On this day, Licensing Program Analyst (LPA) Andrea Mendivil made an unannounced visit to conduct a required annual visit. LPA was greeted and granted entry into the facility and explained the reason for the visit. The facility is a single story home with four private resident bedrooms, one staff bedroom, four resident bathrooms, one staff bathroom, a living room, a dining room, a TV room, a kitchen, a laundry room, and an attached two car garage. The facility is licensed for 6 residents of which 5 may be non-ambulatory and 1 may be bedridden. The facility has a current census of 4 residents. During today’s visit, LPA toured the facility and inspected the physical plant, including but not limited to testing all smoke detectors, testing hot water temperature in resident bathrooms. The hot water temperature measured at 105.4 to 106.8 Fahrenheit and all smoke detectors were operational. LPA inspected the facility food supply and observed the facility retained a minimum of two days perishable and seven days non-perishable food on hand. Facility conducted emergency earthquake drill on April 15, 2026. LPA Mendivil observed a secured medication cart stored in facility living room. LPA reviewed medication and medications appear to be given as prescribed. LPA reviewed three out of three staff fingerprint records and records contained all required documents and training. LPA conducted a complete review of all resident records. LPA confirmed that administrator has a current administrator certificate which expires October 16, 2027. Based on observations made, no deficiencies are being cited during today's visit. On May 19, 2025, Licensing Program Analyst (LPA) Brandon Lopez made an unannounced visit to the facility to conduct the required annual inspection. LPA was greeted and granted entry into the facility by staff after explaining the purpose for the visit. Administrator (AD) Magaly Quinteros was notified via telephone and later arrived to assist with the inspection. LPA observed that Magaly Quinteros submitted a Administrator renewal application to the Administrator Certification Bureau (ACB) on May 2, 2025. The facility is a Residential Care Facility for the Elderly (RCFE) licensed for six non-ambulatory residents, of which one can be bedridden, and has a hospice waiver for six. The facility is a single story home with four private resident bedrooms, one staff bedroom, four resident bathrooms, one staff bathroom, a living room, a dining room, a TV room, a kitchen, a laundry room, and an attached two car garage. LPA, accompanied by the AD conducted a tour of the interior portion of the facility. On today's visit, LPA observed four residents in care and two staff present. LPA observed residents relaxing in their respective bedrooms and in common areas. LPA observed the See Something, Say Something poster (PUB 475) mounted on the wall in the TV room. LPA inspected the four private resident bedrooms and they were observed to be free of hazards. LPA observed residents bedrooms to have the required furnishings of a bed, a chair, a chest of drawers, and a lamp. LPA observed resident beds had clean linens and blankets. LPA inspected the four resident bathrooms. Resident bathrooms were clean. Resident bathrooms were equipped with grab bars and non-skid floors. Faucets and toilets were operational. Hot water temperature measured between 106.2 and 109 degrees Fahrenheit. LPA observed the staff bedroom to be clear of hazards. LPA observed the kitchen has a two day perishable and a seven day nonperishable food supply on hand. LPA observed kitchen knives and sharps to be stored in a locked kitchen cabinet. CONTINUED ON 809-C LPA observed fire extinguishers to be mounted in the kitchen and in the resident hallway. Fire extinguishers were observed to be charged and serviced as of January 30, 2025. LPA tested the individual smoke detectors and carbon monoxide detectors which tested operational. LPA observed the facility conducted their last emergency disaster drill on May 16, 2025 LPA observed the centrally stored medication to be kept in a locked medicine cart. LPA observed the facility has a First Aid Kit stored in the medicine cart and it had all the required components. LPA observed the door leading to the laundry room to be kept locked and inaccessible to residents in care. LPA observed additional linens to be stored in the laundry room. LPA observed chemicals and toxins to be stored in the locked laundry room. LPA observed the door leading to the attached two car garage to be kept locked and inaccessible to residents in care. LPA observed the garage to be used for storage. LPA observed the facility has a three day emergency food and water supply. LPA, accompanied by the AD, conducted a tour of the exterior portion of the facility. The exterior portion was observed to be free of hazards and obstructions. LPA observed a shaded outdoor seating area with furniture for resident use. LPA observed the perimeter gates on the north side and southside of the facility to be self latching and can be opened in an evacuation. LPA observed a pool in the backyard which was observed to be adequately fenced and kept locked for resident safety. LPA reviewed all four resident files. All the required documentation were present and current in the resident files reviewed. LPA reviewed residents' medication and medication records. LPA reviewed six staff files. All staff are background cleared and associated to the facility. Based on the observations made during today's visit, no deficiencies are being cited per the Title 22 of the California Code of Regulations. Type of visit: Case Management - Incident

Licensing Program Analyst (LPA) Ruth Martinez made an unannounced visit to the facility for the purpose of a case management visit. LPA was greeted and granted entry into the facility by staff. LPA met with Magaly Quintero, Administrator and explained the nature of the visit. The purpose of the visit is to conduct a case management visit to follow up on a death of resident 1 (R1) who passed away on October 06, 2024. During today’s visit LPA interviewed staff, toured the physical plant of the facility, spoke to alert residents and reviewed pertinent documents pertaining to R1. LPA interviewed staff for further information regarding the death of R1 and the events that led up to R1’s death. Facility has not received a copy of OC Sheriff Corner’s office report or death certificate. LPA advised Administrator to forward any copies of reports or information regarding R1’s death to LPA as soon as it is available. Based on observations made during today’s inspection, no deficiencies are being cited per Title 22 Division 6 of the California Code of Regulations. Type of visit: Case Management - Health Checks

