Type of visit: Required - 1 Year
On February 11, 2026, Licensing Program Analyst (LPA) Brandon Lopez made an unannounced visit to the facility to conduct the required annual inspection. LPA was greeted and granted entry into the facility by staff after explaining the purpose for the visit. Licensee (LI) Renee Kelada was notified via telephone and later arrived to assist with the inspection. LPA observed that Renee Kelada has a valid Administrator certificate which expires on December 14, 2026. The facility is a Residential Care Facility for the Elderly (RCFE) licensed for six residents, of which five can be non-ambulatory and of which one may be bedridden in bedroom number three. The facility also has a hospice waiver for three residents. The facility is currently under construction at the time of visit. The facility is converting their sun room into two resident bedrooms. However, Community Care Licensing was notified prior to construction starting and verified the facility had all the necessary permits prior to construction beginning. The facility is currently a single story home with four private resident bedrooms, four bathrooms, three of which are dedicated for resident use, a living room, a kitchen, a laundry room, and an unattached two car garage. There is also an ADU and a pool located in the backyard. LPA accompanied by the LI conducted a tour of the interior portion of the facility. On today's visit, LPA observed three residents in care and two care giving staff present. LPA observed the See Something, Say Something poster (PUB 475) mounted on the wall by the entryway of the facility. LPA inspected the four private resident bedrooms. LPA observed residents bedrooms to be clean and free of hazards. LPA observed the resident bedrooms had the required furnishing of a bed, a chair, and chest of drawers, and a lamp. LPA observed resident blankets and linens to be clean. LPA observed additional linens to be stored in a closet in the resident hallway. LPA inspected the three resident bathrooms. Resident bathrooms were clean. Bathrooms are equipped with grab bars and non-skid floors. Faucets and toilets were operational. Hot water temperature measured between 113.3 and 116.7 degrees Fahrenheit. CONTINUED ON LIC809-C LPA observed the kitchen has a two day perishable and seven day nonperishable food supply on hand. LPA observed kitchen appliances to be clean and operational. LPA observed the five burner gas stove lights unassisted. LPA observed kitchen knives are stored in a locked kitchen cabinet. The centrally stored medication is kept in a locked cabinet in the kitchen. A First Aid kit is stored in a locked cabinet in the kitchen and it was observed to have all the required components. A fire extinguisher is located in the laundry room and it was observed the be charged and purchased on January 8, 2026. LPA observed chemicals and toxins to be stored in a locked cabinet in the laundry room. LPA tested the wired smoke detectors/carbon monoxide detectors which tested operational. LPA observed the facility conducted their most recent emergency disaster drill on December 1, 2025. LPA, accompanied by the LI, conducted a tour of the exterior portion of the facility. LPA observed the exterior portion to be clear of obstructions and hazards. LPA observed a shaded outdoor seating area with furniture for resident use. LPA observed the unattached two car garage is kept locked and inaccessible to resident in care. LPA observed the garage to be used for storage. LPA observed the facility has a three emergency food and water supply stored in the garage. The perimeter gate of the facility is self-latching and can be opened in an evacuation. LPA observed the ADU located in the backyard is for staff use only and is kept inaccessible to resident's in care. LPA observed the pool in the backyard has a net for resident safety. LPA reviewed all three residents' files. All the required documentation were present and current in the resident files reviewed. LPA reviewed the three residents’ medication and medication records. LPA reviewed six staff files. All staff are background cleared and associated to the facility. Based on today's observations, there are no deficiencies being cited per Title 22 of the California Code of Regulations. Type of visit: Required - 1 Year
Licensing Program Analyst (LPA) Brandon Lopez made an unannounced visit to the facility for the purpose of conducting the required annual inspection. LPA was greeted and granted entry by care giving staff after explaining the purpose for the visit. Administrator (AD) Renee Kelada was notified via telephone and later arrived to assist with the inspection. LPA observed that Renee Kelada's Administrator certificate expired on December 14, 2024. However, LPA verified and confirmed that Renee Kelada submitted a renewal application to the Administrator Certification Bureau (ACB) on November 18, 2024. The facility is a Residential Care Facility for the Elderly (RCFE) licensed for five non ambulatory and one bedridden resident, of which three may be on hospice. The facility is a single story home with four private resident bedrooms, three resident bathrooms, two of which are shared, one staff bathroom, a living room, a kitchen, a dining room, a sun room, a laundry room, and has an unattached two car garage. There is also a ADU and pool located in the backyard. LPA accompanied by the AD conducted a tour of the interior portion of the facility. On today's visit, LPA observed four residents in care, one of which is receiving hospice care, and two care giver staff present. LPA observed residents relaxing in the dining room and in their respective bedrooms. LPA observed the See Something, Say Something poster (PUB 475) mounted on the wall by the entryway of the facility. LPA inspected the four private resident bedrooms. LPA observed residents bedrooms to be clean and free of hazards. LPA observed the resident bedrooms had the required furnishing of a bed, a chair, and chest of drawers, and a lamp. LPA observed resident blankets and linens to be clean. LPA observed additional linens to be stored in a closet in the resident hallway. LPA inspected the three resident bathrooms. Resident bathrooms were clean. Bathrooms are equipped with grab bars and non-skid floors. Faucets and toilets were operational. Hot water temperature measured between 107.2 and 107.6 degrees Fahrenheit. LPA observed the kitchen has a two day perishable and seven day nonperishable food supply on hand. LPA observed kitchen appliances to be clean and operational. CONTINUED ON LIC809-C The five burner gas stove lights unassisted. LPA observed