Allegation investigated: Staff does not ensure resident does not have access to drugs. Staff does not provide a safe environment for resident in care.

Licensing Program Analyst (LPA) Jose Tan an unannounced subsequent complaint visit to this facility to further investigate the above allegations. LPA met with Andy Terner and explained the reason for the visit. LPA conducted a physical plant tour at 9:18 AM, requested copies of facility documents at 9:44 AM, reviewed records between 9:45 AM to 10:45 AM and interviewed staff and residents between 12:00 PM to 1:00 PM. Regarding the allegation that Staff does not ensure resident does not have access to drugs, it was alleged that Resident #1 (R1)’s roommate or Resident #2 (R2) is using drugs and keeps R1 up all hours of the night. LPA’s record review today at 9:45 AM revealed that R2 had a medical condition wherein R2 has a limited mobility and is unable to physically move at a normal pace. LPA’s interview with R2 on prior visit on 02/13/26 at around 11:00 AM, revealed that R2 denied using any illegal drugs and all R2’s medication are kept by the staff and given to R2 as prescribed. LPA’s interview with five (5) other residents on prior visit between 10:30 AM to 1:00 PM also revealed that five (5) out of five (5) residents stated that they did not witness any other resident at the facility using illegal drugs. (continued on LIC 9099-C) Unsubstantiated (continued from LIC 9099) Regarding the allegation that Staff do not provide a safe environment for residents in care, it was alleged that R2 hit R1 in the stomach while sleeping resulting in a bruise in R1’s arm. LPA’s record review today at 9:45 AM revealed that R2 had a medical condition wherein R2 has a limited mobility and is unable to physically move at a normal pace. LPA’s observation during this and prior visit revealed that R2 was unable to move and drag half of R2’s body and seems improbable to be physically aggressive with anyone. LPA’s interview with R2 revealed that R2 did not have any altercation, physical or verbal with R1 and denied even touching R1. Further, R2 was told by R1 that R1 wanted to be alone in their room many times. LPA attempted to interview R1 but per the Administrator, R1 moved out of the facility on_____ Based on the information gathered during this and prior visit, these allegations are deemed unsubstantiated at this time. Licensing Program Analyst (LPA) Gary Tan initially met Co Administrator Andranik Ter-nersesian for a One (1) Year Required visit for this facility. LPA explained the reason for the visit. A tour of the physical plant was conducted at 12:116 PM and the following was noted: There is only one entrance being utilized at the facility. Screening area is located immediately upon entrance. Sign in sheet, hand sanitizer, gloves and masks are available. The facility had submitted and approved Infection Control and Mitigation plan. The facility has a designated visitors' area at the backyard. The facility has sufficient stock of PPE in the storage room. The facility has four (4) bedrooms and two (2) bathrooms currently occupying six (6) residents. One (1) bathroom is designated for staff use. The facility is fire cleared for six (6) non-ambulatory residents. Hospice waiver for four (4) residents. Living and dining room furniture were also checked. The living room is neat and clean. The facility maintains a comfortable temperature at 75°F. The smoke detectors are hardwired and interconnected and observed to be operational. There is a carbon monoxide installed at the facility. Fire extinguisher is located in the dining area and observed to be full and last bought on 10/30/25. The backyard of the facility has outdoor furniture, with a covered shaded area for clients. There is a sitting area in the front yard for residents to conduct outdoor activities. There is a shed at the backyard being used as a storage for frozen food, PPE and other supplies. Laundry area is located at the back. Laundry detergents, cleaning agents and other toxins are stored in a locked cabinet in the laundry area. Food Service/Kitchen area was sufficiently stocked with two (2) days perishable and seven (7) days non-perishable food. Frozen foods are properly wrapped and stored appropriately. Food storage and preparation areas are clean and inaccessible to pests. Knives and sharps are observed to be locked in a kitchen drawer and inaccessible to residents. The Clients' rooms are adequately furnished with appropriate furniture and lighting system. Hall ways/passage ways are lit. Clients have sufficient amounts of personal hygiene product which is provided by the licensee. The bathroom was checked for cleanliness and proper operation. LPA observed the appropriate grab bars in the toilet and shower. The hot water temperature was measured at 117.3°F. Towels and washcloths are not shared. There was enough clean linen available in stock at the cabinet. Medications: LPA observed medication in the hallway cabinet to be locked and inaccessible to residents. There is a complete first aid kits located in the kitchen. Client records: Client records are reviewed. Client records appear to be complete and updated. Staff records: LPA conducted a complete file review of staff records. Staff records appeared to be complete and updated. Disaster drill was last conducted on 09/20/25. Allegation investigated: Facility staff spoke inappropriately to resident. Facility staff did not safeguard resident's confidential information.

