On April 13, 2026, Licensing Program Analyst (LPA) Brandon Lopez made an unannounced visit to the facility to conducting the required annual inspection. LPA was greeted and granted entry by care giving staff after explaining the purpose for the visit. Administrator (AD) Claudia Olteanu was notified via telephone and later arrived to assist with the inspection. LPA observed that Administrator Claudia Olteanu has a valid Administrator certificate which expires on June, 13, 2026. The facility is a Residential Care Facility for the Elderly (RCFE) licensed for six non-ambulatory residents, of which six may be bedridden, and has a hospice waiver for two. The facility is a single story home with six resident bedrooms, four shared resident bathrooms, three staff bedrooms, a living room, a dining room, a kitchen, a staff office, a laundry room, a storage room, an attached two car garage, and an attached ADU with three staff bedrooms, a staff kitchen, and a staff living room. LPA, accompanied by the AD, conducted a tour of the interior portion of the facility. On today's visit, LPA observed six residents in care and three care giving staff present. LPA observed residents relaxing in their respective bedrooms and in the living room. LPA observed the See Something, Say Something poster (PUB 475) mounted by the entryway of the facility. LPA inspected the six resident bedrooms and they were observed to be free of hazards. Resident bedrooms had the required furnishings of a bed, a chair, a chest of drawers, and a lamp. Resident linens and blankets were clean. LPA observed additional linens to be stored in the storage room. LPA inspected the four resident bathrooms and they were observed to be clean. Resident bathrooms were equipped with grab bars and non skid floor mats. Faucets and toilets were operational. Hot water temperature measured between 109.2 and 116.2 degrees Fahrenheit. LPA observed all staff bedrooms to be kept locked and inaccessible to residents in care. CONTINUED ON LIC809-C LPA observed the kitchen has a two day perishable and a seven day non-perishable food supply on hand. LPA observed kitchen appliances to be clean and operational. The five burner gas stove lights unassisted. LPA observed kitchen knives and sharps to be stored in a locked kitchen cabinet. LPA observed chemicals and toxins to be stored in a locked kitchen cabinet under the sink. LPA observed fire extinguishers to be mounted in the living room and in the resident hallway. Fire extinguishers were observed to be charged and serviced as of January 22, 026. LPA tested the dual smoke detectors/carbon monoxide detectors which tested operational. LPA observed the facility conducted their last emergency disaster drill on April 10, 2026. The centrally stored medication is kept in a locked hallway closet. LPA observed a first aid kit to be stored in the locked hallway closet and it had all the required components. LPA observed Personal Protective Equipment and hygiene supplies to be stored in the storage room. LPA observed chemicals and toxins to be stored in a locked cabinet located in the laundry room. LPA observed the door leading to the two car garage is kept locked and inaccessible to residents in care. LPA observed the two car garage to be used for storage. LPA observed the facility has a three day emergency food and water supply stored in the garage. LPA observed the door leading to the attached ADU is kept locked and inaccessible to residents in care. LPA observed the ADU is for staff use only and is off limits to residents in care. LPA, accompanied by the AD, conducted a tour of the exterior portion of the facility. LPA observed the exterior to be clear of obstructions and hazards. LPA observed a shaded outdoor seating area with furniture for resident use. The perimeter gates of the facility are self-latching and can be opened in an evacuation. There are no bodies of water on the premises. LPA reviewed all six resident files. All the required documentation were present and current in the resident files reviewed. LPA reviewed all six residents’ medication and medication administration records. LPA reviewed the Personal and Incidental funds (P&I) expense monies for the six residents. LPA reviewed four staff files. All staff are background cleared and associated to the facility. Based on today's observations, no deficiencies are being cited per Title 22 of the California Code of Regulations.
