Type of visit: Required - 1 Year
On August 8, 2025, Licensing Program Analyst (LPA) Brandon Lopez made an unannounced visit to the facility to conduct the required annual inspection. LPA was greeted and granted entry into the facility by care giving staff after explaining the purpose for the visit. Licensee (LI) Sonia Garcia and Administrator (AD) Jonathan Harlan were notified via telephone and later arrived to assist with the inspection. LPA observed that Jonathan Harlan has a valid Administrator certificate which expires on September 3, 2025. The facility is a Residential Care Facility for the Elderly (RCFE) licensed for six non-ambulatory residents and has a hospice waiver for six. The facility is a single story home with six private resident bedrooms, one staff bedroom, five resident bathrooms, a living room, a dining room, a kitchen, a TV room, a lounge, and an attached three car garage. LPA, accompanied by the AD, conducted a tour of the interior portions of the facility. On today's visit, LPA observed six resident in care and two care giving staff present. LPA observed residents watching TV in the living room. LPA observed the See Something, Say Something poster, (PUB 475) mounted on the wall by the entryway of the facility. LPA inspected all six resident bedrooms and observed them to be free of hazards. LPA observed resident bedrooms to have the required furnishings of a bed, a chair, a chest of drawers, and a lamp. LPA observed resident beds to have clean linens and blankets. LPA observed additional linens to be stored in a hallway cabinet. LPA inspected the five resident bathrooms and observed them to be clean. Bathrooms were equipped with grab bars and non-skid floor mats. Faucets and toilets were operational. Hot water temperature measured between 108.3 to 111.9 degrees Fahrenheit. LPA observed the staff bedroom to be kept locked and inaccessible to residents in care. LPA observed the kitchen has a two day perishable and a seven day non-perishable food supply on hand. LPA observed kitchen appliances to be clean and operational. CONTINUED ON LIC809-C LPA observed the four burner gas stove lights unassisted. LPA observed kitchen knives and sharps to be stored in a locked kitchen cabinet. LPA observed chemicals and toxins to be stored in a separate locked kitchen cabinet under the sink. LPA observed fire extinguishers to be mounted on the wall in the kitchen and it was observed to be charged and serviced as of March 6, 2025. LPA tested the individual smoke detectors/carbon monoxide detectors which tested operational. LPA observed the facility conducted their last emergency disaster drill on August 8, 2025. LPA observed the centrally stored medication to be kept in a locked kitchen cabinet. LPA observed the facility has a First Aid Kit stored in a closet by the entryway of the facility and it had all the required components. LPA observed the facility has a three day emergency food and water supply stored in the closet by the entryway of the facility. LPA observed the door leading to the attached three car garage to be kept locked and inaccessible to residents in care. LPA observed the three car garage to be used for storage and laundry. LPA observed chemicals and toxins to be stored in a locked cabinet in the garage. LPA observed additional incontinence supplies stored in the garage. LPA, accompanied by the AD, conducted a tour of the exterior portion of the facility. The exterior portion was observed to be free of hazards and obstructions. LPA observed a shaded outdoor seating area with furniture for resident use. LPA observed the perimeter gates of the facility to be self latching and can be opened in an evacuation. There are no bodies of water on the premises. LPA reviewed all six resident files. All the required documentation were present and current in the resident file reviewed. LPA reviewed the residents' medication and medication records. LPA reviewed five staff files. All staff are background cleared and associated to the facility. Based on the observations made during today's visit, no deficiencies are being cited per the Title 22 of the California Code of Regulations. Type of visit: Required - 1 Year
On this day, Licensing Program Analyst (LPA) Kevin Saborit-Guasch made an unannounced visit to the facility for the purpose of conducting the Required Annual Inspection. LPA was greeted and granted entry by facility caregiving staff after introducing himself and stating the reason of the visit. Administrators Kelly Francia and Jonathan Harlan arrived later to assist with the visit. During the inspection, LPA and staff conducted a tour of the physical plant and observed the following: The facility is a one-story home with six private bedrooms in addition to one staff room. There are five bathrooms throughout the facility. All bathrooms are observed to be equipped with grab bars and slip mats. All resident bedrooms have the required furnishings. LPA observed all beds have linen and blankets. One bed