Type of visit: Required - 1 Year
Licensing Program Analyst (LPA) Melissa Parks arrived on Tuesday July 16, 2026, to conduct the unannounced annual inspection. During today's annual inspection, the Compliance and Regulatory Enforcement Tool was used. LPA reviewed resident (6) and staff (3) files. All resident files contained the required paperwork. All staff files contained the required paperwork and training. LPA and Administrator Placida toured the facility together to ensure the health and safety of residents in care. The following areas were inspected: resident bedrooms, bathrooms, kitchen, common areas and backyard. Facility was clean and well organized. Facility was current on fire drills. Water temperatures were within the required range. Facility had a fully stocked first aid kit. All required postings were observed. All chemicals and knives were kept locked and inaccessible to residents. Carbon monoxide detector was observed. Fire extinguisher had a current inspection tag. LPA obtained a copy of current liability insurance. Exit interview conducted. No deficiencies cited. Type of visit: Required - 1 Year
Licensing Program Analyst (LPA) Angela Hood arrived at the facility unannounced and met with the Administrator to conduct a Required-1 Year Inspection. LPA conducted an inspection of the care home to ensure compliance with Title 22 regulations. There are six (6) bedrooms and two (2) bathrooms for resident use. LPA observed bedrooms to be properly furnished, with appropriate bedding and lighting. The bathrooms were in sanitary condition, properly maintained, and the hot water temperature was observed to be 115.6 degrees F. LPA checked the kitchen area for the ability to prepare and store food. Care home has required (2) two-day perishable and (7) seven-day non-perishable food supply on hand. LPA observed knives, cleaning products and other toxins to be locked away and inaccessible to residents. LPA observed the outdoor area and perimeter of the care home to be free of clutter and debris and there appeared to be no potential safety hazards to the residents in care. Smoke and carbon monoxide detectors are operational. Fire extinguisher and first aid kit are maintained and ready for emergency use. LPA checked medication storage and found medications to be locked away and inaccessible to the residents. LPA reviewed three (3) resident files and also reviewed two (2) staff files. As a result of this visit, no deficiencies were cited per California Code of Regulations, Title 22. Type of visit: Required - 1 Year
On 6/13/2024, Licensing Program Analyst (LPA) Cassie Yang and Department of Consumer Affairs Associate Governmental Program Analyst (AGPA) Jordan McLaughlin arrived unannounced at the facility to conduct a required annual inspection utilizing the care tool. LPA met with caregiver and explained the purpose of the visit. Administrator arrived to the facility after tour was completed. Today's census is six residents in care with two residents on hospice services. Facility is licensed for six non-ambulatory with hospice waiver of six. During today's visit, LPA, AGPA and Caregiver conducted a tour of the interior and exterior of the facility. LPA informed Caregiver that master key to medication cabinet should be inaccessible to residents in care. LPA and AGPA observed bleach, laundry detergent and Pinesol located by the laundry area. Toxins were immediately stored after observation. Additionally, LPA and AGPA observed toilet cleaner solution located in the bathroom located near the common areas. File review conducted for four personnel and six residents. LPA and Administrator discussed that residents with dementia are to be medically assessed annually and residents with no dementia diagnosis medical assessment shall be updated when required by the Department. LPA observed facility files to be completed with the required documents. LPA observed the presence active facility liability insurance. LPA observed Administrator Certificate #6032256740 to be pending renewal. Deficiencies cited. Please see LIC 809-D.
