Licensing Program Analyst (LPA) Marisol Cuadra arrived unannounced to the facility to conduct a case management visit and met with administrator Juanita McDaniel. The purpose of this case management visit is to follow up on a self-incident report submitted to CCL on 09/11/2026 involving client (C1). Per incident report, on 9/6/26, staff contacted the administrator about C1’s medication received on 9/5/26 were missing two medications Olanzapine 2.5mg and Divalproez Sodium 500mg. Administrator contacted the pharmacy vendor, but they were closed due to long weekend holiday, there were no physicians available to approve the refills of the medications, and there were no records of C1’s medications since it was an outside vendor from doctor's office. According to administrator, they had to take C1 to the emergency room to get the medication issue resolved and C1 got discharged with the medications needed without any adverse reactions. On 9/10/26, a different medication Venlafaxine 37.5mg was not received, when the administrator called the psychiatrist office, they learned that the medication records indicated that the medication was discontinued as of 5/8/25, but it wasn’t the case because C1 has been taking the medication as usual. After receiving clarification of medication is still prescribed and not discontinued, the pharmacy released five pills to hold them over due to missed dosages on 9/6 through 9/9/26. During today’s visit, LPA confirmed that the pharmacy gave C1 medication refills for the entire month. Also, administrator's plan to prevent any further incidents includes having a discussion with the pharmacy that if there is no reply from the doctor's office on their first attempt to obtain approval for medication refills, they need to contact the administrator for them to follow up with clients' physicians. An office meeting was conducted today in the Santa Rosa Regional Office. Present in the meeting were: Licensing Program Manager (LPM) Victoria Bertozzi, Licensing Program Analysts (LPAs) Dina Alviso, Robert Frank, Marisol Cuadra, Licensee Sarah Lawrence, facility representatives Megan Lawrence-Ferreira, Lamon Lawrence and Jacqueline Lawrence via phone. The purpose of this office meeting is to have a conversation regarding the possibility of development a change of ownership plan for this facility and the Licensee's other facilities Sarah's Adult Residential Care #496800668, Sarah's Bancroft Drive Adult Home #496800869, Sarah’s Country Home #490108916, Sarah's Dutton Avenue Manor #496801432, Sarah's Group Home for Young Adults #490107871, Sarah's Retirement Home for DD Seniors #496803001, and Sarah's Young Adult Home #496801313 in which Sarah Lawrence is the identified Licensee. Currently, there are no other areas of concern regarding care and supervision provided to residents in care. However, The Department learned about licensee's health incidents that could result in the licensee not been able to operate the facilities in the event of their absence. Based on records review, LIC309 Administrative Organization form indicates that Sarah Lawrence is the licensee for this facility and other facilities above mentioned where it is listed as a sole proprietorship. During today’s meeting it was discussed with licensee the possible option that they can choose to continue the operation of all the facilities. The current option discussed is the following: -Submit a change of ownership application to the Centralized Application Bureau (CAB) for each of the facilities indicating the percentage of ownership of each member. CAB contact information was given. -Provide an updated LIC500 reflecting who will be the designated administrator responsible for the management of the facility including sufficient number of hours to permit adequate attention to the management and administration of the facility by not later than 10/02/2026. Allegation investigated: -Staff did not follow proper reporting requirements.

Licensing Program Analyst (LPA) Cuadra conducted an unannounced visit and met with Back up Administrator, Juanita McDaniel. Licensee, Sarah Lawrence arrived later. The purpose is to investigate complaint allegation listed above and delivered findings. The Department received an allegation of staff did not follow proper reporting requirements. Per Reporting Party, on 2/19/26 there was a suspected abuse incident report filed involving client (C1) where the facility staff requested an earlier medical evaluation for C1 and notified C1's psychiatrist. During today's visit, LPA conducted interviews with Licensee and Back up Administrator confirmed that placement agency and Community Care Licensing were not notified about the incident because they were under the impression that since the suspected incident didn't happen at the facility, they did not need to report it to pertinent agencies. Based on records review, staff provided LPA with incident report dated 3/6/26 about suspected incident that happened at a different location. However, facility staff did not follow reporting requirements and did not report it to pertinent agencies. The suspected abuse is being investigated under complaint #21-AS-20260313091722. The preponderance of evidence standard has been met; therefore, the above allegation is found to be SUBSTANTIATED. Health and Safety Code is being cited on the attached LIC 9099D. Licensing Program Analyst (LPA), Cuadra arrived unannounced to conduct an Annual Required Inspection and met with Juanita McDaniel (Back up Administrator) and Sarah Lawrence Licensee arrived later. Clients were at day program at the time of visit. Contact information reviewed. Required postings were observed. LPA/Licensee initiated a tour of the facility at 12:30 pm and observed the following: Facility was a comfortable temperature and pathways were free from obstructions. Fire extinguishers were last inspected January 8, 2025. Smoke detectors and the carbon monoxide detector were tested and operational during inspection. Last Disaster drill was conducted on September 8, 2025. P&I and cash resources were reviewed. Resident bathroom has required bath mat and grab bars. Water temperature in resident bathroom measured at 110.1, 112.3 and 114.5 degrees F which is within the range allowed by regulation of 105 to 120 degrees F. Toxins are located under the sink in the kitchen, garage and bathroom and all were locked. At least two days of perishable and seven days of non-perishable foods were available. LPA initiated file review at 1:00 pm. Four resident files including medical assessment and care plans. Three staff files were reviewed. Staff have required CPR/1st aid certificates. Licensing Program Analysts (LPAs), Cuadra and Frank, arrived unannounced to conduct an Annual Required Inspection and met with Administrator, Sarah Araya. Clients were at day program at the time of visit. Annual fees current. Contact information reviewed. Required postings were observed. LPA/Administrator initiated a tour of the facility at 2:00 pm and observed the following: Facility was a comfortable temperature and pathways were free from obstructions. However, during the tour of the facility there were some trip hazards observed in the backyard. LPAs had a conversation with Administrator regarding the importance to keep area of potential hazard for residents with poor balance or eyesight challenges well maintained. Fire extinguishers were last inspected January 8, 2025. Smoke detectors and the carbon monoxide detector were tested and operational during inspection. Last Disaster drill was conducted on October 18, 2024. P&I and cash resources were reviewed. First aid kit was missing the tweezers (technical violation issued) Resident bathroom has required bath mat and grab bars. Water temperature in resident bathroom measured at 113.5, 111.8 and 115.6 degrees F which is within the range allowed by regulation of 105 to 120 degrees F. Toxins are located under the sink in the kitchen, garage and bathroom and all were locked. At least two days of perishable and seven days of non-perishable foods were available. At approximate at 2:15pm, during the tour of the facility, LPA/Administrator observed that medications were centrally stored and locked, but the key is kept hanging on the wall accessible to residents in care. Administrator immediately removed the key and will get a lock box. At approximate at 2:20pm, A spot check of medications revealed a loose white tablet "Atorvastatin" was found in the container of resident's (R1) medication. Administrator will review medication and their records and will conduct a staff training. LPAs initiated file review at 2:30 pm. Four resident files including medical assessment and care plans. Two staff files were reviewed. Staff have required CPR/1st aid certificates. Training records were reviewed. Administrator Certificate for Sarah Araya, 6010373740 expires on 4/10/2025. Administrator agreed to submit updates of forms by 1/17/2025: Designation of Administrative Responsibility (LIC308), Personnel Report (LIC500), Liability Insurance and Surety Bond.
