Allegation investigated: Staff hit resident. Resident sustained an injury due to staff neglect or physical abuse.

Licensing Program Analyst (LPA) Cynthia Chan conducted a subsequent visit to continue the investigation regarding the allegations listed above. LPA met with licensee, Carina Demman, and explained the purpose of the visit. During the initial visit on 5/5/26, LPA collected a copy of the resident roster and interviewed the Administrator, Staff #1-#2, and two (2) residents. LPA interviewed two (2) more residents today. The investigation revealed the following: Allegation – Staff hit resident. It is alleged that a staff hit Resident #1 (R1). LPA interviewed three (3) staff, and all three (3) denied hitting the resident(s). Staff stated that Staff #1 (S1) was changing and cleaning R1 when R1 struck S1 on the temple of the face. S1 did not retaliate or hit back. Unsubstantiated S1 finished cleaning R1 and left the room. Staff interviewed stated they have never seen a staff hit a resident. LPA interviewed four (4) residents, and none had seen staff hit a resident. Residents stated the staff treat them well. Allegation – Resident sustained an injury due to staff neglect or physical abuse. It is alleged that Resident #1 (R1) fell and sustained a skin tear due to staff neglect. Staff stated that in the early morning on 4/8/26, R1 fell and had a skin tear. Staff checked on R1 around 4 am and found the resident on the floor. Staff reported the incident to the family member who took R1 to the doctor. According to staff, R1 did not ask for assistance while residing at the facility. Staff would often check on R1 and offer to assist R1 with the activities of daily living (ADLs); however, R1 declined most of the time. R1 did things himself/herself such as dressing and toileting. Staff were able to assist the resident with showers and cleaning the room with permission. Interviews with residents revealed that staff check on them, assist them with ADLs, and have never been injured due to staff neglect. Although the allegations may have happened or are valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur; therefore, the allegations are UNSUBSTANTIATED. Licensing Program Analyst (LPA) Cynthia Chan conducted the required annual inspection using the Compliance and Regulatory Enforcement (CARE) tools. LPA met with staff and explained the reason for the visit. The administrator, Carina Demman, arrived shortly after to assist with the visit. The fire clearance is granted for six (6) residents, ages 60 and above. The rooms are cleared for non-ambulatory residents. Facility has a hospice waiver approved for (1) resident. The facility is a single story house consisting of 7 bedrooms (5 resident rooms & 2 staff rooms), 3.5 bathrooms, living room, family room, dining room, and kitchen, laundry room, and an attached garage. The resident bathrooms have non-skid mats and grab bars. The facility is well maintained and has an outdoor area with a city view. The smoke detectors and carbon monoxide detector are operable. Knives, cleaning supplies, and medications are locked. Facility has sufficient space to accommodate indoor and outdoor activities. Sufficient food supplies of 2-day perishable and a week of non-perishable items are observed. Information for appropriate reporting agencies are posted. The facility can accept or retain residents with dementia. The liability insurance is current for the coverage of at least $1 million (per occurrence) and $3 million (total annual aggregate). LPA reviewed the licensee/administrator and (2) other staff file. The Administrator's certificate expires on 12/12/25. Staff have current CPR & First Aid certificates. They are fingerprint cleared and associated. LPA reviewed (6) resident files and their medications. The files contain the required documents and the medications are given as prescribed. The Emergency Disaster Plan is reviewed at least annually and facility is conducting drills monthly. No deficiencies issued today. An exit interview was held. The following 12 (CARE) tool domains were utilized during the inspection: Infection Control: Infection control practices and Personal Protective Equipment (PPEs) were observed. COVID-19 screening is no longer in place. Infection Control Plan observed. Operational Requirements: A current Plan of Operation observed. A fire clearance for 6 residents of which (6) may be non ambulatory Hospice care waiver approved for one (1) resident. Physical Plant/Environment Safety: Facility is a single story home consisting of 7 bedrooms (5 resident rooms & 2 staff rooms), 3 bathrooms, living room, family room, dining room, kitchen, laundry room, backyard with shaded patio furniture, and an attached garage The interior and exterior physical plant was inspected. Exit doors are free of any obstruction and there are no pools or large bodies of water. Cleaning supplies and toxic substances are inaccessible. Water temperature readings measured within the required 105 - 120 degrees Fahrenheit. Staffing: A total of (4) staff members provide supervision to the residents.
