Allegation investigated: Staff did not ensure resident was appropriately clothed. Staff did not assist resident with hygiene care needs.
Licensing Program Analyst (LPA) Talwinder Bains arrived at the facility unannounced on 2/23/26 to deliver complaint findings for above allegations. LPA met with Administrator, Sid Ali Mesloub and explained the purpose of the visit. The department conducted interviews, facility observation and record review to investigate above allegations. During interviews with two (2) facility staff and three (3) residents, it has been discovered that facility was providing appropriate care to the residents based on resident’s documented needs and service plans. During department visits on 01/22/26, Department observed that staff were attentive to residents care needs and helping them with their care needs. Staff interviews reflected that facility provides adequate staffing. Resident’s interviews indicated their satisfaction with their care needs including toileting, dental, showers and other care needs and did not express any concerns in this area, therefore this allegation is UNFOUNDED. A finding that the allegations are unfounded means that the allegations are false, could not have happened, and/or is without a reasonable basis. Exit meeting conducted. Type of visit: Required - 1 Year
Licensing Program Analyst (LPA) Talwinder Bains arrived on 2/23/26 to conduct the annual inspection. LPA met with administrator, Sid Ali Mesloub who assisted LPA during today's inspection. During today's annual inspection, the Compliance and Regulatory Enforcement Tool was used. LPA reviewed three (3) residents and two (2) staff files. All residents and staff files contained the required paperwork. All required postings were observed. LPA and administrator toured the facility together to ensure the health and safety of residents in care. The areas toured included residents rooms, bathrooms, kitchen, common areas and outside area. The food supply is within compliance, 2 days of perishable and 7 days worth of non-perishable food items. All exits were unobstructed. The administrator's certificate is current. LPA checked the kitchen area for the ability to prepare and store food. LPA observed the area used for medication to be locked and inaccessible to residents. LPA observed smoke detectors and carbon monoxide detector at the care home are operational. Fire extinguisher was serviced on 2/2/26 and was ready for emergency use .Water temperature is within compliance (105 degree F) .Inside temperature was observed to be 74 degree F. Facility has a pool in the backyard area which was properly fenced, secured and locked. LPA requested a copy of the LIC500, LIC610E and current liability insurance to be sent to the Department by 3/10/26. Deficiencies were observed and cited per Title 22, CCR Regulations as listed on LIC 809-D. Civil penalties shall be assessed if facility does not comply with POC requirements which were issued today. Type of visit: POC
On 03/18/25, Licensing Program Analyst (LPA) Talwinder Bains conducted an unannounced visit for the purpose of a Plan of Corrections visit (POC's) that were issued during annual facility visit on 02/06/25. LPA met with Licensee/Administrator , Sid Ali Mesloub and explained the purpose of the visit. During today's visit, LPA observed fire extinguisher was serviced on 02/07/25 and was ready for emergency use. LPA audited all 6 residents files and found out that all residents were listed as Ambulatory or Non-ambulatory on thier Fire Clearance sections in their medical assessment (LIC602) per their physicians which was in compliance with facility's current 'Fire Clearance Status'. Staff files audit indicated that all current staff has required Physical Exam and TB test . Based on gathered information. all citations issued on 02/06/25 were cleared during today's visit as facility fulfilled the POC requirements . No citations were issued during this visit. On 02/13/25 at 01:30PM, an informal conference was conducted at Sacramento Regional Office. The purpose of this informal conference meeting is to discuss issues which were identified during facility’s annual visit on 02/06/25. Present in the meeting is, Licensing Program Manager (LPM) Laura Munoz, Licensing Program Analyst (LPA) Talwinder Bains and Licensee/Administrator, Sid Ali Mesloub. The purpose of the informal conference is to have open discussion related to the current issues at the facility. During this meeting the licensee was made aware that this Informal conference is a part of the Administrative Action process. The informal conference process was explained during this meeting. Issues discussed during the meeting were: I. Issues related to Fire Clearance II. Incomplete Residents Records III. Recent Deficiencies IV. Personal Records To support the facility maintaining substantial compliance with Health and Safety Statute and Title 22 regulations, the Department is developing a plan with the licensee to address causes for concerns. Plan to address compliance concerns by 03/10/25 : Submission of new LIC308, new LIC500, Completing the pending POC requirements issued on 02/06/25 Administrator was notified that the Department will provide additional case management visits and complete a referral to TSP (Technical Support Program). Type of visit: Required - 1 Year
