On 08/05/2026, Licensing Program Analyst (LPA) Yi Sam Jian conducted an unannounced annual inspection. LPA met with Leonora Aureus, Administrator. LPA explained the purpose of the visit. The ground floor had staff room, and garage. The second floor had 3 shared bed rooms, dinning room, kitchen, bathroom. Backyard was fenced, secured, and in good condition. All outdoor and indoor passageway were free and clear of obstruction. No accessible bodies of water or fire safety hazards observed. Kitchen was inspected, sufficient supply of food observed. Infection control practices reviewed. Medications, toxins and sharps stored appropriately and inaccessible to clients, a comfortable temperature was maintained, hot water temperature inspected to be compliant, furnishing and lighting was sufficient for comfort and safety. Carbon monoxide detector and smoke detector system inspected and met the requirements. fire extinguisher checked and fully charged. Facility has a written emergency disaster plan. Licensee has at least one completed first aid kit located in the office. Criminal record clearances or exemptions for facility staff or other individuals who have client contact have been reviewed. No deficiencies were cited. Technical violations were noted for no detail documentation for the quarterly emergency drill and documentation for reappraisal date. On 7/7/2025, Licensing Program Analyst (LPA) Tobola conducted an unannounced Annual Required – 1 yr. inspection for this facility and was greeted by Administrator, Nora Aureus. The facility currently provides care for 5 residents, none of which are receiving hospice services or with a diagnosis of dementia. LPA continued with a tour of the facility with staff, facility found to be clean and at a comfortable temperature with all exits free from obstruction. Resident’s bedrooms, common areas, kitchen & food storage areas were inspected. Fire Extinguishers located in kitchen was found to be charged. Smoke and carbon monoxide detectors were present and in working order. There was a sufficient supply of both perishable and nonperishable foods, with food stored in the kitchen refrigerator found to have appropriate coverings, enough for residents in care. There was a supply of hygiene products and paper products available for residents. Resident's bedroom have lighting & appropriate furnishings and bedding items. Restrooms for resident use were equipped with non-slip mats, grab bars and kept in good condition. There is a large outdoor backyard patio for resident use. Medications are stored in kitchen and found to be secured. Upon spot review of medications, LPA found that the facility has centrally store medication records updated and in order. LPA conducted a sample file review for residents and found that all residents in care are in need of updated care appraisals. Technical Violation. Upon a sample review of staff files LPA found that all staff are in need of updated 1st Aid & CPR certification. In addition, caregiver staff do not have current annual training on file. Technical Violation. Administrator stated that they are the primary staff present and providing care for residents. Additional staff are on call when needed. Licensee agrees to submit documentation of all items listed by POC date 7/21/2025: - Updated Needs & Service Appraisals for all residents - 1st Aid & CPR Certification for all staff providing care - Proof of staff annual training started Licensee understands that failure to provide documentation of corrections may lead to deficiencies. On 8/7/2024, Licensing Program Analyst (LPA) Tobola conducted an unannounced Annual Required – 1 yr. inspection for this facility and was greeted by Administrator, Leonara Aureus. The facility currently provides care for 6 residents, none of which are receiving hospice services or with a diagnosis of dementia. LPA continued with a tour of the facility with Administrator, facility found to be clean and at a comfortable temperature with all exits free from obstruction. Resident’s bedrooms, common areas, kitchen & food storage areas were inspected. Fire Extinguisher was found to be last charged on 8/26/2022, however the extinguisher gauge is still within charged meter. LPA spoke with fire inspector and requested for Administrator to immediately contact Fire Department and ensure extinguisher is charged and updated today. Citation issued. Smoke and carbon monoxide detectors found in the primary hallway, were tested and found to be functioning. LPA found that two (2) resident bedrooms did not have smoke alarms installed. However, Administrator provided proof of newly purchased alarms and will have them installed. Administrator to submit photo proof of corrections. There was a sufficient supply of both perishable and nonperishable foods as required by Title 22 Regulations, with food stored in the kitchen refrigerator found to have appropriate coverings, enough for residents in care. Cleaning supplies and other toxins are safely stored in locked cabinets in the, bathroom, garage and under kitchen sinks, all of which were secured upon inspection. There was a supply of hygiene products and paper products available for residents. All resident’s bedrooms have lighting & appropriate furnishings and bedding items. Water measured at faucets accessible to residents measured at 116.4 degrees F and within regulation. Residents that were awake during the inspection were observed interacting with staff in the common area, or in their bedroom resting. The facility encourages regular family visits and utilizes outings for resident exercise and mobility. In addition, there is an outdoor patio with shade and large outdoor space for residents. Continued onto LIC809-C LPA conducted a sample file review for 3 residents and found all items to be in order. Upon a check of staff files, LPA found that caregiving staff have 1st aid and CPR and annual training up to date. In addition, upon a spot check of medications all medication counts and records are in order. Leonora Aureus's Administrator Certificate 7005729740 is currently active through 8/14/2024. Administrator has submitted their training requirements for re-certification. LPA requested the following documents be sent to CCL by COB 8/21/2024: LIC 308 Designated Facility Responsibility LIC 500 Personnel Summary LIC 610 Emergency Disaster Plan LIC 9020 Register of Facility Client’s/Resident’s Liability Insurance Deficiency cited from the California Code of Regulations, Title 22, Division 6 of California Regulation.