Licensing Program Analyst (LPA) Alvaro Ramirez, Jr made an unannounced case management visit for the purpose of conducting a health and safety inspection. LPA was greeted and granted entry by Administrator (AD) Magaly Quinteros. LPA explained the reason for the visit. LPA conducted a case management visit to follow up on a death report dated October 07, 2024 for Resident 1 (R1). LPA and AD conducted a toured of the facility and observed the facility has electricity, water, and gas. Water temperature tested at 107.1 degrees Fahrenheit. Resident bedrooms were observed to have the required furnishings. Certificate of liability insurance was observed to current. The kitchen was observed to be clean and organized and a 2-day supply of perishable and a 7-day supply of non-perishable food was observed. Medications are kept locked in a cabinet by the family room. Knives are kept locked in a kitchen drawer. All and any toxic chemicals, cleaning solutions, laundry toxins, and disinfects were observed to be inaccessible to clients. During today's visit LPA observed as the caregivers prepared red sauce spaghetti, chicken, potatoes and juice for lunch. Based on observations made during today’s inspection, no deficiencies are being cited per Title 22 Division 6 of the California Code of Regulations. Type of visit: Required - 1 Year

Licensing Program Analyst (LPA) Ruth Martinez is conducting this unannounced visit for the purpose of completing an annual required inspection. LPA arrived at the facility and was greeted and granted entry by Magaly Quintero, Administrator and LPA explained the nature of the visit. There are four residents at the facility and there are no residents receiving hospice services currently. LPA accompanied by the Administrator began the tour of the inside and outside of the facility. LPA observed required department postings throughout the facility. Facility stays within the capacity limitations. There is a minimum of one week of non-perishables foods and two days of perishables foods available. There is additional food storage in a freezer located in the garage. The facility is maintained at a comfortable temperature. LPA inspected that medication is centrally stored in a safe locked medication cart located in the tv room, LPA observed to be a small refrigerator with a lock for medications that require refrigeration in the dining room. LPA reviewed medication and observed medication was labeled and stored inaccessible to residents in care. LPA inspected the bathroom. LPA measured the hot water temperature which measured 105.9 Fahrenheit degrees. All bathrooms observed to have a supply of soap, toilet paper and towels. Bathrooms are equipped with required safety measures such as non-skid mats and grab bars. Lighting is sufficient to ensure safety and comfort. The facility is equipped with sufficient hand hygiene, cleaning, and disinfecting supplies. LPA observed that toxic chemicals, cleaning solutions and disinfectants are stored locked in a storage cabinet. The facility has an available clean supply of linens. LPA inspected residents’ bedrooms which has sufficient lighting to ensure the safety and comfort. All bedrooms observed to have all required components. Storage space is provided for residents in their bedroom. Smoke detectors were tested and found to be operational. LPA toured the outside of the facility and observed outdoor passageways are free of obstructions. LPA observed there are shaded seating areas for residents’ enjoyment. LPA Continued on LIC809-C observed a swimming pool adjacent to the house with a fence around it. LPA observed the pool gate has a self-latching entry door which opens away from the pool. The fence has a key lock at the gate door for inaccessibility. LPA measured the pool fence which measured 4ft 7.5in from base of the floor to the top of the fence and it was observed to enclose the entire pool area. LPA observed a fire extinguisher mounted on the wall in the kitchen. Fire drills are conducted every three months. LPA began review of records. LPA reviewed four resident records. All the required documentation was present and current in the residents’ files reviewed. LPA reviewed two employee records. All employees present have a criminal record clearance and are associated to the facility. LPA observed records reviewed have a current First Aid certificate. Based on the observations made during today’s visit, no deficiencies were noted today in the areas inspected per Title 22 Division 6 of the California Code of Regulations. Licensing Program Analyst (LPA) Claudia Gutierrez made an unannounced visit for the purpose of conducting a Required/Annual Inspection. LPA met with Administrator (AD) Magaly Quinteros and discussed the purpose of the inspection. During the inspection LPA and AD conducted a tour of the inside and outside of the facility, common areas, resident rooms, kitchen, garage and observed the following: This is a one-story home with five bedrooms, five bathrooms, living room, tv room, dining area, kitchen, laundry room, and attached two car garage. The pool in the backyard was observed to be fenced as required by regulations. All resident bedrooms had the required furnishings. LPA observed all resident beds had linens and blankets. The back yard, front yard, and side courtyard have a shaded sitting area. LPA observed two staff and three residents present. Residents were observed engaging in day program and listening to music in the tv room. Bathrooms were observed to be free of debris and mildew, faucets and toilets were operational. Water temperature tested at 106.8 F degrees. LPA observed emergency disaster plan with means of exiting and emergency phone numbers listed and posted. A 2-day supply of perishable and a 7-day supply of non-perishable foods was observed during today’s visit. Smoke detectors and carbon monoxide detectors tested operational. Fire extinguisher was observed to be fully charged. Appliances were all inspected. Sharps were observed locked in a kitchen drawer. All and any toxic chemicals, cleaning solutions, laundry toxins and disinfectants are inaccessible to residents. Medication cart was observed to be locked. LPA reviewed three resident files and three staff files. LPA interviewed staff and clients present. Based on the observations made during today’s inspection, no deficiencies are being cited per Title 22 Division 6 of the California Code of Regulations.
