common areas and outside area.The food supply is within compliance, 2 days of perishable and 7 days worth of non-perishable food items. Grab bars were present at the toilet and in the shower. All exits were unobstructed. The disaster drill is current. The administrator's certificate is current. There is a side gate for emergency access. LPA checked the kitchen area for the ability to prepare and store food. Knives and Sharp objects found to be locked . LPA observed cleaning products and other toxins to be locked away. LPA observed the area used for medication to be locked and inaccessible to residents. LPA observed smoke detectors and carbon monoxide detector at the care home are operational. Fire extinguisher is ready for emergency use. Water temperature is within compliance. In the areas toured, there were no health or safety violations observed. LPA requested a copy of the LIC 500, LIC610E and current liability insurance to be sent to the Department by end of the month. On 2/22/2024, Licensing Program Analyst (LPA) Cassie Yang arrived unannounced at the facility to conduct a Required annual inspection utilizing the CARE tool. LPA met with Administrator and explained the purpose of the visit. During today's visit, LPA and Administrator conducted a tour of the interior and exterior of the facility. Areas toured included but not limited to: six private residents room, three caregiver rooms, kitchen, laundry room, backyard and the common areas. LPA observed the posted license to be incorrect to current licensure of hospice waiver for four. LPA informed Administrator a new license will be sent to the facility as soon as possible. LPA observed facility to have ample supply of perishable and non perishable foods. LPA observed medications, toxins and sharps to be stored separately and to be inaccessible to residents in care. LPA conducted a file review for three residents and three staff. LPA observed the required documents to be present in the binders. Administrator and LPA discussed there is no concerns at the facility. Administrator reported no concerns with staffing. CARE tool completed and facility was found to be in substantial compliance with California Code Regulation, Title 22.
