On 07/01/2026 licensing program analyst Marisa Chiarelli arrived at the facility announced for a case management visit. LPA Chiarelli met with Anthony Faulkner and explained the purpose of the visit. Following an annual inspection conducted on 06/11/2026 LPA Chiarelli was informed that Anthony Faulkner wanted to become the new administrator for Sunshine assisted living - The House. Anthony would be replacing the previous administrator Maria Boulangger Falla who is no longer working at the facility since April 2026. It was determined that Anthony's administrator certificate is expired. Resident Care Coordinator Dayna Mock is getting her administrator certificate and possibly will be taking over as administrator for The House. On 06/11/2026, Licensing program analyst (LPA) Marisa Chiarelli arrived to the facility unannounced to conduct a one year required inspection. LPA Chiarelli met with administrator Anthony Faulkner and explained the purpose of the visit. LPA Chiarelli and administrator toured the facility together to ensure the health and safety of residents in care. Areas toured included but not limited to, residents bedrooms, bathrooms, and kitchen. In the areas toured no immediate health, safety, or personal rights violations were observed. Staff and resident files were reviewed (all staff and residents files are kept in sister facility Sunshine Assisted Living - The Cottage). Medication is kept in a locked room. LPA Chiarelli did a medication review. The common area was clean, odor-free and in good repair. All bedrooms had required furniture, bedding, and lighting. The bathrooms were clean and in good repair. The kitchen was clean and in good repair. Sharps are kept in a locked drawer. Food appears to be stored and prepared properly. Facility has required (7) seven-day non-perishable and (2) day perishable supply of food. The facility was observed to be at a comfortable temperature. First aid kit fully stocked and ready for emergency use. Fire extinguisher fully charged. Smoke and carbon monoxide detectors are all in working order. Hot water temperature measured within required Title 22 regulations of 105 degrees F and 120 degrees F. All employees requiring background checks are cleared. No pools/bodies of water are on the premises. No firearms are on premises. On July 28, 2025, Licensing Program Analyst (LPA) Kayla Adkison, arrived at the facility unannounced to conduct a 1-Year Required Annual Inspection. LPA met with Facility Administrators, Anthony Faulkner and Maria Boulangger Falla, and explained the purpose of the visit. LPA and Administrators toured facility together to ensure health and safety of residents in care. LPA observed the facility to be clean, in good repair and odor-free. In the areas toured no immediate health, safety, or personal rights violations were observed. Areas toured include but are not limited to: common areas, resident bedrooms, kitchen, common restrooms, storage area, and exterior courtyards. Facility has a 2-day perishable and a 7-day non-perishable amount of food. LPA observed all sharps and cleaning supplies to be kept locked and inaccessible to residents in care. LPA observed the medications to be kept in a locked room and inaccessible to residents. LPA observed fire extinguishers, fire detectors, and carbon monoxide detectors which were last inspected in July 2025. LPA observed the first aid kit to be complete and ready for emergency use. The facility was unable to provide documentation that the emergency disaster drills has been conducted since December, 2024. LPA reviewed a total of three (3) residents' files and three (3) staff files. All staff files contained the required documentation. Two of three resident files were missing record of a tuberculosis exam. Deficiencies cited from Title 22 Regulations and or the California Health and Safety Code. On 06/20/2024, Licensing Program Analyst (LPA) Jaynae Boyles, arrived at the facility unannounced to conduct a 1-Year Required Annual Inspection. LPA met with Facility Administrator, Chad Young, and explained the purpose of the visit. LPA Boyles and Administrator toured facility together to ensure health and safety of residents in care. LPA observed the facility to be clean, in good repair and odor-free. In the areas toured no immediate health, safety, or personal rights violations were observed. Areas toured include but are not limited to: common areas, resident bedrooms, and common restrooms. LPA observed each bathroom to have the necessary grab bars, non-skid flooring or shower chair, paper towels, and trash can with lids. Facility has a 2-day perishable and a 7-day non-perishable amount of food and sharps to be locked. Hot water temperature was measured above 120 degrees F in three locations within the facility. LPA observed the medications to be locked and inaccessible to residents. LPA observed fire extinguishers, fire detectors, and carbon monoxide detectors. LPA observed the first aid kit to be complete and ready for emergency use. LPA observed the emergency disaster plan to be complete but not updated or reviewed since 2021. The facility was unable to provide documentation that the emergency disaster drills were conducted over the last 12 months. LPA reviewed a total of three (3) residents' files and five (5) staff files which contained all of the required documentation. Several topics were discussed.
