Type of visit: Required - 1 Year
Licensing Program Analysts (LPA's) Sarah Hurt and Daiquiri Boyd, conducted an unannounced visit today for the facility’s annual inspection. LPA met with Administrator, Desiree Valero, Continual Administrator's Certification expires 02/26/2028. There are currently 253 residents who reside at this home and there is 5 residents on hospice at this time. LPA inspected the interior and the exterior of the facility including the common living spaces, resident bedrooms and bathrooms, activity rooms, medication storage, kitchen, and outdoor areas. Bedrooms were clean and in good repair. There is a locked storage for medications. Food supply is adequate for 2-day perishable and 7-day nonperishable. Fire extinguisher is within the safety regulation period. Smoke alarms were tested and are operational. The home has a carbon monoxide detector and performs disaster drills as required. Water temperature was tested between 117 and 118 degrees in all 7 rooms tested. First Aid kit is on site and complete. Toxins and cleaning supplies are locked and inaccessible. LPA's reviewed 10 staff files, and 10 resident files (including 2 hospice binders), Plan of Operation, Emergency Disaster Plan, and Infection Control Plan. There were no deficiencies observed or cited during today's inspection per California Code of Regulations, Title 22. LPA requested the following documents: LIC 500 Personnel Report, LIC 308 Designation of Administrative Responsibility, LIC 610-E the Emergency Disaster Plan and copy of current Administrator’s Certificate to update the facility file. Allegation investigated: Staff did not ensure the plumbing was not is disrepair Staff did not ensure the elevator was not in disrepair Staff are not providing adequate food service to residents Staff did not ensure that the facility had electricity Staff did not ensure that the facility telephone was working
Licensing Program Analyst (LPA) Sarah Hurt conducted an unannounced facility visit to deliver findings on the allegations listed above. LPA met with facility Administrator Desiree Valero, and explained the purpose of today's visit. Regarding the allegation staff did not ensure the plumbing was not in disrepair. The investigation revealed that the facility experienced an unexpected plumbing failure in the dishwashing drainage system. The facility immediately hired licensed plumbers to assess and repair the issue. During the repair process, the contracted plumbers hired a jackhammer operator who unintentionally struck additional pipes and electrical lines, resulting in an extended outage. This secondary damage was due to contractor error and not a lack of maintenance by the facility. The facility provided documentation of all repair efforts and timelines. Based on the information obtained during the investigation, there is insufficient evidence to demonstrate that the facility failed to maintain adequate plumbing. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is unsubstantiated. Continued.. Unsubstantiated Regarding the allegation staff did not ensure the elevator was not in disrepair: Interviews and records confirmed one elevator was temporarily out of service; however, two additional elevators remained fully operational, and no residents were unable to access their rooms or common areas. Residents reported only inconvenience. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is unsubstantiated. Regarding the allegation staff are not providing adequate food service to residents: Although the kitchen was affected, the facility rented a mobile kitchen, and mobile food storage, continued meal service, and delivered food and beverages to all residents. Residents confirmed receiving meals, and the county health inspector reported the facility acted appropriately. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is unsubstantiated. Regarding the allegation staff did not ensure that the facility had electricity: The facility experienced an unplanned outage on 9/4/25; power was restored the same evening after immediate contact with PG&E and emergency electricians. Staff conducted frequent in-person safety checks, and residents reported feeling safe. LPA reviewed facilities Emergency disaster plan. LPA reviewed records documenting staff is trained on the facilities Emergency Disaster Plan during initial orientation. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is unsubstantiated. Regarding the allegation staff did not ensure that the facility telephone was working: Internet and phone service went down temporarily but Staff 1 immediately restored service via a temporary ethernet solution the same day. Staff used cell phones for communication as needed, and residents’ safety was not impacted. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is unsubstantiated. Oct 30, 2025Facility evaluation reportReport on file
Type of visit: Case Management - Other
Licensing Program Analyst (LPA) Sarah Hurt arrived unannounced to conduct a Case Management visit on 02/11/2025. LPA met with facility Administrator Desiree Valerio, and stated the purpose of the visit. LPA received an Incident report on 10/30/2025 documenting during the early morning hours of October 25, 2025, approximately 5:00am, the building's fire alarm system was activated. The system responded as designed, immediately alerting staff to the situation, The Fire Department arrived promptly, assessed the area, confirmed there was no active fire, and assisted in ensuring resident and building safety. It was determined that a sprinkler head had been triggered, which resulted in water discharge in several areas of the community. Staff responded to contain the affected area and begin cleanup procedures. The facilities Health and Wellness team conducted resident safety checks and assisted any residents impacted by the situation. A few residents whose apartments were more directly affected by the water intrusion will be temporarily relocated for a few days while repairs and drying are completed. Families of those residents temporarily relocated have been notified. The facilities management and maintenance teams are coordinating with outside vendors who are on-site performing necessary repairs and water mitigation. Fans and drying equipment have been placed in the affected areas to ensure proper drying and to prevent secondary damage. At this time no resident injuries were reported, and all residents remain safe. LPA informed Administrator Desiree Valerio any incident reports must be sent to State Licensing's fax (559)243-8088, mailed to the Regional office (1314 E. Shaw Avenue, Fresno, CA, 93710, or hand delivered the Fresno regional office, and not sent to LPA's direct email boxes. Allegation investigated: Licensee is not ensuring that the facility has hot water.
