Allegation investigated: Refund not provided after resident passing

Licensing Program Analyst (LPA) Cassandra Mikkelson arrived unannounced to deliver findings to allegations listed above. LPA met with Maria Veres,during today’s visit. During today's inspection LPA conducted interviews, toured the facility and reviewed records pertinent to the investigation. ***Continued on 9099-C page** Unfounded Refund not provided after resident passing Interview conducted indicated that resident R1 moved into the facility on April 1, 2026 and passed away on April 12, 2026. R1 signed the admission agreement on March 31, 2026. The admission agreement signed and agreed upon between the resident and Licensee/facility indicated that the amount of $7,000.00 would be paid each month for rent and care level. R1 was receiving hospice care upon move in which the admission agreement indicated “hospice services: $1,000.00 per day, not to exceed monthly charge”. R1 resided at the facility from April 1, 2026 to April 12, 2026, which exceeded the hospice charge per day, not exceeding the monthly charge rate of $7,000.00. Documents reviewed indicated that the admission agreement was signed by both R1 and a co-signer (R1’s family member). R1 was their own responsible party but R1’s co-signer assisted with decision making. Both signed all sections of the admission agreement in understanding the facility policies. Therefore, the allegation refund not provided after resident passing is unfounded. Based on records reviewed and interviews, LPA finds the above allegations to be UNFOUNDED- meaning that the allegations were false, could not have happened and/or is without reasonable basis. Type of visit: Required - 1 Year

Licensing Program Analyst (LPA) Cassandra Mikkelson arrived unannounced and met with Licensee Maria Veres and Administrator Vasile Veres to conduct an annual inspection utilizing the inspection tool. LPA conducted an inspection of the care home to ensure compliance with Title 22 regulations. LPA observed resident room, common area bathrooms, kitchen, common areas and perimeter of care home. LPA observed rooms to be properly furnished, with appropriate bedding and lighting. The bathrooms were in sanitary condition, properly maintained, and the hot water temperature was observed to be 115.1 degrees F. LPA checked the kitchen area for the ability to prepare and store food. Care home has required (2) two day perishable and (7) seven day non-perishable food supply on hand. Smoke detectors and carbon monoxide detectors are operational in the care home. Fire extinguishers and first aid kit are maintained and ready for emergency use. LPA checked medication storage and found medications to be locked away and inaccessible to the residents. LPA reviewed six (6) resident files, two (2) staff files and resident medications. Facility has a current copy of certificate of liability insurance and LPA obtained a copy. As a result of this visit, no deficiencies were cited pursuant to California Code of Regulations, Title 22, Division 6, Chapter 8. Type of visit: Required - 1 Year

On July 15, 2025, Licensing Program Analyst (LPA) Cassie Yang arrived unannounced at the facility to conduct a required annual inspection utilizing the inspection tool. LPA met with Licensee and explained the purpose of the visit. LPA observed two residents in the common areas and two residents in their private rooms, Facility has four residents in care with no residents on hospice services. Facility is compliance to licensure. Today's inspection, LPA and Licensee conducted a tour of the interior and exterior of the facility to ensure the health and safety of residents in care. Areas toured included but not limited to residents bedrooms, bathroom, laundry room, staff room, kitchen, backyard and the common areas. LPA observed facility to have amply supply of linens. LPA observed common areas to have the required posters. LPA observed facility to be at a comfortable temperature of 76*. LPA observed toxins to be locked and secured. LPA observed facility to have at least two days of perishable and seven days of nonperishable foods. LPA observed facility to be clean and in good repair. File review of emergency drill revealed it was conducted on January 1, 2025, March 10, 2025 and June 20, 2025. LPA observed posted Administrator Certificate #7005717740 to be active with expiration date of 11/17/2024 to 11/16/2026. Emergency Disaster Plan was recently signed on April 14, 2025. LPA reminded Licensee to mark if plan was "reviewed" and/or "updated".LPA reviewed three resident files and three personnel files. LPA observed all required documents complete and present. LPA advised Licensee of Licensing's updated LIC602A, LPA informed Licensee the new LIC602A has additional information that satisfied the new neurocognitive disorder regulations. Inspection tool completed and no deficiencies cited. At this time, LPA is requesting the following documents to be emailed to LPA: LIC 500, LIC 308 and liability insurance by Friday July 18, 2025. Type of visit: Required - 1 Year

Licensing Program Analyst (LPA) Sabrina Calzada arrived unannounced to conduct a required annual. LPA met with Maria Veres, Licensee, and Vasile Veres, Administrator, and explained purpose of inspection. LPA observed (3) residents to be later eating lunch in the common area and (3) residents in their rooms. The facility is licensed for (6) non-ambulatory residents and has a hospice waiver for (4) residents. Currently, there are (3) residents on hospice. LPA observed a hospice nurse present at the start of the inspection. LPA and Administrator toured the interior and exterior of the facility including the common areas, resident bedrooms, resident bathrooms (2), kitchen, and laundry. LPA observed the facility to be clean, in good repair and to have sufficient furniture and lighting throughout. Both bathrooms have the necessary grab bars, non-skid flooring, paper towels, and hand-washing poster. There is sufficient 2+day perishable and 7+day non-perishable supply of food and locked sharps and medications are stored in the kitchen. There are locked toxins in the laundry room. The inside temperature measured at 76*F, and the hot water measured 106*F in the kitchen. The water heater shows the hot water set at 120*F for the resident side of the facility.. Fire extinguisher was last serviced 6/17/24, and the smoke/monoxide alarms are in working order. Outside, there is a covered patio area with seating, storage sheds with paper and other supplies and (2) unlocked exit gates. All required postings up. Infection Control Plan and Emergency Disaster Plan reviewed. There was a new A/C unit installed last week. LPA reviewed (3) resident files and found them to be organized, complete and contain current documentation. Medications were reviewed for (2) residents- orders matched medications being given. LPA reviewed (2) of (2) staff files and found them to be organized, complete and contain current First Aid/CPR certifications. All staff are cleared/associated. Administrator certificate #6019191740- exp 11/16/24. LPA obtained an updated copy of insurance and requested updated copy of LIC500- due by 7/22/24. There are no deficiencies issued; however, a Technical Advisory Note is issued since staff training was last completed July 2023 (due by July 2024). Exit interview.
