Licensing Program Analyst (LPA) Michael Tea conducted an unannounced annual inspection of the facility. Upon arrival, LPA was greeted by caregiver staff and granted entry. Administrator (AD) Geraldine Doan arrived shortly thereafter and assisted with the inspection. The facility is licensed to serve six non-ambulatory residents, with an approved hospice waiver for all six residents and allowance for one bedridden resident in Bedroom #1. At the time of the visit, the facility was operating at full capacity with six residents, three of whom were receiving hospice care. During the visit, LPA Tea toured the facility alongside staff and the Administrator. The home consists of five resident bedrooms, including one shared room, two staff rooms, two and a half bathrooms, a living room, family room, dining area, kitchen, and an attached garage. Throughout the inspection, the facility was observed to be clean, organized, and free of hazards. Pathways and exits were clear and unobstructed. Safety features were checked and found to be in good working condition. Smoke detectors and carbon monoxide detectors were operational throughout the home, and fire extinguishers were fully charged. Documentation showed that the last fire drill was conducted on January 13, 2026. Resident bedrooms were adequately furnished with appropriate beds, clean linens, and sufficient storage space to meet residents’ needs. Bathrooms were clean and well maintained, with functioning toilets and faucets, secure grab bars, and showers free of mold or mildew. Water temperature was measured between 107.1 and 111.4 degrees Fahrenheit, which is within the required range. Towels, toiletries, and personal (Annual Inspection continued on LIC809C) hygiene supplies were readily available. The kitchen was inspected and found to be clean and fully operational. The facility maintained an adequate supply of both perishable and non-perishable food. Emergency food and water supplies were also observed and stored in a locked closet near the kitchen. Sharps were secured in a locked drawer, and cleaning supplies and other toxic substances were properly locked and kept inaccessible to residents. The first aid kit was complete and contained all required items. The outdoor area was also inspected and found to be safe and accessible. There was ample shaded seating available for residents, and the exit gate on the side of the home was self-latching and functioning properly. An unused pool in the backyard was secured with a locked gate. The facility provides activities based on residents’ preferences. During the visit, LPA observed residents participating in light exercise. Staff indicated that residents also enjoy reading and engaging in other activities of their choosing. LPA reviewed six resident files and two staff files. All files were found to contain the required documentation. The Administrator’s certificate is current and valid through September 27, 2026. Medication storage and administration practices were also reviewed. Medications were securely stored in a locked cabinet near the kitchen and were being administered in accordance with physician orders. LPA conducted interviews with residents and staff during the visit. Residents indicated they were satisfied with the care they receive, and staff demonstrated knowledge of their duties and responsibilities. Based on observations made and records reviewed during today’s visit, no deficiencies were cited in the areas inspected in accordance with Title 22, Division 6 of the California Code of Regulations. An exit interview was conducted with Administrator Geraldine Doan. Copies of LIC 809, LIC 809-C, LIC 858, and LIC 859 were reviewed and provided to the facility. Licensing Program Analyst (LPA) Michael Tea conducted an unannounced visit. The purpose of today’s visit was to conduct the Annual Required inspection. At around 1:38 PM, LPA Tea was greeted and granted entry into the facility by the caregiver staff. Administrator (AD) Geraldine Doan arrived shortly to assist with the visit. Facility is licensed for 6 non-ambulatory residents, with a hospice waiver for six and one maybe bedridden in Bedroom #1. Currently there are four residents, and two are on hospice during today's visit. At 1:48 PM, LPA Tea reviewed four resident files and two staff files. Residents’ files and staff files contained all required documentation. Administrator certificates expire on September 27, 2026. LPA Tea along with AD Doan toured the facility at around 2:30 PM. LPA toured the physical plant, checked food service, and the first aid kit. The home consists of 5 resident bedrooms, which one room is shared, 2 staff room, 2.5 bathrooms, a living room, a family room, a dining room, kitchen and attached garage. LPA observed smoke detectors/carbon monoxide in common areas and bedrooms are operational. Resident bedrooms had the required furniture, bed linens and closet/drawer space to accommodate each resident comfortably. Resident bathrooms were checked. Toilets and water faucets worked properly, grab bars were secure and shower was free of mold/mildew. Water temperature measured