Type of visit: Required - 1 Year

This unannounced inspection is being conducted by Licensing Program Analyst (LPA) Sean Haddad for the purpose of conducting a Required – 1 Year Inspection. LPA met with Administrator (AD) Ines Otbo and discussed the purpose of the inspection. LPA reviewed Infection Control requirements. At about 3:30PM, LPA and AD conducted a tour of the inside and outside of the facility, common areas, resident rooms, kitchen, and garage and observed the following: Structure: facility is a 5-bedroom, 4-bathroom, two-story house with an attached garage that is used for storage. There is a back yard with a patio cover for the residents. LPA observed 2 staff and 6 residents present at the facility in addition to AD. Resident Bedrooms: the 4 resident bedrooms are spacious and will easily accommodate the residents’ furnishings. Furniture for each resident bedroom inspected. Staff Bedrooms: LPA inspected the 1 staff bedroom. Bathrooms: the bathrooms were clean, faucets and toilets were operational. Water temperature: tested between 107 and 108 degrees F in the 2 resident bathrooms. Linens & Hygiene Supplies: new linens and fully stocked linen closets were observed. Emergency Phone Numbers, Exit Plan & Menu: reviewed. Food Service: LPA observed the facility has a 2-day supply of perishables and a 7-day supply of non-perishable food is available as required by regulations. Carbon Monoxide, Smoke Detectors, Fire Extinguisher: observed and tested. Appliances: stove burners, microwave, washer, and dryer inspected. Knives: observed locked in the kitchen. Toxins: observed locked in the laundry room. Medication cabinet: observed to be locked. First-Aid Kit and Activity Supplies: observed and available. Facility’s licensing fees are paid. At about 2:15PM, LPA reviewed 5 resident files and 3 staff files, interviewed 2 residents and 2 staff, and inspected medications for 5 residents. Facility does not handle resident money. During the inspection, LPA and AD observed the following: based on documents, the licensee did not have complete documentation of quarterly emergency disaster drills for the past year. Based on the observations made during today’s inspection, deficiencies are being cited per Title 22 Division 6 of the California Code of Regulations. See LIC809D. During the inspection, LPA and AD observed the following: based on documents and admission, the licensee did not ensure 1 out of 2 staff had completed the 20-hour annual training; based on documents, the licensee has been giving one resident supplements and over the counter allergy medication without a doctor's written order; based on observation and admission, the facility has two floors but does not have an evacuation chair. Based on the observations made during today’s inspection, deficiencies are being cited per Title 22 Division 6 of the California Code of Regulations. See LIC809D. On 7/29/2024, Licensing Program Analyst (LPA) Janette Romero conducted an unannounced visit to the facility for a required annual inspection. LPA was greeted and granted entry by Caregiver, Abigail Ortiz who was informed of the purpose of the visit. Administrator, Ines Otbo arrived during the visit and was also informed of the purpose of LPA's visit. The facility has a fire clearance for six (6) non-ambulatory elderly residents and an approved hospice waiver for five (5), and LPA was informed there are currently two (2) residents receiving hospice services at the facility. LPA toured the facility with Business Manager (BM), Michelle Otbo and observed the facility is made up of a two-story home with four (4) resident bedrooms, two (2) bathrooms, two (2) staff rooms, a kitchen, dining room, living room, and attached garage. During the tour, BM Otbo tested one (1) of the smoke alarm/carbon monoxide detectors and LPA observed it to be operational. LPA also observed charged fire extinguishers mounted throughout the facility. Indoor and outdoor passageways were free of obstruction. The facility has outdoor shaded seating for the residents in care. No bodies of water were observed on the premises. Medications are stored in a dining room cabinet that is secured with a master lock. Resident bedrooms had the required furniture and lighting and resident bathrooms had grab bars in the shower. LPA toured the kitchen and observed food was stored in a safe and healthful manner. The facility had more than a 2-day supply of perishable foods and 7-day supply of non-perishable food items. Additional food, water and incontinent supplies are stored in the garage. LPA reviewed random staff and resident files. Resident files reviewed had signed admission agreements and updated medical assessments. Staff present had a criminal record clearance and a valid first aid/CPR certification. During today's visit, LPA did not observe any issues or concerns.
