Type of visit: Case Management - Deficiencies

On 8/20/26, Licensing Program Analyst (LPA) J. Leffall conducted a case management visit to follow up and confirm details of an incident report that was received by the Department. The incident occurred on 8/16/26, in which R1 was given medication that is not prescribed to said resident. LPA met with Administrator Brandon Ayala-Montelongo. S1 has received a demotion of the Caregiver position. S1 will no longer perform care giving duties in Assisted Living. S1 has transitioned duties in Memory Care and is no longer permitted to distribute medication. A citation and civil penalty is being issued on the attached 809-D and 421FC regarding medications error. The incident occurred on 5/30/226, in which R1 was given medication at the wrong scheduled time. LPA met with Administrator Brandon Ayala-Montelongo. S1 has received disciplinary action including termination of employment. A citation and civil penalty and Appeal Rights are being issued on the attached 809-D and 421IM regarding medications error.