Licensing Program Analyst (LPA) Tan conducted an unannounced complaint visit at this facility to investigate the above allegations. LPA met with co Administrator Andy Terner and explained the reason for the visit. LPA conducted physical plant tour at 9:13 AM, requested copy of facility documents relevant to the investigation at 9:42 AM and interviewed residents and staff between 10:00 AM to 12:30 PM. Regarding the allegation that a Facility staff spoke inappropriately to resident, it was alleged that a staff mocked and made fun of Resident #1 (R1) in front of other residents. LPA’s interview with four (4) residents today between 10:00 AM to 12:30 PM revealed that four (4) out of four (4) residents interviewed did not experience being mocked or made fun of nor witness any staff mocked or made fun of R1. LPA attempted to interview R1 but per interview with the Administrator, R1 moved out of the facility on R1’s own volition on 02/25/25 and did not respond to LPA’s phone call. LPA’s interview with the staff revealed that staff did not make fun or mock R1 at any time nor witness any staff made fun of or mocked R1. (continued on LIC 9099-C) Unsubstantiated (continued from LIC 9099) Regarding the allegation that Facility staff did not safeguard resident's confidential information, it was alleged that Staff #1 (S1) discussed R1’s medical conditions with other residents. LPA’s interview with four (4) residents today between 10:00 AM to 12:30 PM revealed that four (4) out of four (4) residents interviewed did not hear S1 discussing R1’s medical condition to any of them at any time. LPA attempted to interview R1 but per interview with the Administrator, R1 moved out of the facility on R1’s own volition on 02/25/25 and did not respond to LPA’s phone call. LPA also interview with hospice nurse today at 10:20 AM revealed R1 had a psychiatric condition that made R1 hearing and seeing things that are non-existent, further, the nurse denied hearing S1 discussing anything about R1 at any time. LPA’s interview with S1 revealed that S1 did not discuss R1’s or any resident’s medical condition to anyone. Based on the information gathered during this visit, these allegations are deemed unsubstantiated at this time. Exit interview conducted. LPA explained the reason for the visit. The co Administrator designated Ms. Gasparyan to sign the report. A tour of the physical plant was conducted at 9:19 AM and the following was noted: There is only one entrance being utilized at the facility. Screening area is located immediately upon entrance. Sign in sheet, hand sanitizer, gloves and masks are available. The facility had submitted and approved Infection Control and Mitigation plan. Signs to wear a mask and other Covid 19 prevention protocol signs were posted inside. Hand washing, coughing etiquette, physical distancing and other necessary signs were posted in the bathroom and all over the facility. The facility has a designated visitors' area at the backyard. The facility has sufficient stock of PPE in the storage room. The facility has four (4) bedrooms and two (2) bathrooms currently occupying five (5) residents. Allegation investigated: Staff are not following resident's special diet order Staff speak inappropriately to resident

Licensing Program Analyst (LPA) Gary Tan conducted an unannounced initial complaint visit at this facility to investigate the above allegations. LPA met with co- Administrator Andy Terner and explained the reason for the visit. LPA conducted physical plan tour at 9:15 AM, requested copy of facility documents relevant to the investigation at 9:55 AM, reviewed facility documents between 9:55 AM to 10:45 AM and interviewed residents between 11:00 AM to 12:45 PM. Regarding the allegation that Staff speak inappropriately to resident, it was alleged that staff told Resident #1 (R1) will make R1 pay for toilet paper. LPA's interview with R1 today at 11:00 AM revealed that the staff and facility owner provided everything R1 needed and did not have to pay for it. LPA's interview with three (3) residents between 11:00 AM to 12:45 PM revealed that all three (3) of them did not experience nor heard or witnessed any staff speaking inappropriately or asked to pay for anything in the facility. (continued on LIC 9099-C) Unsubstantiated (continued from LIC 9099) Regarding the allegation that Staff are not following resident's special diet order, LPA's interview with R1 today revealed that the staff prepares food in accordance with R1's diet. LPA's interview with the staff today at 12:02 PM confirmed that the staff had a renal diet guide in the kitchen for preparing R1's food. LPA's record review today between 9:45 AM to 10:45 AM revealed that there are two (2) residents at this facility on renal diet. LPA's interview with Resident #2 (R2) confirmed that the staff prepared their food based on the guidelines provided to the staff, usually no or very little salt with a lot of fruits and vegetables. LPA observed that the staff prepared cauliflower, spinach and couscous soup with watermelon and cantaloupe for lunch for their renal diet residents. Based on the information gathered during this visit, these allegations are deemed unsubstantiated at this time. Exit interview conducted.