Licensing Program Analyst (LPA) Talwinder Bains arrived on 02/06/25 to conduct the annual inspection. LPA met with Administrator, Sid Ali Mesloub and explained the purpose of today's visit. During today's annual inspection, the Compliance and Regulatory Enforcement Tool was used. LPA reviewed medications of two (2) residents comparing with physician orders. LPA reviewed two (2)residents files and two (2) staff files. LPA and Administrator toured the facility together to ensure the health and safety of residents in care. The areas toured included resident rooms, bathrooms, kitchen, and common areas and outside area. The food supply is within compliance, 2 days of perishable and 7 days worth of non-perishable food items. Grab bars were present at the toilet and in the shower. All exits were unobstructed. There is a side gate for emergency access. LPA checked the kitchen area for the ability to prepare and store food. LPA observed the area used for medication to be locked and inaccessible to residents. LPA observed smoke detectors and carbon monoxide detector at the care home are operational. Hot water temperature was observed to be 111 degrees F, which is within the regulation range of 105-120 degree. LPA requested a copy of the LIC308, LIC 500, LIC610E and current liability insurance to be sent to the Department by 02/21/25. Deficiencies were observed and cited per Title 22, CCR Regulations as listed on LIC 809-D. Civil penalties shall be assessed if facility does not comply with POC requirements which were issued today. On December 26, 2024, Licensing Program Analyst (LPA) Cassie Yang arrived unannounced at the facility to conduct a case management visit regarding the effective exclusion of an individual effective date December 26, 2024. LPA met with caregiver who then contacted Administrator who arrived to the facility shortly afterwards. LPA and Administrator discussed S1 worked at the facility for approximately a month. Administrator confirmed S1 is no longer working at the facility. S1 is now separate from facility roster on Guardian. No deficiencies cited. Type of visit: Post Licensing
7/16/2024, Licensing Program Analyst (LPA) Cassie Yang arrived unannounced at the facility to conduct a post licensing inspection. LPA met with Caregivers, and explained the purpose of the visit. Administrator then arrived shortly after tour was conducted. Today's census is four residents with one resident on hospice services, facility is licensed for six non-ambulatory, hospice waiver of six. LPA conducted a tour of the interior of the facility. Areas toured included but not limited to: kitchen, bathroom, residents bedrooms and the common areas. LPA observed four residents in care with two caregivers. LPA observed medications and sharps to be locked and inaccessible to residents in care. LPA informed Administrator that all staff are to have the required initial and annual training as per §1569.625 Staff training; legislative findings; contents. LPA and Administrator discussed first aid training for all staff and at least one staff per shift with cardiopulmonary resuscitation (CPR) certification. LPA and Administrator discussed that all residents file are to have physician orders for each medication. LPA and Administrator discussed keeping communications and charting notes with hospice/home health agencies regarding residents condition. File review conducted for residents and staff. Type of visit: Prelicensing
Licensing Program Analyst (LPA) Cassie Yang arrived to the facility to conduct a Pre-Licensing inspection. LPA met with applicant, Sid Mesloub, and explained the purpose of the visit. During today's inspection, LPA and Applicant conducted a tour of the interior and exterior of the facility. Areas toured included but not limited to: resident bedrooms, caregiver room, bathrooms, laundry room, kitchen and common areas. LPA observed all six resident bedrooms to have the proper furniture. LPA observed the locked storage space for knives, medications and toxins. LPA observed facility to have the appropriate postage in the hallway. LPA observed facility to have fire extinguishers, operable fire alarm and carbon monoxide present. LPA conducted a tour in the exterior and observe the pool to be locked and inaccessible. LPA and Applicant discussed ensuring the pool gate is locked at all times. LPA informed Applicant LPA will email Applicant a copy of Health and Safety Code for staff training and LIC 311F File Checklist. Today's inspection, LPA and Applicant completed Comp III and no corrections are needed. LPA informed Applicant LPA will notify CAB of prelicensing completion.
