On 1/7/26 Licensing Program Analyst (LPA) J. Leffall arrived unannounced to conduct an Annual Inspection. LPA introduced self, stated the purpose of the visit, and was greeted by Staff (S1) Irene Cayco. LPA was granted entry. All 6 residents were present during inspection. Administrator (A1) Annabelle Hurley arrived shortly after LPA’s arrival. LPA toured facility with A1. The facility was observed to be at a comfortable temperature, clean, in good repair, and no passageway obstructions or fire hazards were observed inside. An adequate supply of perishable and non-perishable food was observed. Freezer temperature was maintained at 0 degrees F and refrigerator temperature was maintained at 40 degrees F. Fire extinguisher was observed with a purchase date of: 8/15/25. Fire drill last completed on 1/5/26. LPA observed 1 sharp object in unlocked drawer accessible to residents. Washer and dryer observed operational during visit. Carbon monoxide and smoke detectors were tested and observed to be operational. Residents' bedrooms were toured and observed to be adequately furnished with bed, dresser, and adequate lighting. All bathrooms are toured and observed to be operational. Hot water temperature was tested at a temperature range of 108.4 to 114 degrees F in 6 bathrooms. Non-skid mat and grab bars observed in bathrooms. Outside of facility toured. Side gate was self-closing and self-latching. Outside was observed with adequate outdoor seatings available for residents. Samples of medications were checked and observed kept locked in the hall closet. Residents’ MARS was reviewed. First aide kit observed with all the required items. All residents and samples of staff files reviewed to have all of the required documents. Deficiency is being cited on the attached 809D in accordance with California Code of Regulations, Title 22, Division 6. Exit Interview conducted. The following documents requested to be updated and submitted to Fresno CCL by 1/21/26: Lic 308, Lic 500, Lic 610E, Current Liability Insurance and current Administrator’s certificate. Licensee (L1) and Administrator (A1) Jack and Annabelle Hurley arrived shortly after LPA’s arrival. LPA toured facility with L1. The facility was observed to be at a comfortable temperature, clean, in good repair, and no passageway obstructions or fire hazards were observed inside. An adequate supply of perishable and non-perishable food was observed. Freezer temperature was maintained at 0 degrees F and refrigerator temperature was maintained at 40 degrees F. Fire extinguisher was observed with a purchase date of: 8/15/24. Fire drill last completed on 12/23/24. Washer and dryer observed operational during visit. Carbon monoxide and smoke detectors were tested and observed to be operational. Residents' bedrooms were toured and observed to be adequately furnished with bed, dresser, and adequate lighting. All bathrooms are toured and observed to be operational. Hot water temperature was tested at a temperature of 107.4 in bathroom 1, 109.7 in bathroom 2, 109.5 in bathroom 3, 111.0 in bathroom 4, 107.7 in bathroom 5, 106.7 in bathroom 6 and 111.3 degrees in bathroom 7. Non-skid mat and grab bars observed in bathrooms. Outside of facility toured. Side gate was self-closing and self-latching. Outside was observed with adequate outdoor seatings available for residents. Samples of medications were checked and observed kept locked cabinet. Residents’ MARS was reviewed. First aide kit observed with all the required items. All residents and samples of staff files reviewed to have all the required documents. No deficiencies issued during this inspection. On 02/26/24, Licensing Program Analyst (LPA) M. Yang arrived unannounced to conduct an annual visit. introduce self, stated the purpose of the visit and met caregiver John Ramirez. LPA toured facility with caregiver. Administrator Jack Hurley was called. Assistant Administrator (AA) Natalie Gako arrived shortly during tour. LPA our facility with AA. All six residents were present during inspection. The facility was observed to be at a comfortable temperature, clean, in good repair were observed inside or outside. Secure lock above top front door was observed. Medications were observed locked in kitchen shelves. MARs were reviewed. Extra linens were observed. An adequate supply of perishable and non-perishable food was observed. Refrigerator temperature is maintained at 40 degrees F and freezer at 0 degrees F. Fire extinguisher was observed with a service date of: 8/3/24. Cleaning supplies and chemicals stored and locked in garage and laundry room. Washer and dryer observed operational during inspection. Laundry detergent observed unlock in the garage. All bedrooms were observed to have the required furnishings and with adequate lightening. LPA observed in resident bedroom 3 with child locked doorknob. The bathrooms were toured and observed operational during inspection. Non-skid mat and grabbed bars were observed. Hot water temperature was tested at 115.7 in the bathroom 1, 105.4 in room 2 bathroom, 110.2 in room 6 bathroom, and 105.4 in room 1 bathroom. Outside of facility toured and observed to be free of debris. Side gate observed self-closing and self-latching. Adequate outdoor seatings available for residents. Carbon monoxide and smoke detectors were tested and observed to be operational. Half of the residents’ and 3 staff files were reviewed to have all the required documents. A deficiency is being cited on the attached Lic 809D in accordance to California Code of Regulations, Title 22, Division 6. Exit Interview conducted.
