On 11/4/2025, Licensing Program Analyst (LPA) Murial Han conducted an unannounced visit to follow-up on a plan of correction, LPA met with caregiver, Rosema Caspillan and explained the purpose of today's visit. During the annual inspection on 10/7/2025, the facility received 2 B citations: - Other Provision 1569.625(b)(2) -Reappraisals 87463(a) The plan of correction for the above citations were due on 10/21/2025. However, the administrator did not submit the plan correction until 11/3/2025. The administrator stated that the delay of submission was due to computer issues but the administrator did not take the initiative to contact CCL rather LPA called to administrator on 10/22/2025 and 10/30/2025 to follow-up. During today's visit, LPA spoke with the administrator over the phone and reminded the administrator of the importance of being compliance with POC due date and if failure to submit plan of correction in the future may result in civil penalty. On 10/7/2025, Licensing Program Analyst (LPA) Murial Han conducted an unannounced an Annual Required – 1 yr. inspection. LPA met with staff, Rosema Caspillan. and LPA explained the purpose of today's visit. Licensee/administrator, Fernand Farol was contacted via phone of LPA's visit. LPA toured the facility inside out and inspected the living room, dining area, kitchen, bedrooms, bathrooms, and backyard. This is a 1-story building that consists of 5 rooms and 2 bathrooms. Water in kitchen and bathrooms was measured between the required 108-118 degrees Fahrenheit. Bathrooms were observed to have the required grab bars, non-skid mats and in good operating condition. The indoor and outdoor passageways were free of obstruction. Furniture and furnishings were observed to be sufficient. Food supplies were sufficient of 2- days perishables and 7- days of non-perishables. Comfortable temperature is maintained and lighting is sufficient for comfort Bed sheets, linens, and towels were observed to be sufficient and able to meet the needs of the residents at this time. Central storage for medications, sharps and chemicals were observed to be locked and inaccessible to residents in care. A review of (5) resident files was conducted and noted on the LIC 858. A review of (1) staff files was conducted and noted on the LIC 859. LPA requested a copy of the administrator certification by 10/21/2025. Based on observation, deficiency is cited under California Code of Regulations, Title, 22 cited on the LIC 809D. Failure to correct the deficiencies may result in civil penalties. . On 10/3/2024, Licensing Program Analyst (LPA) Tobola conducted an unannounced Annual Required – 1 yr. inspection for this facility and was greeted by staff, Rose Caspillion. Licensee, Fernand Farol was contacted and arrived later in the visit. The facility is a two story home that currently provides care for 4 residents, one of which is on hospice and none of which with dementia. LPA continued with a tour of the facility with staff, facility found to be clean and at a comfortable temperature with all exits free from obstruction. Resident’s bedrooms, common areas, kitchen & food storage areas were inspected. Fire Extinguishers located in thehallway and kitchen were observed and found to be charged. Smoke and carbon monoxide detectors were present and functioning. LPA requested for Licensee to change batteries in carbon monoxide detector. Technical Assistance. There was a sufficient supply of both perishable and nonperishable foods as required by Title 22 Regulations, with food stored in the kitchen, sufficient for residents in care. Food supply is replenished weekly and stored properly. Cleaning supplies and other toxins stored in the kitchen were fond to be secured. There was a supply of hygiene products and paper products available for residents. All resident’s bedrooms have lighting & appropriate furnishings and bedding items, with hospital bed orders on file. There is an backyard patio space for resident use, that is equipped with appropriate shading. During the inspection, residents were observed interacting with staff and one another in the common areas, or in their bedrooms resting. Residents appeared to have a positive relationship with the staff. Upon a spot check of medications, records and count were found to be in order. LPA reviewed resident records and found that residents (R1, R2) are in need of updating appraisals which are less than a month past annual updating. Technical Violation issued. In addition, upon review of staff records, Licensee is to update staff training and 1st aid & CPR certification that is expired less than a month prior to visit. Technical Violation issued. On February 4, 2024, Licensing Program Analysts(LPAs) Grace Donato and John Calandra arrived at the facility at 10:00 AM to conduct a Case Management follow up visit to the visit on December 31, 2023. LPA Calandra and LPA