between around 109.4 Fahrenheit degrees to 113.1 Fahrenheit degrees. Resident bath towels, toiletries and personal hygiene supplies were adequately stocked at time of visit. Common areas were clean and clear of hazards, doorways were free of obstructions. First aid kit had all the required elements including dressing, bandages, tweezers, thermometer, and scissors. Kitchen was inspected. Perishable and non-perishable food supply was checked and adequately stocked at time of visit. LPA observed sharps secured in a locked drawer. LPA also observed toxin substances to be secured and locked and inaccessible to clients underneath the kitchen sink Annual Inspection continued on LIC809C and garage. The fire extinguishers through out the facility are fully charged. The facility’s last fire drill was conducted on February 12, 2025. Kitchen appliances are operational during today's visit. LPA observed emergency food and water supply in a locked closet by the kitchen. LPA toured the outside grounds and there is ample seating with shade and an exit gate on the left side of the house that is self-latching and operational. In the backyard there is a unused small pool that is secured by a locked gate. There is also a barbecue grill that is often used in the summer. Facility provides activities based on resident's personal preference. The residents like to read and do exercises. At the time of the visit, LPA observed residents reading and doing some puzzles.. At 3:00 PM LPA reviewed medication storage and administration. Medications are stored in a locked cabinet by the kitchen. Medications are being administered per physician order. LPA interviewed clients regarding their quality of care and spoke to staff present regarding care provided. Based on the observation made during today’s visit, no deficiencies were noted today in the areas inspected per Title 22 Division 6 of the California Code of Regulations. Licensing Program Analyst (LPA) Ruth Martinez is conducting this unannounced visit for the purpose of completing an annual required inspection. LPA arrived at the facility and was greeted and granted entry by staff and LPA explained the nature of the visit. Geraldine Lasala, Administrator arrived shortly after and met with LPA. There are five residents at the facility and there is one resident receiving hospice services currently. LPA accompanied began the tour of the inside and outside of the facility. LPA observed required department postings throughout the facility. Facility stays within the capacity limitations. There is a minimum of one week of non-perishables foods and two days of perishables foods available. There is additional food storage in storage in a spare refrigerator located in the garage storage. The facility is maintained at a comfortable temperature. LPA inspected that medication is centrally stored in a safe locked storage cabinet located in the dining room. LPA reviewed medication and observed medication was labeled and stored inaccessible to residents in care. LPA inspected the bathroom. LPA measured the hot water temperature which measured 112.8 Fahrenheit degrees. All bathrooms observed to have a supply of soap, toilet paper and towels. Bathrooms are equipped with required safety measures such as non-skid mats and grab bars. Lighting is sufficient to ensure safety and comfort. The facility is equipped with sufficient hand hygiene, cleaning, and disinfecting supplies. LPA observed that toxic chemicals, cleaning solutions and disinfectants are stored locked underneath kitchen sink and locked storage cabinet in the garage. The facility has an available clean supply of linens. LPA inspected residents’ bedrooms which has sufficient lighting to ensure the safety and comfort. All bedrooms observed to have all required components. Storage space is provided for residents in their bedroom. Smoke detectors were tested and found to be operational. LPA observed a fire extinguisher with service date August 23, 2023, in the kitchen and hallway. Fire drills are conducted every three months. Continued on LIC809-C LPA toured the outside of the facility and observed outdoor passageways are free of obstructions. LPA observed there are shaded seating areas for residents’ enjoyment. There is a large spa with very little water that is fenced and locked. LPA observed the spa fence has a self-latching entry door which opens away from the spa. The fence has a key lock at the gate door for inaccessibility. LPA measured the fence which measured 5ft from base of the floor to the top of the fence and it was observed to enclose the entire spa area. LPA began review of records. LPA reviewed five resident records. All the required documentation was present and current in the residents’ files reviewed. LPA reviewed two employee records. All employees present have a criminal record clearance and are associated to the facility. LPA observed records reviewed have a current First Aid certificate. Based on the observations made during today’s visit, no deficiencies were noted today in the areas inspected per Title 22 Division 6 of the California Code of Regulations.
