On April 8, 2026 Licensing Program Analysts (LPAs) Brandon Lopez and Nancy Guillen made an unannounced visit to the facility to conduct the required annual inspection. LPAs were greeted and granted entry into the facility by staff after explaining the purpose for the visit. Administrator (AD) Ester Dela Cruz was notified via telephone and later arrived to assist with the inspection. LPAs observed that Ester Dela Cruz has a valid administrator certificate which expires on March 28, 2027. The facility is a Residential Care Facility for the Elderly (RCFE) licensed for six non-ambulatory residents, and has a hospice waiver for five. The facility is a single story home with four resident bedrooms, two of which can be shared, one staff bedroom, three bathrooms, a living room, a dining room, a kitchen, and an unattached two car garage. LPAs, accompanied by the AD, conducted a tour of the interior portion of the facility. On today's visit, LPAs observed four residents in care and two staff present. LPAs observed residents relaxing in their respective bedrooms and in common areas. LPAs observed the See Something, Say Something poster (PUB 475) mounted on the wall by the dining room. LPAs inspected the four resident bedrooms and they were observed to be free of hazards. Resident bedrooms had the required furnishings of a bed, a chair, a chest of drawers, and a lamp. LPAs observed that Resident #4 (R4) has full bed rails, however, R4 is currently not receiving hospice care. LPAs observed resident beds had clean linens and blankets. LPAs observed additional linens to be stored in a hallway cabinet. LPAs tested the individual smoke detectors in each of the resident rooms which tested operational. LPAs observed the staff bedroom to be kept locked and inaccessible to residents in care. LPAs inspected the three bathrooms and they were observed to be clean. Resident bathrooms were equipped with grab bars and nonskid floor mats. Faucets and toilets were operational. Hot water temperature measured between 107.7 and 109.7 degrees Fahrenheit. CONTINUED ON LIC809-C LPAs observed the kitchen has a two day perishable and a seven day nonperishable food supply on hand. LPAs observed kitchen appliances to be clean and operational. LPAs observed the four burner gas stove lights unassisted. LPAs observed kitchen knives and sharps to be stored in a locked kitchen cabinet. LPAs observed chemicals and toxins to be stored in a locked kitchen cabinet under the sink. LPAs observed three fire extinguishers to be mounted in the facility. Fire extinguishers were observed to be charged and serviced as of March 10, 2026. LPAs tested the individual carbon monoxide detectors which tested operational. LPAs observed the facility did not complete an emergency disaster drill in the previous quarter with the last emergency disaster drill completed on November 25, 2025. LPAs observed the centrally stored medication to be stored in a locked storage cabinet in the kitchen. However, LPAs observed that there were two medications stored in an hallway cabinet with a broken lock. LPAs observed the facility has a first aid kit stored in a closet by the living room and it had all the required components. LPAs observed a fire place in the living room to not be in operation at time of visit. LPAs, accompanied by the AD, conducted a tour of the exterior portion of the facility. The exterior portion was observed to be free of hazards and obstructions. LPAs observed a shaded outdoor seating area with furniture for resident use. LPAs observed the perimeter gates of the facility to be self latching and can be opened in an evacuation. LPAs observed the door leading to the unattached garage to be kept locked and inaccessible to residents in care. LPAs observed the garage to be used for storage. LPAs observed the facility has a three day emergency food and water supply to be stored in the garage. There are no bodies of water on the premises. LPAs reviewed all four resident files. All the required documentation were present and current in the resident files reviewed. LPAs reviewed all four residents' medication and medication records. LPAs reviewed five staff files. All staff are background cleared and associated to the facility. Based on the observations made during today's visit, deficiencies are being cited on the attached LIC809-D pages. LPA observed kitchen appliances to be clean and operational. LPA observed the four burner gas stove lights unassisted. LPA observed kitchen knives and sharps to be stored in a locked storage container in the kitchen. LPA observed chemicals and toxins to be stored in a locked kitchen cabinet under the sink. LPA observed fire extinguishers to be mounted on the wall in the kitchen and near the exterior shaded outdoor seating area. Fire extinguishers were observed to be charged and serviced as of April 29, 2024. LPA tested the individual carbon monoxide detectors which tested operational. LPA observed the facility conducted their last emergency disaster drill on March 31, 2025. LPA observed the centrally stored medication to be stored in a locked storage cabinet in the kitchen. LPA observed the facility has a First Aid Kit stored in a closet by the living room and it had all the required components. LPA observed a fire place in the living room to not be in operation at time of visit. LPA