On 5/22/25 Licensing Program Analyst (LPA) M. Garza arrived to complete an unannounced complaint visit. LPA met with General Manager/Administrator, Desiree Valero, explained reason for visit and was permitted entry into the facility. LPA toured the facility and completed a health and safety check on residents in care. Residents observed in common areas, in rooms, dining area and outside. During investigation LPA completed interviews, gathered and reviewed documentation (resident roster with room numbers, staff roster, staff schedule, maintence logs, tested water temperatures, maintence repair receipts, water logs for the last 2 months). Interviews conducted show the facility has been having issues with water temperatures for approximately 2 months. Repairs to a main pipe, recirculation pump and cartridges have been completed. The facility is still pending additional work to be completed. Water temperature tested in rooms (W132, W326, W328, W330) tested below the required 105 degrees F. The preponderance of evidence standard has been meet per Title 22. 87303 Maintenance and Operation (e) Water supplies and plumbing fixtures shall be maintained as follows: (2) Faucets used by residents for personal care such as shaving and grooming shall deliver hot water. Hot water temperature controls shall be maintained to automatically regulate the temperature of hot water used by residents to attain a temperature of not less than 105 degree F (41 degree C) and not more than 120 degree F (49 degree C). This requirement was not met as evidcene by: LPA observation and interviews completed, the licensee did not comply with the section cited above in that water temperatrures tested in rooms (W132, W326, W328, W330) tested below the required 105 degrees F to 120 degrees F. Type of visit: Case Management - Annual Continuation
On 02/06/25, Licensing Program Analyst (LPA) K. Kaur conducted a required unannounced Annual Inspection continuation visit. LPA introduced herself, stated purpose of visit, and met with Administrator Desiree Valero. A tour of the kitchen and dinning room was conducted. Resident's records contained signed Admission Agreements, Personal Rights, and ID Documentation. 1 out of 6 residents did not have a current Physician's Report and documentation of TB clearance. Medication audit was completed and met regulation requirements. Staff files did not contain annual continuation training. Deficiencies are being cited on the attached 809D in accordance with California Code of Regulations, Title 22, Division 6. Type of visit: Required - 1 Year
On 02/02/25, Licensing Program Analyst (LPA) K. Kaur conducted a required unannounced Annual Inspection visit. LPA introduced herself, stated purpose of visit, and met with Administrator Desiree Valero. LPA toured the facility inside and out to include entry, dining area, common areas on all 3 floors, sample of bedrooms, bathrooms, and exterior. LPA observed License, Fire clearance, and personal rights posted in the facility. LPA did not observe Residential Care Facility for the Elderly RCFE Complaint Poster (PUB 475) posted in the main entryway of the facility. Facility has individual apartments for residents with individual bathrooms. Residents do not share bedrooms except couples. LPA observed resident rooms with chemicals, disinfectants, cleaning solutions, knives and sharp objects. All fire exit routes were clear and free from obstructions. LPA observed facility to be clean, clutter free, and odor free. Fire extinguishers were last serviced 8/12/2024 and are in good standing. Sample of staff files were reviewed. Staff files had health screenings/ TB Clearance. It was verified that current staff on duty are CPR certified. Due to time constraints, LPA will return later for an annual continuation. Deficiencies are being cited on the attached 809D in accordance with California Code of Regulations, Title 22,Division 6. LPA is requesting the following documents be submitted to the Fresno CCL office by 2/11/2025: Current copy of Administrator Certificate, Designation of Facility Responsibility (LIC308), Administrator Organization (LIC 309), Affidavit regarding Client/Resident Cash Resources (LIC 400), Emergency and Disaster Plan, Personnel Report (LIC500), Register of Facility Clients/Residents LIC9020. Type of visit: Case Management - Other
On 9/10/2024 Licensing Program Analyst (LPA) B. Miranda arrived at the facility unannounced to conduct a case management visit. LPA met with Administrator Desiree Valero. LPA reviewed R1 and R2's charts. R1 was a resident at the facility from 5/30/24 and passed on 7/17/2024. R1 did not have any major medical issues listed on physician report. R1 did not have previous complaints regarding medical concerns. R1 was found by housekeeping in their apartment. Administrator was not informed of cause of death. R2 was a resident at the facility since 4/1/2024 and was sent to the hospital on 5/2/2024. R2's physician report listed diagnoses of kidney disease and passed at the hospital from septic shock. Neither resident had prior incidents recorded at the facility before their passing. LPA spoke with staff who did not have additional information to provide. No deficiencies were noted at this time and no citations were issued. Type of visit: Required - 1 Year
