Type of visit: Required - 1 Year
Licensing Program Analyst (LPA) Lavinia Muscan arrived on December 2, 2025 to conduct the annual inspection. During today's annual inspection, the Compliance and Regulatory Enforcement Tool was used. LPA reviewed resident (3) and staff files (2). All resident files contained the required paperwork. All staff files contained the required paperwork. All staff have current first aid and CPR training. Facility was clean and well organized. Facility is current on fire drills. Staff training contained the required initial training. LPA and Manager toured the facility together to ensure the health and safety of residents in care. The areas toured included resident rooms, bathrooms, kitchen, and common areas and outside area.The food supply is within compliance, 2 days of perishable and 7 days worth of non-perishable food items. Grab bars were present at the toilet and in the shower. All exits were unobstructed. The disaster drill is current. The administrator's certificate is current. There is a side gate for emergency access. LPA checked the kitchen area for the ability to prepare and store food. Knives and Sharp objects found to be locked . LPA observed cleaning products and other toxins to be locked away. LPA observed the area used for medication to be locked and inaccessible to residents. LPA observed smoke detectors and carbon monoxide detector at the care home are operational. Fire extinguisher is ready for emergency use. Water temperature is within compliance. In the areas toured, there were no health or safety violations observed. LPA requested a copy of the LIC 500, LIC610E and current liability insurance to be sent to the Department by end of the month. Jan 22, 2025Facility evaluation reportReport on file
Type of visit: Required - 1 Year
On 1/22/2025 LPA Tryon visited the facility to do an annual visit and inspection. LPA was greeted by staff, who contacted the Administrator and licensee. Administrator and licensee arrived a short time later. LPA toured the facility including common areas, kitchen, bedrooms, bathrooms, hallways, storage areas, yard. Food supplies were reviewed and appear adequate to meet the requirement of 2 days perishable and 7 days non-perishable. Food appears varied and of generally good quality. Medications are centrally stored, logged and locked. Some medications are kept in a separate, locked area. The house is clean, nicely-furnished and well-taken care of. There is a large patio in back that is enclosed by a fence, allowing plenty of outside space. All bedrooms are spacious and appropriately furnished; the facility has 6 individual, private rooms for residents. Smoke detectors and carbon monoxide detectors installed, fire extinguisher charged. Bathrooms are clean and functional. LPA reviewed 2 resident files and 2 staff files. LPA spoke with 2 staff, and one resident who was able to answer questions. LPA completed the CARE Tool with licensee and administrator. LPA requested updated copies of LIC 500, Liability Insurance. LPA viewed current Administrator Certificate on the wall of entryway. Type of visit: Required - 1 Year
Licensing Program Analyst (LPA) Cassie Yang arrived unannounced at the facility to conduct a require annual inspection utilizing the care tool. LPA met with house manager, Kelly Conley, and explained the purpose of the visit. Today's census is six residents with two residents on hospice services, compliance to facility's licensure. LPA and House Manager conducted a tour of the facility. Areas toured included but limited to: six private resident rooms, five bathrooms, kitchen and common areas. LPA observed four residents present in the common areas and two residents in their bedrooms. LPA observed the facility to be 76* at a comfortable temperature for residents in care. LPA observed the facility to have 2+ days of perishable and 7+ days on nonperishable foods. LPA observed facility to be free from safety hazards. LPA and House Manager discussed facility submitting R1's exception request. LPA provided a copy of California Code Regulation 87616 Exceptions for Health Conditions. Additionally, LPA and House Manager discussed annual fees as it is not up to date. LPA conducted a file review for personnel and residents in care. Care tool completed and no deficiencies cited.