observed the facility has an internet device for dedicated resident use stored in the dining room. LPA, accompanied by the AD, conducted a tour of the exterior portion of the facility. The exterior portion was observed to be free of hazards and obstructions. LPA observed a shaded outdoor seating area with furniture for resident use. LPA observed the perimeter gates on the west side and east side of the facility to be self latching and can be opened in an evacuation. LPA observed two laundry detergent containers left unattended by the unattached garage. LPA observed the door leading to the unattached garage to be kept locked and inaccessible to residents in care. LPA observed the garage to be used for storage. LPA observed the facility has a three day emergency food and water supply to be stored in the garage. LPA reviewed all four resident files. All the required documentation were present and current in the resident files reviewed. LPA reviewed all four residents' medication and medication records. LPA reviewed four staff files. LPA observed that the facility did not have a Health Screening report on file for the four staff. All staff are background cleared and associated to the facility. Based on the observations made during today's visit, deficiencies are being cited on the attached 809-D. An exit interview was conducted with Administrator Ester Dela Cruz. On April 26, 2024, at 9:10 am, Licensing Program Analyst (LPA) Edward Kim and Licensing Program Manager (LPM) Lourdes Montoya conducted a continuation visit for a required 1-year annual visit. LPA Kim and LPM Montoya were greeted and gained entry to the facility by staff. Administrator Ester Dela Cruz arrived 10:00 am. LPA Kim conducted a record review on resident files (R1-R5) and staff files (S1-S2). LPA Kim conducted a resident interview (R5) and staff interviews (S1-S3). LPA Kim reviewed resident medications and medication administration record. Deficiencies were cited during this inspection visit according to the California Code of Regulations (Title 22, Division 6, Chapter 8). Civil Penalty assessed. On April 17,2024 8:30am, Licensing Program Analyst (LPA) Edward Kim and Licensing Program Manager (LPM) Lourdes Montoya conducted an unannounced required 1-year annual visit using the CARE Inspection Tool. Upon arrival at the facility, LPA Kim and LPM Montoya met with Administrative (AD) Ester De La Cruz and explained the purpose of the visit. The facility is licensed to operate for six (6) non-ambulatory residents and have a hospice waiver for 5 residents. The facility is a single-story structure located in a residential neighborhood. It consists of the following: four (4) resident bedrooms, one (1) staff bedroom, three (3) bathrooms, living area, dining area, kitchen, and outside covered patio area. LPA Kim and LPM Montoya toured physical plant with AD De La Cruz. There were no bodies of water or obstructions on the premises. All rooms were inspected. Beds and bedding supplies were in good condition, adequate lighting was provided, storage for resident’s personal belongings was observed. Bed linens, comforters, and bath towels were adequately stocked at the time of visit. The Resident’s rooms were inspected: Room 1, Room 2, Room 3, Room 4, and 1 Staff Bedroom. Bathrooms were found to be within Title 22 regulations and were clean and operational. The water temperature measured at 134 degrees F in Resident 3 bathroom, 132 degrees F in the common bathroom, and 137.6 degrees F in Resident 2 Bathroom. A comfortable temperature of 73 degrees F was maintained in the facility. LPA Kim and LPM Montoya observed the facility to be sanitary and appropriately furnished. The Kitchen was inspected and there is sufficient supply of two (2) day perishable and seven (7) day non-perishable food available. The facility has operable smoke detectors and carbon monoxide. A working telephone (714-525-1566) remains available.. Evaluation Report Continues on LIC 809-C During the visit, LPA Kim observed the facility's infection control practices. LPA observed screening protocols for visitors, staff, and residents, and sanitizing stations in common areas and restrooms. LPA observed the facility has a 30-day supply of Personal Protective Equipment (PPE). LPA Kim and LPM conducted an audit of residents #1-5 (R1-R5) service files, and staff #1-#2 (S1-S2) personnel files. One out of five resident's medical assessment is not current. The following observations were made: fire extinguisher was not mounted and last serviced on 2/10/2023, facility has an outdoor make shift kitchen with refrigerator, kitchen cabinets filled with canned food and fruits and eggs on the counter, administrator's certificate expired on March 28, 2023, first aid kit was incomplete with missing tweezers, manual, gauze, medical tape, thermometer, and no activity was observed during the entire visit. Due to time constraints these deficiencies will be addressed on the continuation inspection on a later date; and staff/resident interviews and medication administration records review will be conducted on the next visit. Deficiencies were cited during this inspection visit according to the California Code of Regulations (Title 22, Division 6, Chapter 8). Note: *Citations not cleared by the due date will be a $100 fine assessed for each citation until it is cleared.