is observed to be equipped with full bed rails and another one with half rails. Physician order and hospice plan of care reviewed as well as physician order for the half rails. There are currently five residents admitted to the facility, one of which is receiving hospice care. Bathrooms faucets and toilets are operational. Water temperature was measured to be approximately 116F and 118F. LPA observed emergency disaster plan with means of exiting and emergency phone numbers listed and posted. Drills are conducted quarterly, and are documented and reviewed. LPA observed the facility has a 2-day supply of perishables and a 7-day supply of non-perishable. Smoke and carbon monoxide detectors tested operational. Fire extinguisher present is fully charged and has been maintained in 2024. There is adequately shaded outside space with outdoor furniture present. There are self-latching gates on both sides of the property. The routes of egress are free of obstructions. There are no bodies of water on the premises. CONTINUED ON FORM LIC809-C CONTINUED FROM FORM LIC809 Medication, cleaning products and sharp items are confirmed to be inaccessible throughout the physical plant. The medication central storage was also observed to be secure and reviewed to be accurate and up to date with the resident's prescription orders. LPA reviewed five resident files and three staff files. Three staff and three resident interviews conducted. Resident records include all necessary components. All staff members are confirmed to be cleared and associated with this particular licensed location. Training records, health screenings and CPR training are on file and up to date. Based on the observations made during today’s inspection, no deficiencies are being cited per Title 22 Division 6 of the California Code of Regulations. Type of visit: POC
Licensing Program Analyst (LPA) Claudia Gutierrez made an unannounced visit for the purpose of conducting a Plan of Correction (POC) inspection. LPA was greeted and granted entry by Staff Stephanie Magana and explained the purpose of the inspection. Administrator (AD) Sonia Garcia was contacted by phone and arrived at 2:48 p.m. LPA is following up on deficiencies cited during facility’s annual inspection on 11/06/23. Deficiency 87202(a) was cited due to lounge room being used as staff bedroom. During today’s visit, LPA confirmed lounge room is no longer being used a staff bedroom and is free of personal belongings. Deficiency under health and safety code 1569.695(c) was cited because disaster drills were not being conducted. AD provided LPA with a copy of disaster drill log, which indicated facility conducted a fire drill on 11/26/23. AD has met both POCs and deficiencies previously cited are being cleared. Based on today’s observations no deficiencies are being cited per Title 22 Division 6 of the California Code of Regulations. Type of visit: Required - 1 Year
Licensing Program Analyst (LPA) Claudia Gutierrez made an unannounced visit for the purpose of conducting a Required/Annual Inspection. LPA was greeted and granted entry by Staff Roy Bueno. LPA discussed the purpose of the inspection and Administrator (AD) Sonia Garcia and Kelly Francia were contacted by phone and arrived at 9:46 a.m. During the inspection LPA and ADs conducted a tour of the inside and outside of the facility, common areas, resident rooms, kitchen, garage and observed the following: This is a one-story home with six resident bedrooms, five bathrooms, and one staff bedroom, two living room areas, one dining area, one lounge room and attached three car garage. The lounge room is currently being used as a staff bedroom and contains a bunk bed; a deficiency was cited on this date. All resident bedrooms had the required furnishings. LPA observed all resident beds had linens and blankets. LPA observed all windows were screened. The back yard has a shaded sitting area. LPA observed two staff and five resident present. Bathrooms were observed to be free of debris and mildew, faucets and toilets were operational. Water temperature tested between 109.7-115.5 F degrees. LPA observed emergency disaster plan with means of exiting and emergency phone numbers listed and posted. Food menu was also posted and visible. LPA observed the facility has a 2-day supply of perishables and a 7-day supply of non-perishable food as required by regulations. Smoke detectors and carbon monoxide detectors tested operational. Fire extinguisher was observed to be fully charged. Appliances were all inspected. Sharps were observed locked in a kitchen drawer. All and any toxic chemicals, cleaning solutions, laundry toxins and disinfectants are inaccessible to residents. Medication cabinet was observed to be locked. LPA reviewed five resident files and four staff files. LPA interviewed staff and residents present. Based on the observations made during today’s inspection, two deficiencies are being cited per Title 22 Division 6 of the California Code of Regulations.