kitchen knives are stored in a locked kitchen cabinet. A fire extinguisher is located in the laundry room and it was observed the be charged and up to date on service. LPA observed chemicals and toxins to be stored in a locked cabinet in the laundry room. LPA tested the wired smoke detectors/carbon monoxide detectors which tested operational. The centrally stored medication is kept in a locked cabinet in the kitchen. A First Aid kit is stored in a locked cabinet in the kitchen and it was observed to have all the required components. The unattached two car garage is kept locked and inaccessible to resident in care. The garage is used for storage. LPA observed the facility has a three emergency food and water supply stored in the garage. LPA and AD conducted a tour of the exterior portion of the facility. LPA observed the exterior portion to be clear of obstructions and hazards. LPA observed a shaded outdoor seating area with furniture for resident use. LPA observed the ADU located in the backyard is for staff use only and is kept inaccessible to resident's in care. LPA observed the pool in the backyard has a net for resident safety. LPA reviewed all four resident files. All the required documentation were present and current in the resident files reviewed. LPA reviewed the four residents’ medication and medication records. LPA reviewed four staff files. All staff are background cleared and associated to the facility. Based on today's observations, there are no deficiencies being cited per Title 22 of the California Code of Regulations. Nov 23, 2024Facility evaluation reportReport on file
Type of visit: Required - 1 Year
On 11/23/2024 at 08:30 AM, Licensing Program Analyst LPA) Jose Calderon conducted an unannounced annual inspection visit at the Casa Palma Facility. LPA Calderon was allowed entry into the facility by Administrator Renee Kelada. Administrator Kelada asked infection control questions and took LPA Calderon temperature prior to entrance into the facility. The facility is licensed to operate 6 non-ambulatory and 1 bedridden resident 60 and older. Currently, there are four (4) residents residing in the facility, 60 and older. LPA Calderon explained to Administrator Kelada, the purpose of the one-year Annual Inspection visit, and escorted LPA Calderon on a tour of the entire inside and outside facility grounds. As part of the inspection, LPA Calderon reviewed: Four (4) client service records, four (4) client medication records (MAR), three (3) staff records, and inspected the inside facility and outside grounds. The facilities’ last fire drill was conducted on 08/01/2024. The one-story residential home consists of four (4) client bedrooms, three (3) client bathrooms, living room, dining room, kitchen, staff room, office area, attached garage with washer and dryer/ storage area, backyard with table and chairs. No weapons are stored in the premises. Kitchen was inspected and observed to be clean and operational. A two-day supply perishable and seven-day supply of non-perishable foods are present in the facility. Emergency Water Storage is in the garage and kitchen area. LPA Calderon observed that all facility rooms are clean and in good repair. A comfortable temperature was observed, and the facility has central air and heating. LPA Calderon observed the following during inspection of client’s rooms: mattresses are in good condition, adequate lighting present, plenty of dresser/closet space is present, and all bed linens present. All bedrooms contain furniture, lighting fixtures and personal storage space as required, all beds have the required amount of linen and mattress covers, LPA Calderon observed fully stocked closet with bedding, towels, and toiletries supplies. Bathroom fixtures are clean, in good repair, and working properly and contain the required nonskid mats and grab bars. LPA Calderon observed bathrooms were found to be within Title 22 regulation. Bathroom #1 hot water temperature properly measured at 115 degrees Fahrenheit, bathroom #2 hot water temperature properly measured at 113 degrees Fahrenheit, and bathroom #3 hot water temperature properly measured at 112 degrees Fahrenheit. Kitchen hot water temperature properly measured at 118 degrees Fahrenheit. Facility (6) Carbon Monoxide and (6) Smoke Detectors hard wired operated and connected were tested and are working properly. The facility one (1) Fire Extinguishers was checked and found to be fully charged and accessible. All exit doors in the facility have alarm systems. The facility has a working landline telephone. All toxins and knifes are locked/secured and inaccessible to clients. Medications are centrally stored and in a locked storage cabinet. Facility 2 first aid kit is fully stocked with manuals was checked and in order. Outside grounds were toured and there is a pool observed. All Exits/ Walkways around the home were free of debris and hazards. Outside patio accessible to clients. Four (4) client files were reviewed and found to be complete. LPA Calderon reviewed four (4) resident medications (MAR) and they were all found to be administered according to doctor's orders. Three (3) staff files were checked and have the required documents. LPA Calderon noted the Administrator Renee Kelada Certification # 6042684740 expiration date of 12/14/2024 was valid at time of inspection. The facility does not handle client's money/cash resources and no surety bond is needed. Commercial General Liability Policy #PCI15669902203 policy period from 02/21/2024 to 02/21/2025 underwritten by Primary Care Insurance Company, coverage 1,000,000/3,000,000 is valid at time of inspection. Administrator Kelada to email LPA Calderon a full copy of the commercial insurance policy including all endorsements no later than 12/10/2024. All the required documents are posted in the facility in a clearly visible area. During the visit, LPA observed the facility infection control practices. LPA observed screening protocols for visitors, staff, and clients, sanitizing stations (Located in common areas and restrooms). LPA observed staff and clients were NOT wearing face coverings. LPA observed the facility has a 30-day supply of Personal Protective Equipment (PPE). LPA Calderon advised the Administrator Kelada to continuously monitor the Centers for Disease Control (CDC) website and Community Care Licensing Provider Informational Notices (PIN) for any updates relating to COVID-19 guidance. According to the California Code of Regulations (Title 22, Division 6, Chapter 8), LPA Calderon did not observe deficiencies therefore no citations were issued at this time. Annual Licensing Fee is CURRENT.
