Licensing Program Analyst (LPA) Sarah Hurt conducted an unannounced visit today for the facility’s annual inspection. LPA met with Administrator, Matthew Magsambol Continual Administrator's Certification expires 04/08/2026. There are currently 6 residents who reside at this home and there is 0 residents on hospice at this time. LPA inspected the interior and the exterior of the facility including the common living spaces, resident bedrooms and bathrooms, activity rooms, medication storage, kitchen, garage and outdoor areas. There is a locked storage for medications. Food supply is adequate for 2-day perishable and 7-day nonperishable. Fire extinguisher is within the safety regulation period. Smoke alarms were tested and are operational. The home has a carbon monoxide detector and performs disaster drills as required. First Aid kit is on site and complete. Toxins and cleaning supplies are locked and inaccessible. LPA reviewed 3 staff files, 4 resident files, Plan of Operation, Infection Control Plan, and Emergency Disaster Plan. LPA observed resident 1's bedroom door is broken. LPA observed Resident 2 does not have required annual updated assessment. Resident 2's as needed PRN medications are not logged correctly. Resident 2's admission agreement does not include basic rate charged for services. LPA observed the facilities yard does not have patio furniture or covered patio space. LPA observed the facilities hallways bathroom sink water temperature measures at 124 degrees. LPA observed cobwebs throughout the facility with spiders, and multiple smoke alarms chirping. The facility does not have an accurate record tracking resident monies. The following deficiencies observed or cited during today's inspection per California Code of Regulations, Title 22. Licensing Program Analysts (LPA) Sarah Hurt conducted an unannounced visit today for the facility’s annual inspection. LPA met with Mathew Magsambol, Continual Administrator's Certification for Rosario Magsambol expires for 06/20/2025. There are currently 6 residents who reside at this home and there is 0 residents on hospice at this time. LPA reviewed 4 facility resident files, and 3 facility staff files. LPA reviewed residents funds, and resident medications. LPA reviewed Centrally Stored Medication Log for 3 facility residents. Residents cash was not correctly logged (only February 2025 was accounted). The facility temperature is 62 degrees. Staff 1 does not have a Health Screening Report (LIC503) or required first aid training. Staff 2 does not have first aid training. The following deficiencies observed or cited during today's inspection per California Code of Regulations, Title 22. LPA met with Administrator Mathew Magsambol, Continual Administrator's Certification for Licensee Rosario Magsambol expires 06/20/2025. There are currently 6 residents who reside at this home and there is 0 residents on hospice at this time. LPA inspected the interior and the exterior of the facility including the common living spaces, resident bedrooms and bathrooms, activity rooms, medication storage, kitchen, garage and outdoor areas. Bedrooms were clean and in good repair. There is a locked storage for medications. Food supply is adequate for 2-day perishable and 7-day nonperishable. Fire extinguisher is within the safety regulation period. Smoke alarms were tested and are operational. The home has a carbon monoxide detector and performs disaster drills as required. Water temperature was tested at 105 degrees. Toxins and cleaning supplies are locked and inaccessible. First aid kit is not complete. Facility room temperature measured at 66 which is below required minimum of 68 degrees. LPA observed a screen door leading to outside with small tear and installed backwards. Facilities 610 Emergency Disaster plan is not updated or accurate. Infection Control Plan has not been updated annually as required. LPA Hurt will return at a later date to review client and staff records. The following deficiencies observed or cited during today's inspection per California Code of Regulations, Title 22. Allegation investigated: Staff are not providing adequate food service Staff are mismanaging resident's medication
Licensing Program Analyst (LPA) Sarah Hurt arrived at the facility unannounced on October 3,2024 at 11:00 a.m. to investigate the above allegations. LPA met with facility Administrator Mathew Magsambol, and explained the purpose for today’s visit. Regarding the allegation Staff are not providing adequate food service. LPA Hurt observed vegetables in the refrigerator molding and expired. LPA Hurt observed soy sauce, and juice opened and not refrigerated. LPA Hurt observed the facility refrigerator does not have adequate fresh food supply. Based on observation the preponderance of evidence standard has been met, therefore the above allegation is found to be SUBSTANTIATED. Substantiated Continued.. Regarding the allegation Staff are mismanaging resident's medication. Resident 1 has not been given a prescribed medication since 10/01/2024. Resident missed the 10/01/2024 dose, 10/02/2024 dose, and the 10/03/2024 dose. Based on records reviewed, and observation the preponderance of evidence standard has been met, therefore the above allegation is found to be SUBSTANTIATED. This is an amended report. LPA met with Administrator Mathew Magsambol Continual Administrator's Certification expires April 08, 2024. There are currently 5 residents who reside at this home and there is 0 residents on hospice at this time. LPA inspected the interior and the exterior of the facility including the common living spaces, resident bedrooms and bathrooms, activity rooms, medication storage, kitchen, garage and outdoor areas. Bedrooms were clean and in good repair. There is a locked storage for medications. Food supply is adequate for 2-day perishable and 7-day nonperishable. Fire extinguisher is within the safety regulation period. Smoke alarms were tested and are operational. The home has a carbon monoxide detector and performs disaster drills as required. Water temperature was tested at 119 degrees. First Aid kit is on site and complete. Toxins and cleaning supplies are locked and inaccessible. Licensee does not have Licensing Complaint poster (PUB 475) visible inside facility. Licensee does not have device solely for purpose of residents accessing internet. The facilities Emergency Disaster plan does not have marked outline on sketch for meeting place during emergencies or disasters. Licensee did not have current Admission Agreements in resident files. LPA Hurt observed food items in the refrigerator not in Tupperware with no labels or dates. There were no deficiencies observed or cited during today's inspection per California Code of Regulations, Title 22.
