Licensing Program Analyst (LPA) Nancy Guillen made an unannounced visit for the purpose of conducting a required annual Inspection. LPA was greeted and granted entry by staff after explaining the purpose of the visit. Administrator (AD) Daniel Datcu was notified via phone and later arrived to assist with the visit. LPA observed the administrator certificate was current with an expiration date of March 12, 2028. This is a Residential Care Facility for the Elderly (RCFE) licensed to six non-ambulatory residents with a hospice waiver for four. The facility is a one-story home with six resident bedrooms, one staff bedroom, four bathrooms, a living room, kitchen, dining room and an attached garage. During the inspection, LPA and AD conducted a tour of the inside and outside of the facility, common areas, resident rooms, kitchen, garage and observed the following: LPA observed residents watching television in their respective bedrooms. LPA observed six residents in care and three staff present. LPA observed the See Something Say Something Poster (PUB 475) mounted in the living room. LPA observed that all resident bedrooms had the required furnishings. LPA observed all resident beds had linens and blankets with additional linens stored in a shared bathroom cabinet. LPA observed bathrooms were clean and equipped with grab bars, toilet paper, paper towels, and non skid floor mats. Bathrooms were observed to be free of debris and mildew, faucets and toilets were operational. Water temperature tested between 105 and 110.4 degrees Fahrenheit. LPA observed all windows were appropriately screened. LPA toured the outside of the facility and observed outdoor passageways were free of obstruction. LPA observed the backyard had a shaded sitting area with furniture for resident use. Continued on LIC 809C LPA observed the facility had a 2-day supply of perishables and a 7-day supply of non-perishable food as required by regulations. Gas stove, microwave, dishwasher, washer and drier were inspected and observed to be operable. Smoke detectors and carbon monoxide detectors tested operational. Fire extinguisher was observed to be fully charged with a service date of July 11,2025 and located in the dining room. Toxic chemicals, cleaning solutions, and disinfectants were observed to be locked and inaccessible to residents under the kitchen sink. Medication cabinet was observed to be locked and centrally stored in a kitchen cabinet. LPA observed the First Aid Kit had all the required components. LPA observed the facility conducted their last emergency disaster drill on March 15, 2026, however names of staff present were not documented. LPA began review of the records. LPA reviewed six resident records. All the required documentation were present and current in the resident files reviewed, however R2 did not have a signed resident rights form on file. LPA reviewed two employee records. All employees present have a criminal record clearance and were associated to the facility. LPA observed records reviewed have a current First Aid certificate. Based on the observations made during today’s inspection, a deficiency is being cited per Title 22 Division 6 of the California Code of Regulations. Licensing Program Analyst (LPA) William Vanegas made an unannounced inspection for the purposes of conducting an annual inspection. Upon arrival LPA Vanegas was greeted and granted entry to the facility by Care Giver (CG) Gessica Tudurachi and explained the nature of the inspection. LPA Vanegas set up equipment and began a tour of the facility and observed the following. This is a one storied home with seven bedrooms six of which are resident rooms, and one of which is a staff bedroom. There are four bathrooms, two of which are private bathrooms, one of which is a shared bathroom, and one of which is a shared staff and visitor bathroom. LPA Vanegas observed kitchen area to be clean and free of debris. LPA Vanegas observed there to be a microwave, gas stove, dishwasher, refrigerator, washer, and dryer all in good repair and operational. LPA Vanegas observed a two day supply of perishable food, and a seven day supply of non-perishable food along with a sufficient amount of emergency water for residents in care, and staff on duty. LPA Vanegas observed resident bathrooms to be clean and free of debris or mildew. LPA Vanegas observed bathrooms to have required furnishings such as shower chair, slip resistant floor matts, and grab bars. Water faucets and toilets tested operational and water temperature tested between 116.5 and 118.2 degrees. LPA Vanegas toured the outside of the facility and observed a large backyard big enough to participate in outdoor activities upon resident request. There is a shaded sitting area available for resident use. LPA Vanegas observed no obstructions a long emergency exit routes, and side doors are self latching with no locks on them. CONTINUED ON LIC809C LPA Vanegas observed all client rooms to consist of all required furnishings such as a bed, clean linens in good repair; meaning no strains or tears, a chest drawer, enough storage space for personal belongings, and a chair available at client request. LPA Vanegas observed all required postings at entry way of the facility however the PUB475 sign was not the correct dimensions. A technical Violation was issued and Administrator ordered a PUB475 sign with the correct dimensions. LPA Vanegas observed all fire extinguishers to be fully charged and up to date. LPA Vanegas observed all smoke and carbon monoxide detectors to be in good repair and operational. LPA Vanegas observed first aid kit to have all required items such as a thermometer, tweezers, scissors, bandages, adhesive tape, and a first aid manual. LPA Vanegas observed all toxins, sharps, and medications to be locked away and inaccessible to residents in care. LPA Vanegas reviewed medications with Administrator (AD) Daniel Datcu and observed all medications to be documented correctly and per LPA Vanegas review all medications are being administered per physicians orders. LPA Vanegas reviewed three staff files, and six resident files, all files (staff and resident) contained all required documents and staff training is up to date and documented accordingly. Based on observations made during today's inspection no deficiencies will be issued per tittle 22 chapter 8 division 6 of the California Code of Regulations. On 8/14/2024, Licensing Program Analyst (LPA) Janette Romero conducted an unannounced visit to the facility for a required annual inspection. LPA was greeted and granted entry by Administrator, Daniel Datcu who was informed of the purpose of LPA's visit. The facility has a fire clearance for six (6) non-ambulatory elderly residents and an approved hospice waiver for four (4). LPA was informed four (4) residents are currently receiving hospice services at the facility. During today's visit, there was five (5) residents and two (2) care staff present. LPA toured the facility with Administrator Datcu and observed the facility is made up of a one-story home with six (6) resident bedrooms, three (3) bathrooms, one (1) staff room, a kitchen, dining room, living room, and attached garage. During the tour, Administrator Datcu tested one (1) of the smoke alarms/carbon monoxide detectors and LPA observed it to be operational. LPA also observed charged fire extinguishers mounted throughout the facility, last serviced on 7/18/2024. Indoor and outdoor passageways were free of obstruction. The facility has outdoor shaded seating for the residents in care. There were no bodies of water observed on the premises. Medications are secured in a locked kitchen cabinet. Resident bedrooms had the required furniture and lighting. Bathrooms had grab bars near the toilet and in the showers. LPA toured the kitchen and observed the facility had more than a 2-day supply of perishable foods and 7-day supply of non-perishable food items. Resident files reviewed had signed admission agreements and residents with a dementia diagnosis had updated physician reports. Staff present have a criminal record clearance and valid first aid/CPR certification. A record review revealed the facility's last fire drill was conducted on 7/20/2024 and earthquake drill on 6/19/2024. LPA toured the garage and observed emergency food, two refrigerators with additional perishable foods, non-perishable food items, incontinent supplies, along with an operable laundry service. During today's visit, LPA did not observe any issues or concerns.