On 03/11/24, Licensing Program Analyst (LPA) B. Miranda conducted a required unannounced Annual Inspection visit. LPA introduced herself, stated purpose of visit, and met with Administrator Desiree Valero. LPA toured the facility inside and out to include entry, kitchen, dining area, common areas on all 3 floors, sample of bedrooms, bathrooms, and exterior. All fire exit routes were clear and free from obstructions. LPA observed facility to be clean, clutter free, and odor free. Medications are stored in med rooms which are inaccessible to residents. Toxins, cleaning supplies, knives and sharp objects are secured. Facility has individual apartments for residents with individual bathrooms. Residents do not share bedrooms except couples. Facility has a capacity of 270 and currently has a census of 251. Fire extinguishers were last serviced 8/14/23 and are in good standing. Smoke detectors were last service 1/2024 Verification of carbon monoxide detectors will be provided to LPA by 3/25/24. Water temperature was checked in a common bathroom and read at 120 degree Fahrenheit. LPA toured the kitchen at the facility. LPA observed some cabinets in the entry way of the kitchen needing to be cleaned, freezer holding ice cream needs to be cleaned if no longer using, and defrosting food items need to be properly covered. Citation was issued per the California Code of Regulations Tittle 22. Sample of resident files were reviewed. Liability insurance policy is current. Sample of staff files were reviewed and some files did not reflect proper training to be current. Citation was issued per the California Code of Regulations Tittle 22. On 12/01/23 at 1:21 p.m. Licensing Program Analyst (LPA) B. Miranda arrived to the facility unannounced to deliver the finding for the allegations listed above. LPA introduced herself and explained the reason for the visit. Administrator (AD) Desire Valero was contacted. 1. The Department investigated the allegation: Facility does not ensure sufficient staffing is on the premises for overnight supervision. LPA interviewed various sources. Some interviewees stated there was enough staff and others stated there was not enough. Staff schedules were reviewed for June & July 2023, there did not appear to be staff shortage according to interviews and the schedules at this time. Unsubstantiated 2. The Department investigated the allegation: Staff yell at residents in care. LPA interviewed various sources, none of the interviewees stated they have witnessed or heard staff yelling at residents in care. 3. The Department investigated the allegation: Staff do not ensure residents are provided bathing assistance in a timely manner. LPA interviewed various sources, some sources stated bath times are pushed back or not completed during the assigned shift due to time constraints. Opportunities are given to reschedule bathes. After observation, conducting interviews, and record reviews the following was found for all three allegations. Although the allegation(s) may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation(s) did or did not occur, therefore the allegation(s) are unsubstantiated. Allegation investigated: Facility is not storing an adequate amount of food at the facility. Staff are denying food to residents. Staff are serving food that is not of quality. Facility is not following resident's care plan. Facility is not delivering water for residents in care. Facility staff threatened residents with an eviction. Residents are unable to properly maneuver throughout the facility.
On 12/01/23 at 1:21 p.m. Licensing Program Analyst B. Miranda arrived to the facility unannounced to deliver the finding for the allegations listed above. LPA introduced herself and explained the reason for the visit. Administrator (AD) Desire Valero was contacted. 1. The Department investigated the allegation: Facility is not storing an adequate amount of food at the facility. LPA interviewed various sources. Some interviewees stated the portions were small however when they requested more food it was given without an issue. 2. The Department investigated the allegation: Staff are denying food to residents. LPA interviewed various sources, none of the interviewees’ stated food has ever been denied to residents. Unsubstantiated 3. The Department investigated the allegation: Staff are serving food that is not of quality. LPA interviewed various sources. Some interviewees stated there was boxed food for a short period of time due to facility having emergency repairs completed. 4. The Department investigated the allegation: Facility is not following resident's care plan. LPA interviewed various sources, none of the interviewees stated they have witnessed or heard of resident’s care plans not being followed. 5. The Department investigated the allegation: Facility is not delivering water for residents in care. LPA interviewed various sources, none of the interviewees stated they have witnessed or heard of residents not being given water during repairs. 6. The Department investigated the allegation: Facility staff threatened residents with an eviction. LPA interviewed various sources, none of the interviewees stated they have witnessed or heard of residents being threatened with an eviction. 7. The Department investigated the allegation: Residents are unable to properly maneuver throughout the facility. LPA interviewed various sources, some interviewees stated it was difficult to maneuver into the public bathrooms, but not the facility in general. After observation, conducting interviews, and record reviews the following was found for all seven allegations. Although the allegation(s) may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation(s) did or did not occur, therefore the allegation(s) are unsubstantiated.