Donato met with Administrator, Ferdinand Farol and explained the purpose of the visit. The LPAs delivered an Amended report from 12/31/2023 and followed up on a check for the Annual fee in 2023 that bounced. The LPAs interviewed the Administrator and learned that the check had bounced likely due to insufficient funds in the account at the time the check was written. No deficiencies were cited during today's visit. On February 2, 2024, Licensing Program Analysts(LPAs) John Calandra and Grace Donato, arrived at the facility at 10:00 AM to conduct an unannounced Annual 1-year required inspection. LPAs Calandra and Donato met with Jerry Caspillan, Caretaker. Administrator/Licensee, Fernand Farol joined the visit later. LPA Calandra toured the physical plant. This is a 1-story building that consists of 5 rooms and 2 bathrooms. Water in all bathrooms was measured between the required 105-120 degrees Fahrenheit. Bathrooms were observed to have the required grab bars and anti-skid mats. Fire extinguishers in the facility were observed to be fully charged and were checked while the LPAs were at the facility. The facility had the required 7 days of non-perishables and 2 days of perishables on site. No food was expired. The kitchen refrigerators and freezers temperature were within the required range. All bedrooms were sufficiently lit and had the required furniture. The backyard was clear from obstructions. No accessible bodies of water or hazards were observed. The facility's first aid kit was observed to be complete. All knives and sharp objects were observed to be locked and in-accessible to persons in care. All medications, soaps, and detergents were observed to be locked and in-accessible to persons in care. A review of Centrally stored medications indicated that medications for residents were properly labeled with instructions on dosage and times of day and matched the Centrally Stored Medication records kept at the facility. Administrator, Fernand Farol does not currently have control of property. Administrator currently leases property from his parents but did not have a sublease agreement or the current lease agreement available for LPAs to review. 5 Resident files and 4 staff files were reviewed. All were observed to be complete. Deficiencies are cited under Health and Safety Code,cited on the LIC 809D. Failure to correct the deficiencies may result in civil penalties. On 02/02/2024, Licensing Program Analysts (LPAs) Grace Donato & John Calandra conducted a case management visit regarding and audit report that was requested last April 2022. LPA met with Administrator Fernand Farol. LPA explained the purpose of today's visit. LPA discussed with the administrator the audit request that was done last April 2022. Administrator mentioned that he wasn't able to send the requested documents for this facility to the Audits Section. Administrator mentioned that, currently, there is no issue finances. Deficiencies are being cited today as the facility did not ensure that documents requested are submitted to the requesting auditor. Deficiencies are cited under California Code of Regulations, Title, 22 cited on the LIC 809D. Failure to correct the deficiencies may result in civil penalties. Type of visit: Case Management - Health Checks

**THIS IS AN AMENDED REPORT** On December 21, 2023, Licensing Program Analyst(LPA) John Calandra and Licensing Program Manager, Cara Smith conducted an attempted unannounced case management health check visit in relation to the facility sending a check to Sacramento that bounced for the Annual fee, the facility phone number being no longer in service, and being unable to contact the property manager and Administrator. LPA and LPM knocked on the door once, and a woman answered the door. LPA Calandra and LPM Smith introduced themselves and explained the purpose of their visit. The woman, Rose Caspillian is a caretaker at the facility. Rose informed LPA Calandra and LPM Smith that she and her husband, Jerry Caspillian are the only staff working today. LPA and LPM asked if the Administrator was available and were told that Rose could call him. The LPA and LPM talked to the administrator and explained the purpose of their visit over the phone. Based on an interview with the Administrator, they stated that the bank account did not have funds to cover the Annual fee. Thus, when they were notified by mail that the check had bounced, the Administrator wrote another check. LPA obtained an updated phone number and email address for the facility and was told by the Administrator that they have sent a second check to Sacramento for the annual fee. Review of the department system shows that the check was paid. LPA and LPM were given a tour of the facility by Rose. The facility consists of 5 bedrooms and 2 bathrooms, a kitchen, living room, and dining room. No deficiencies cited during today's visit.
